In order to ensure that an employee receives financial compensation for sustained injuries related<br />
to his/her tour <strong>of</strong> duty, the following procedure has been developed.<br />

Procedure From Policy 1200.0219: Worker's Compensation Report<br />
A. An employee may obtain a Worker's Compensation Injury Form from either the<br />
Administrator's <strong>of</strong>fice or the Assistant Director's <strong>of</strong>fice and are to be fully completed.<br />
B. Once completed, it shall be submitted to the Assistant Director's <strong>of</strong>fice within forty-eight (48)<br />
hours <strong>of</strong> said injury.<br />
C. The Assistant Director's <strong>of</strong>fice will then forward the form to the Office <strong>of</strong> Employee Relations<br />
for processing.
