197. An 89 year old woman was admitted to <strong>the</strong> hospital due to malnutrition. She<br />

refuses convalescent home care. Her son lives at home and works. He is not <strong>the</strong><br />

care provider.<br />

198. A 78 year old diabetic woman claims someone came <strong>in</strong>to her home at night<br />

and stole her credit card and $200 from her purse. The house doors were locked.<br />

She states th<strong>in</strong>gs have turned up miss<strong>in</strong>g on and <strong>of</strong>f for two years.<br />

199. A 78 year old bed bound woman was released from <strong>the</strong> hospital <strong>in</strong> February.<br />

Her daughter and In-Home Supportive <strong>Services</strong> workers were <strong>in</strong>structed to turn<br />

<strong>the</strong> victim every two hours. This is not be<strong>in</strong>g done. The victim was found dirty, <strong>in</strong><br />

a dirty bed, with significant sk<strong>in</strong> break down.<br />

200. A 38 year old woman with severe arthritis <strong>in</strong> her hip and mental health<br />

problems is not be<strong>in</strong>g given her medications appropriately by her In-Home<br />

Supportive <strong>Services</strong> worker. The IHSS worker is not do<strong>in</strong>g o<strong>the</strong>r tasks as well.<br />

201. A 51 year old woman with neuropathy <strong>in</strong> <strong>the</strong> legs, a mental health issue and<br />

diabetes is be<strong>in</strong>g neglected by her In-Home Supportive <strong>Services</strong> care provider.<br />

202. An 85 year old bl<strong>in</strong>d woman's mental state is deteriorat<strong>in</strong>g. She has had six<br />

care providers <strong>in</strong> <strong>the</strong> last five months and has accused all <strong>of</strong> <strong>the</strong>m <strong>of</strong> steal<strong>in</strong>g from<br />

her. The home health agency is term<strong>in</strong>at<strong>in</strong>g services.<br />

203. A 77 year old man with congestive heart failure, dementia and sk<strong>in</strong> breakdown<br />

is non-compliant with tak<strong>in</strong>g his medications and keep<strong>in</strong>g doctor<br />

appo<strong>in</strong>tments. He has no family or friends. There is currently no food <strong>in</strong> <strong>the</strong><br />

home. He is scared and confused.<br />

204. The caregiver is not provid<strong>in</strong>g medication to an 89 year old female with heart<br />

problems, glaucoma, and a colostomy.<br />

205. An 82 year old frail woman's son is tak<strong>in</strong>g her money and neglect<strong>in</strong>g her<br />

care. The woman is very upset by this betrayal.<br />

206. An 83 year old bed bound woman was hospitalized with multiple necrotic<br />

decubitus ulcers on her buttocks and feet from improper care.<br />

207. A 67 year old frail man is depressed and self isolat<strong>in</strong>g. He is not eat<strong>in</strong>g<br />

properly due to loss <strong>of</strong> appetite. He should be us<strong>in</strong>g a walker due to unsteady<br />

gait, but doesn't know how to get one. He needs an advocate.<br />

208. A 65 year old woman was brought to <strong>the</strong> hospital smell<strong>in</strong>g strongly <strong>of</strong> feces<br />

and ur<strong>in</strong>e. She had some bruis<strong>in</strong>g on her forehead and right arm.<br />

209. A 47 year old bra<strong>in</strong> <strong>in</strong>jured man with an <strong>in</strong>fantile mental state was taken<br />

home by his spouse from <strong>the</strong> hospital after <strong>the</strong> accident <strong>in</strong>stead <strong>of</strong> to <strong>the</strong><br />

rehabilitation facility. The spouse is not provid<strong>in</strong>g medications or rehabilitation<br />

treatment. She is not provid<strong>in</strong>g needed assistance <strong>in</strong> toilet<strong>in</strong>g. She is provid<strong>in</strong>g<br />

beer and cigarettes.<br />

210. A 50 year old developmentally disabled man was pushed down and slightly<br />

<strong>in</strong>jured by <strong>the</strong> caregiver's boyfriend.<br />
