HOW TO APPLY FOR THEATER PERFORMANCES, SEAS-ON-TOUR,<br />

SPECIAL EVENING PERFORMANCES AND SEAS PLUS<br />

Please do not remove original application <strong>for</strong>m found in this brochure.<br />

Photocopy as many application <strong>for</strong>ms to attend per<strong>for</strong>mances and SEAS Plus as needed.<br />

• Complete a separate application <strong>for</strong> each show that you are applying to attend.<br />

• Please refer to <strong>the</strong> bus schedule in<strong>for</strong>mation on pages 53 and 54.<br />

• Print all in<strong>for</strong>mation requested on <strong>for</strong>m clearly.<br />

• Applications by fax or telephone are NOT accepted.<br />

• All applications are processed in <strong>the</strong> order received and acknowledged by mail.<br />

INSERVICE POINTS<br />

To receive prior approval from HRD see <strong>the</strong> Inservice Facilitator at your school to obtain <strong>the</strong><br />

external workshop <strong>for</strong>ms and procedures <strong>for</strong> submitting an Independent Study <strong>for</strong> Inservice credit.<br />

HOW TO APPLY FOR SEAS PLUS<br />

SEAS Plus includes a SEAS per<strong>for</strong>mance at <strong>the</strong> <strong>Broward</strong> <strong>Center</strong> and<br />

a learning experience at a nearby cultural site during <strong>the</strong> same school day.<br />

The ONLY dates available <strong>for</strong> SEAS Plus are as follows:<br />

October 1, 2009 - January 31, 2010<br />

If you are interested in attending, please follow <strong>the</strong> procedures below:<br />

1. Check <strong>the</strong> SEAS Plus box on <strong>the</strong> application <strong>for</strong>m<br />

2. Fill in Title of Per<strong>for</strong>mance<br />

3. Select Cultural Site<br />

4. Complete entire <strong>for</strong>m<br />

5. Pony completed <strong>for</strong>m to:<br />

You will be notified by pony mail as to <strong>the</strong> status of your application.<br />

All funding <strong>for</strong> SEAS Plus (admission and busing) will be paid <strong>for</strong> through <strong>the</strong> Arts & Sciences Fund at your school.<br />
