RadiographicFeaturesSuggestiveofCancerintheJawsMalignantlesionsmayariseprimarilyinthemaxillaeormandible,extenddirectlyintothejawsfromprimarysitesinoralsofttissues,ormetastasizetothejawsfromdistantlocations.Certainradiographicsignssuggestthepresenceofcancerinthejaws(GeistJR.Imagingmalignantlesionsoftheheadandneck.OralMaxillofacSurgClinNAmer2001;13(4):697‐712.WoodRE.Malignantdiseasesofthejaws.In:WhiteSC,PharoahMJ.OralRadiology,5thed.St.Louis,Mosby,2003,pp458‐84.FarmanAG,NortjeCJ,WoodRE:OralandMaxillofacialDiagnosticImaging.St.Louis,Mosby,1993,pp1‐64)NumberandlocationoflesionsNeoplasmsaregenerallysolitarylesions.However,somemalignanciessuchasmultiplemyelomaaremultifocal,andmetastasesfromdistantsitescanoccasionallybefoundinmorethanonelocationinthejaws.Mostmalignantlesionsarenotfoundinassociationwiththeteeth,exceptfortherarecasesofcarcinomaarisinginpre‐existingodontogeniccystsortumors.Manycancersdevelopoutsideofthealveolarprocesses,butcanextendintothetoothbearingregions.Figure1InternalcharacteristicsMalignantneoplasmsaregenerallyosteolytic,withtumorcellsproliferatingattheexpenseofthenormalbones.Somelesions,suchasmultiplemyeloma,lymphomasinbone(e.g.,Burkitt 242010 CDEL Re-recognition of the Specialty Report 208 of 279
lymphoma)andprimarycarcinomas,areentirelyradiolucent(Figure1).Manycancers,however,arecharacterizedbyproductionofhardtissueorinductionofreactiveboneformationinandaroundthelesion.Osteosarcomaandchondrosarcomagenerallyproducemixedradiographicpatternswithneoplasticboneandcartilageinterspersedwiththesofttissuetumormasses(Figure2).Somemetastaticmalignancies,notablyprostateandbreastcancer,canstimulateformationofboneperipherally(Figure3).Itshouldalsobenotedthattheterms“radiolucent”and“radiopaque”arerelative.Asofttissuetumordestroyingthemaxillaisradiolucentincontrasttotheadjacentbone,butthesamelesionextendingintothemaxillarysinusproducesafaintradiopacityagainstthebackgroundoftheairspace.
