For purposes <strong>of</strong> sufficient staffing, accountability and a uniformly efficient system, requests for<br />

transfer, leaves <strong>of</strong> absence, retirements and termination’s will be referred to and handled by the<br />

<strong>of</strong>fice <strong>of</strong> the Assistant Director.<br />

Procedure From Policy 1200.0224: Transfer, Request for Leave, Retirement and<br />

Termination<br />

A. All employees requesting a transfer, leave <strong>of</strong> absence, retirement or termination should notify<br />

their superior in writing, accompanied by a Personnel Action Form (Form No. 028) available in<br />

the Administrator's Office.<br />

B. The employee's request should include the reason(s) for the leave and the date(s) involved.<br />

C. The supervisor shall forward all <strong>of</strong> the completed paperwork to the Administrator who shall in<br />

turn notify the Assistant Director <strong>of</strong> the employee's request.<br />

D. Once approved, the Assistant Director shall forward the Personnel Action Form and written<br />

request to the Office <strong>of</strong> Employee Relations for processing.
