101. A reporter claims that a 70 year old woman lives <strong>in</strong> filth and her home is a<br />

fire hazard. The reporter is concerned about her health.<br />

102. A 73 year old woman misses medical appo<strong>in</strong>tments and does not take<br />

medication as directed by her doctor. Her husband appears to have difficulty<br />

assist<strong>in</strong>g his wife with her needs<br />

103. A 50 year old woman is diagnosed with schizophrenia and refuses to<br />

cont<strong>in</strong>ue mental health services. She <strong>in</strong>structed that she wishes her case to be<br />

closed. She is delusional, stat<strong>in</strong>g that "I belong to a congregation. We are <strong>in</strong> our<br />

last days". She only came to <strong>the</strong> cl<strong>in</strong>ic for <strong>in</strong>take and did not follow through with<br />

treatment.<br />

104. An 81 year old man was admitted through <strong>the</strong> emergency room with <strong>in</strong>juries<br />

to his forehead and left eye. The victim has severely <strong>in</strong>fected ulcerations on both<br />

legs and sores throughout his body. The victim is malnourished and weighs 87<br />

lbs. He has no family contacts.<br />

105. A 52 year old man is disabled due to liver and hip problems. He is on SSI.<br />

He cannot leave <strong>the</strong> home to apply for <strong>the</strong> utility assistance programs. His<br />

electricity bill is overdue and it will be disconnected if not paid.<br />

106. A 64 year old man has diabetes, a history <strong>of</strong> prostate cancer, is unable to<br />

ur<strong>in</strong>ate, and has chest pa<strong>in</strong>. He is non-compliant with medical treatment and<br />

takes no medication. He lives alone <strong>in</strong> a trailer parked <strong>in</strong> <strong>the</strong> back yard <strong>of</strong> a<br />

property.<br />

107. A 75 year old woman has kidney disease. She was hospitalized three times<br />

<strong>in</strong> <strong>the</strong> past three months. Her children are suspected <strong>of</strong> not adm<strong>in</strong>ister<strong>in</strong>g her<br />

medications.<br />

108. An 81 year old woman is at risk <strong>of</strong> eviction. She has her mentally ill son<br />

liv<strong>in</strong>g with her and he's damag<strong>in</strong>g <strong>the</strong> apartment. He was physically abusive to<br />

her <strong>in</strong> <strong>the</strong> past, but now is verbally abusive. She was evicted a few times <strong>in</strong> <strong>the</strong><br />

past because <strong>of</strong> her son's behavior. She needs help now to f<strong>in</strong>d an assisted<br />

liv<strong>in</strong>g facility so that she can be away from her son.<br />

109. When asked about domestic violence, an 84 year old woman kept giv<strong>in</strong>g<br />

"<strong>the</strong> eye"; batt<strong>in</strong>g her eyes and shov<strong>in</strong>g her shoulder as if to say that her son is<br />

hurt<strong>in</strong>g her.<br />

110. A 78 year old woman's grandson shook her by <strong>the</strong> shoulders and grabbed<br />

her arms caus<strong>in</strong>g a bruise to her hand. He disconnected <strong>the</strong> telephone and<br />

verbally threatened her.<br />

111. A 79 year old woman's daughter, who resides with her, cont<strong>in</strong>ues to<br />

take/steal items from <strong>the</strong> victim.<br />

112. A 51 year old woman is developmentally disabled. Her lawyer found some<br />

checks were forged on her deceased mo<strong>the</strong>r's account. The victim claimed that<br />

she wrote <strong>the</strong> checks. The service coord<strong>in</strong>ator is concerned.<br />

113. A 67 year old woman is homeless and receives Supplemental Security<br />

Income (SSI). Her daughter had taken her money by us<strong>in</strong>g her ATM card. The<br />

victim is at <strong>the</strong> tra<strong>in</strong> station and needs emergency placement.<br />

114. An 81 year old woman has cervical cancer but she refused surgery. She<br />

has a history <strong>of</strong> suicidal behavior and told <strong>the</strong> hospital staff that she wanted to kill<br />

herself. She has a part-time caretaker at home.<br />
