How to Apply
HOW TO APPLY FOR THEATER PERFORMANCES, SEAS-ON-TOUR,
SPECIAL EVENING PERFORMANCES AND SEAS PLUS
Please do not remove original application form found in this brochure.
Photocopy as many application forms to attend performances and SEAS Plus as needed.
• Complete a separate application for each show that you are applying to attend.
• Please refer to the bus schedule information on pages 53 and 54.
• Print all information requested on form clearly.
• Applications by fax or telephone are NOT accepted.
• All applications are processed in the order received and acknowledged by mail.

INSERVICE POINTS
To receive prior approval from HRD see the Inservice Facilitator at your school to obtain the
external workshop forms and procedures for submitting an Independent Study for Inservice credit.
HOW TO APPLY FOR SEAS PLUS
SEAS Plus includes a SEAS performance at the Broward Center and
a learning experience at a nearby cultural site during the same school day.
The ONLY dates available for SEAS Plus are as follows:
October 1, 2009 - January 31, 2010
If you are interested in attending, please follow the procedures below:
1. Check the SEAS Plus box on the application form
2. Fill in Title of Performance
3. Select Cultural Site
4. Complete entire form
5. Pony completed form to:
Denyse Murguia, SEAS Coordinator, K.C. Wright Bldg., 12th floor.
You will be notified by pony mail as to the status of your application.

All funding for SEAS Plus (admission and busing) will be paid for through the Arts & Sciences Fund at your school.
