_____________________________________________________________________________________________GENERAL INSTRUCTIONSThe <strong>Crash</strong> <strong>Report</strong> Form DMV-349 is to be used by all law enforcement officers to report motorvehicle crashes in <strong>North</strong> <strong>Carolina</strong>.GENERALA reportable motor vehicle traffic crash must meet at least one of the following criteria:1. The crash resulted in a fatality, or2. The crash resulted in a non-fatal personal injury, or3. The crash resulted in total property damage amounting to $1,000.00 or more, or4. The crash resulted in property damage of any amount to a vehicle seized.In addition, a reportable motor vehicle traffic crash must occur on a trafficway (any land wayopen to the public as a matter of right or custom for moving persons or property from one placeto another) or occur after the motor vehicle runs off the roadway but before events arestabilized. The terms collision, accident, and crash are synonymous when describing a motorvehicle crash.After the investigation of the crash is completed, <strong>North</strong> <strong>Carolina</strong> General Statute 20-166.1requires that the investigating agency submit the report to the Division within 10 days.The Division of Motor Vehicles (DMV) requests that:1. The DMV-349 should be typewritten or if handwritten the officer should use black ink.2. The report should be legible. This is of the utmost importance for clarity, when reportsare microfilmed or imaged.3. Some users of the data may include the county engineer planningto resurface a road, the city consultant developing safe school routes, the high school drivereducation teacher planning a curriculum, or the public works director applying for a state grantfor reconstructing a hazardous intersection.
