d) Copy of Service Tax registration certificate.<br />
e) Documents such as 1) Bill of lad<strong>in</strong>g, 2) custom clearance receipt 3) Invoice<br />
and 4) custom bill of entry to verify the ownership of HDD/Blow<strong>in</strong>g<br />
M/C/Splic<strong>in</strong>g mach<strong>in</strong>e of required capacity (<strong>in</strong> case of those who own<br />
HDD/Blow<strong>in</strong>g M/C/Splic<strong>in</strong>g mach<strong>in</strong>e) or lease deed with documents to verify<br />
the ownership of HDD/Blow<strong>in</strong>g M/C/Splic<strong>in</strong>g mach<strong>in</strong>e of required capacity of<br />
the lesser (<strong>in</strong> case of those who have HDD/Blow<strong>in</strong>g M/C/Splic<strong>in</strong>g mach<strong>in</strong>e<br />
on lease), serial number and make of the HDD/Blow<strong>in</strong>g M/C/Splic<strong>in</strong>g<br />
mach<strong>in</strong>e should be clearly mentioned <strong>in</strong> the lease deed. The lease deed<br />
should be valid <strong>for</strong> 240 days from the date of open<strong>in</strong>g of the tender.<br />
f) List of qualified experienced personnel, who are work<strong>in</strong>g <strong>for</strong> the tenderer,<br />
who will be deployed <strong>for</strong> carry<strong>in</strong>g out the work.<br />
g) In case of proprietary firm, proprietor certificate from the proprietor of the<br />
firm that he is the sole proprietor of the firm<br />
h) Authenticated valid copy of partnership deed <strong>in</strong> cases of partnership firm &<br />
<strong>for</strong>m-A issued by Registrar of firms.<br />
i) In case of Limited Company, attested copy of articles of association duly<br />
registered with Registrar of Company affairs.<br />
j) Attested copy of ‘Power of Attorney <strong>in</strong> case person other than the tenderer<br />
has signed the tender document.<br />
k) In case of bidder download<strong>in</strong>g the tender document from Website then he<br />
should enclose DD <strong>for</strong> tender document fee with the tender offer.<br />
l) Self attested photocopy of PAN Card.<br />
m) Bid Form, duly filled <strong>in</strong>, as per Chapter-VIII.<br />
n) Latest Income Tax Return of the firm/ proprietor.<br />
o) Contractor should submit EPF registration of his firm and show evidence of<br />
EPF contribution (Last paid receipt not older than 2010) <strong>in</strong> respect of<br />
labours / employees employed by the contractor <strong>for</strong> the execution of <strong>works</strong>.<br />
Important Note: - If any documents submitted by bidder are not <strong>in</strong> legible<br />
condition that will be treated as null & void and on this ground his bid will be<br />
summarily rejected.<br />
