This template is suggested <strong>for</strong> use <strong>in</strong> ga<strong>the</strong>r<strong>in</strong>g <strong>in</strong><strong>for</strong>mation required <strong>for</strong> risk<br />

assessment when advis<strong>in</strong>g on <strong>malaria</strong> <strong>prevention</strong>. It may be downloaded <strong>for</strong> use<br />

<strong>from</strong> <strong>the</strong> Health Protection Agency website at www.hpa.org.uk or adapted <strong>for</strong> <strong>the</strong><br />

particular circumstances of <strong>in</strong>dividual cl<strong>in</strong>ics.<br />

A condition <strong>in</strong> a first-degree relative may not contra<strong>in</strong>dicate <strong>the</strong> use of an anti<strong>malaria</strong>l,<br />

but may <strong>in</strong>fluence <strong>the</strong> choice of drug.<br />
