Examinetheentireheadandneckincludingtheeyes,ears,nose,skullandscalp.Theeyesshouldtracknormallyandshouldbefreeofswellinganddrainage.Thenoseshouldbepalpated,andtheanteriorportionofthenarescanbeobserved.Theearsshouldbeinspectedandpalpated,includingthepinna,lobe,auricleandvisibleportionsoftheexternalauditorycanal.Theneckshouldalsobeexaminedatthistime.Thepatientshouldberestingagainsttheheadrestinarelaxedposition,andhavingthepatientgentlydrooptheirheadforwardmayenhanceneckpalpation.Bimanuallypalpatetheneck,comparingtherightandleftsidesforasymmetry,lookingforenlargedpainlesslymphnodesandotherabnormalmasses.Thesubmentalnodesareconvenientlypalpatedwiththefingertips(figure7,figure8).Thesubmandibularnodesarepalpatedextraorallybetweenthefingersandthelingualaspectofthemandible,andintraorallyusingglovedfingerstopressagainstthestructuresusingtheotherhandbelowontheoutsideoftheskin.Thejugularchainsarepalpatedusingdeeplyplacedfingersoneithersideofthesternocleidomastoidmusclefromitsoriginattheclavicletoitsinsertionatthemastoidprocess,includingthepreauricular,parotid,andretroauricularnodes.Includetheanteriorscaleneandsupraclavicularnodesabovetheclavicles,andthedelphiannodesneartheinferiormidlineoftheneck(figure9).Palpatethespinalaccessorynodes,movingthetissuesacrossthetrapeziusmusclebodytoaiddetection.Figure8 372010 CDEL Re-recognition of the Specialty Report 221 of 279
