101. A reporter claims that a 70 year old woman lives in filth and her home is a
fire hazard. The reporter is concerned about her health.
102. A 73 year old woman misses medical appointments and does not take
medication as directed by her doctor. Her husband appears to have difficulty
assisting his wife with her needs
103. A 50 year old woman is diagnosed with schizophrenia and refuses to
continue mental health services. She instructed that she wishes her case to be
closed. She is delusional, stating that "I belong to a congregation. We are in our
last days". She only came to the clinic for intake and did not follow through with
treatment.
104. An 81 year old man was admitted through the emergency room with injuries
to his forehead and left eye. The victim has severely infected ulcerations on both
legs and sores throughout his body. The victim is malnourished and weighs 87
lbs. He has no family contacts.
105. A 52 year old man is disabled due to liver and hip problems. He is on SSI.
He cannot leave the home to apply for the utility assistance programs. His
electricity bill is overdue and it will be disconnected if not paid.
106. A 64 year old man has diabetes, a history of prostate cancer, is unable to
urinate, and has chest pain. He is non-compliant with medical treatment and
takes no medication. He lives alone in a trailer parked in the back yard of a
property.
107. A 75 year old woman has kidney disease. She was hospitalized three times
in the past three months. Her children are suspected of not administering her
medications.
108. An 81 year old woman is at risk of eviction. She has her mentally ill son
living with her and he's damaging the apartment. He was physically abusive to
her in the past, but now is verbally abusive. She was evicted a few times in the
past because of her son's behavior. She needs help now to find an assisted
living facility so that she can be away from her son.
109. When asked about domestic violence, an 84 year old woman kept giving
"the eye"; batting her eyes and shoving her shoulder as if to say that her son is
hurting her.
110. A 78 year old woman's grandson shook her by the shoulders and grabbed
her arms causing a bruise to her hand. He disconnected the telephone and
verbally threatened her.
111. A 79 year old woman's daughter, who resides with her, continues to
take/steal items from the victim.
112. A 51 year old woman is developmentally disabled. Her lawyer found some
checks were forged on her deceased mother's account. The victim claimed that
she wrote the checks. The service coordinator is concerned.
113. A 67 year old woman is homeless and receives Supplemental Security
Income (SSI). Her daughter had taken her money by using her ATM card. The
victim is at the train station and needs emergency placement.
114. An 81 year old woman has cervical cancer but she refused surgery. She
has a history of suicidal behavior and told the hospital staff that she wanted to kill
herself. She has a part-time caretaker at home.
