Unusual Incident Report<br />

In order to provide the Administration with specific and accurate facts regarding incidents, an<br />

Unusual Incident Report has been developed and shall be completed by employees for recording<br />

emergencies or other unusual incidents. Employees shall record all pertinent information for daily<br />

review by the Administrator.<br />

Procedure From Policy 1200.0827: Unusual Incident Report<br />

A. The employee directly observing the unusual incident or who is directly involved, shall notify<br />

the supervisory employee on duty (i.e. Unit Manager), and then complete items 1 through 7 <strong>of</strong><br />

the Unusual Incident Report (Form No. 007) and shall sign and date the form.<br />

B. Unusual Incident Reports shall be completed for, but shall not be limited to, the following<br />

circumstances:<br />

1. Emergencies - Destruction <strong>of</strong> State property, bomb threats, fires, riots, suicides,<br />

attempted suicides, resident or staff sudden illness or death, use <strong>of</strong> physical force,<br />

hostage situations;<br />

2. Resident Violations - Assaults <strong>of</strong> employees or residents, injuries, sexual intimidation,<br />

extortion, property damage, stealing, possession <strong>of</strong> drugs and other serious violations.<br />

3. Other Incidents - Observation <strong>of</strong> intruders, motor vehicle accidents, discovery <strong>of</strong> property<br />

damage and other unusual situations.<br />

C. The supervisory employee shall complete items 8 through 12 <strong>of</strong> the Unusual Incident Report<br />

Form and shall also sign and date the form before submitting the report to the <strong>of</strong>fice <strong>of</strong> the<br />

Administrator at the end <strong>of</strong> the tour <strong>of</strong> duty when the incident has occurred.<br />

D. The Administrator may request additional information from the employee involved due to the<br />

serious nature <strong>of</strong> the incident or as a result <strong>of</strong> further investigation and a deadline will be given<br />

for said additional written information.<br />

E. All reports will be completed in a neat, concise and accurate manner as soon as possible after<br />

the incident and no space is to be left blank.<br />

F. Reports must be dated and signed by all employees completing any part <strong>of</strong> the report.<br />

G. In incidents involving resident violation <strong>of</strong> facility rules, discipline reports are also necessary<br />

and should be completed by employees.<br />
