Box 7: Glimpse of healthcare facilities in a Tribal village, Andhra Pradesh<br />

The ANM in the tribal village has been working in the area for over 5 years. A tribal<br />

herself, she enjoys a good rapport with the villagers. The village community too respects<br />

her <strong>and</strong> appears happy that she is fairly regular in her visits. The ANM, however, seemed<br />

indifferent to the health problems in the village despite widespread scabies <strong>and</strong> respiratory<br />

problems. These could be a result of the fact that the village is overrun by parthenium (a<br />

wild shrub that is known to trigger allergies). The ANM seemed quite unaware of the<br />

situation. While the government doctor, to whose private practice almost all the tribal<br />

households go (paying an initial consultation fee of Rs.50), reported that there were 5<br />

HIV+ cases in the village, the ANM herself did not seem aware.<br />

This doctor was our best informant on the health situation not only in the tribal village but<br />

in the entire M<strong>and</strong>al. He said that skin diseases were rampant in the area due to very bad<br />

hygienic conditions <strong>and</strong> the lack of sufficient protected drinking water. He was quite<br />

worried about the spread of HIV in the area saying that since this is migratory population<br />

there is danger of higher risk. He was especially concerned about the risk faced by women<br />

<strong>and</strong> newborn children. He was emphatic that a sustained health education programme is<br />

required in this area. He was critical of the way the food for work programme was being<br />

implemented. In the case of the tribal village better sanitation <strong>and</strong> waste management<br />

would be more beneficial than laying a pucca road. Efforts are misplaced <strong>and</strong> designed to<br />

line the pockets of the rich <strong>and</strong> powerful. Generally he felt the government was neglecting<br />

remote <strong>and</strong> tribal areas. The system as he kept repeating was corrupt <strong>and</strong> dysfunctional.<br />

Ironically he did not see that he himself was corrupting the system by engaging in up<br />

private practice, which is against the rule. We met him in his private clinic at a time when<br />

he should have been in the hospital.<br />

− Existing programmes have not been sufficiently focused on improving overall awareness<br />

levels, instruction on supplementary nutrition for pregnant <strong>and</strong> lactating mothers. The IEC<br />

component of ICDS or three generations of maternal <strong>and</strong> child health programmes (MCH/FP,<br />

CSSM, RCH) seem to have made little impact on the overall awareness of the surveyed<br />

community towards nutrition <strong>and</strong> hygiene/sanitation. ANMs concentrate on family welfare<br />

activities, <strong>and</strong> in AP <strong>and</strong> Karnataka on antenatal care <strong>and</strong> immunisation. Even in the case of<br />

immunisation the focus, as mentioned above, is on polio prevention, which in any case is<br />

being delivered as part of a mass campaign.<br />

- Nevertheless, 4 out of 6 rural children <strong>and</strong> 2 out of the 3 urban children below 3 years in<br />

Uttar Pradesh (Muslim <strong>and</strong> Hindu families) have attained age appropriate development<br />

milestones – they were seen crawling, walking, running, communicating their needs, laughing<br />

<strong>and</strong> playing. The sheer joy of being a child <strong>and</strong> running free is visible in their bright eyes <strong>and</strong><br />

shrill voices.<br />
