Destination of Injured PersonRecord the destination of each injured that is transported from the scene of the crash. Thedestination should be preceded by the unique letter designation from column 1 for theperson involved, if they were taken to a hospital, clinic, doctor's office, or other place ofemergency medical aid. Include both the name of the treatment facility and city or town.This is important in tracing the victim from the scene of the crash through the health caresystem (Example: A - N.C.
