Endangered Species<br />

Change or Die The world <strong>of</strong> primary pediatrics<br />

has changed over the past few decades from a<br />

focus on treatment to focus on prevention, to the<br />

separation <strong>of</strong> inpatient from outpatient medicine,<br />

and to the emergence <strong>of</strong> competing health care<br />

disciplines. Changing generational values, the<br />

evolving pediatrics workforce, the explosion <strong>of</strong><br />

medical information and technology, the vagaries<br />

<strong>of</strong> economic reimbursement, the concern for<br />

patient safety and the push for evidence based<br />

medicine and systems-based practice also contribute<br />

to change. As we examine the forces and<br />

factors that have produced this change, we can<br />

determine what it will take to promote ensuring<br />

the primary care physician as the central person<br />

<strong>of</strong> first and consistent point <strong>of</strong> patient contact<br />

versus enabling other “survival <strong>of</strong> the fittest” disciplines<br />

to take over pediatric primary care.<br />

In 2007 the AAP issued a policy statement<br />

titled, “Cord Blood Banking for Potential Future<br />

Transplantation” (<strong>Pediatrics</strong> 2007 119:165-170), but<br />

a recent survey documented that few pediatricians<br />

were aware <strong>of</strong> its content. This session will review<br />

the potential uses for umbilical cord blood and discuss<br />

the recommendations made in the 2007 policy<br />

F3058 Evaluation <strong>of</strong> the Child With a<br />

Possible Bleeding Disorder<br />

Minor bleeding such as epistaxis and easy bruising<br />

are common complaints during childhood. While a<br />

complete blood count, prothrombin time (PT), and<br />

partial thromboplastin time (PTT) are <strong>of</strong>ten used to<br />

screen such children for a bleeding disorder, they are<br />

relatively insensitive to hematologic conditions such<br />

as von Willebrand disease and platelet dysfunction.<br />

The PT and PTT are also commonly performed prior<br />

to surgical procedures, and if abnormal the child is<br />

<strong>of</strong>ten referred for further evaluation. This session will<br />

review the role <strong>of</strong> these and other hemostatic tests<br />

in the evaluation <strong>of</strong> the child with bleeding manifestations<br />

or who is scheduled for a surgical procedure.<br />

H2067 Section on Home Care Program<br />

The AAP Section on Home Care hosted the organization’s<br />

sole Parent Advisory Group, led by nationally<br />

influential parents and family members <strong>of</strong> children<br />

with special health care needs. Faculty members<br />

from the Parent Advisory Group and experts<br />

in family-centered pediatric practice across the<br />

spectrum <strong>of</strong> care will present successful models<br />

<strong>of</strong> family-pediatrician partnerships that innovate,<br />

enhance, and promote improved care through the<br />

medical home model. Attendees to this session<br />

will learn how these partnerships are central to the<br />

care <strong>of</strong> all children beyond the traditional domains<br />

in which they have been applied in the care <strong>of</strong><br />

children with special health care needs and those<br />

who are medically fragile. Such partnerships operate<br />

successfully at the level <strong>of</strong> a single practice,<br />

a system or practices, or a large medical center<br />

and are applicable across the range <strong>of</strong> health care<br />

financing systems. Session participants will learn<br />

how to create, strengthen, and advocate for these<br />

partnerships in their own places <strong>of</strong> work and soon<br />

enjoy the benefits <strong>of</strong> providing improved care that<br />

is more satisfying and more pr<strong>of</strong>itable.<br />
