iv. Age<br />

v. Nature of accident and cause of accident.<br />

vi. Time and date of accident.<br />

vii. Date and time when admitted <strong>in</strong> hospital.<br />

viii. Date of discharge from the hospital.<br />

ix. Period of treatment and result of treatment.<br />

x. Percentage of loss of earn<strong>in</strong>g capacity and disability as<br />

assessed by medical officer.<br />

xi. Claim required to be paid under workmen’s compensation Act.<br />

xii. Date of payment of compensation.<br />

xiii. Amount paid with details of the person to whom the same was<br />

paid.<br />

xiv. Authority by whom the compensation was assessed.<br />

Remarks<br />

� The contractor shall ma<strong>in</strong>ta<strong>in</strong> a Register of F<strong>in</strong>es <strong>in</strong> the <strong>for</strong>m XII of the<br />

CL (R & A) Rules 1971 The contractor shall display <strong>in</strong> a good condition<br />

and <strong>in</strong> a conspicuous place of work the approved list of acts and<br />

omission <strong>for</strong> which f<strong>in</strong>es can be imposed.<br />

� The contractor shall ma<strong>in</strong>ta<strong>in</strong> a Register of Deductions <strong>for</strong> damage or<br />

loss <strong>in</strong> <strong>for</strong>m XX of the CL (R & A) Rules 1971.<br />

� The contractor shall ma<strong>in</strong>ta<strong>in</strong> a Register of advances <strong>in</strong> <strong>for</strong>m XXIII of the<br />

CL(R & A) Rules 1971.<br />

� The contractor shall ma<strong>in</strong>ta<strong>in</strong> a Register of overtime <strong>in</strong> <strong>for</strong>m XXIII of the<br />

CL (R & A) Rules 1971.<br />

e) Attendance card-cum wage slip<br />

i. The contractor shall issue an Attendance card cum wage slip to<br />

each workman employed by him.<br />

ii. The card shall be valid <strong>for</strong> each wage period.<br />

iii. The contractor shall mark the attendance of each workman on<br />

the card twice each day, once at the commencement of the day<br />

and aga<strong>in</strong> after the rest <strong>in</strong>terval, be<strong>for</strong>e he actually starts work.<br />

iv. The card shall rema<strong>in</strong> <strong>in</strong> possession of the worker dur<strong>in</strong>g the<br />

wage period under reference.<br />

v. The contractor shall complete the wage slip portion on the<br />

reverse of the card at least a day prior to the disbursement of<br />

wages <strong>in</strong> respect of the wage period under reference.<br />

vi. The contractor shall obta<strong>in</strong> the signature or thump impression of<br />

the worker on the wage slip at the time of disbursement of<br />

wages and reta<strong>in</strong> the card with him.<br />

f) Employment Card<br />

The contractor shall issue an Employment Card <strong>in</strong> the <strong>for</strong>m XIV of CL<br />

(R&A) central Rules 1971 to each worker with<strong>in</strong> three days of the<br />

employment of the worker.<br />

g) Service certificate<br />
