All of the contamination cases are<strong>report</strong>ed by IHW Permits Section.Of the seven monitoring programs identified in the preceding narrative, only the programs monitoringreleases (Program Nos. 2, 4, 5, 6, 7) have a potential of being listed in the table. Programs requiring andimplementing assessment of a release (Nos. 2, 4, 6) are considered in Activities Status Code 2. Programsrequiring and conducting corrective action monitoring are considered in Activities Status Code 5. Because ofthe long-term nature of aquifer remediation programs, the following distinction was made when determining afacility's Activities Status:Code 4 - Implement Action - includes facilities in the process of installing a corrective action systemand/or performing source remediation under a specific time frame, such as one or two years;Code 5 - Monitor Action - includes facilities that have completed short-term activities (Code 4activities) and are now performing long-term remediation and monitoring activities for which notime frame for completion can be specified.The remediation activity will be considered complete when the required <strong>groundwater</strong> quality standards havebeen achieved, as determined by the monitoring program.39
