Although mefloqu<strong>in</strong>e may be suitable<br />

<strong>for</strong> scuba divers who have taken and<br />

tolerated <strong>the</strong> drug be<strong>for</strong>e, or those able<br />

to start tak<strong>in</strong>g it early to ensure that no<br />

adverse events occur, it should usually be<br />

avoided: it lowers <strong>the</strong> seizure threshold<br />

(and may <strong>the</strong>re<strong>for</strong>e add to <strong>the</strong><br />

complications of decompression or<br />

narcosis events) and some<br />

neuropsychiatric adverse events, though<br />

excessively rare, can be sudden <strong>in</strong> onset.<br />

Airl<strong>in</strong>e pilots<br />

The UK Civil Aviation Authority advises<br />

that mefloqu<strong>in</strong>e should not be<br />

adm<strong>in</strong>istered to airl<strong>in</strong>e pilots, although<br />

<strong>the</strong>re is no evidence that mefloqu<strong>in</strong>e<br />

impairs function.<br />

Methods of adm<strong>in</strong>istration<br />

Oral. 250mg tablets. Weekly dosage,<br />

start<strong>in</strong>g 2-3 weeks be<strong>for</strong>e enter<strong>in</strong>g a<br />

malarious area to assess tolerability,<br />

cont<strong>in</strong>u<strong>in</strong>g throughout <strong>the</strong> time <strong>in</strong> <strong>the</strong><br />

area and <strong>for</strong> 4 weeks after leav<strong>in</strong>g <strong>the</strong><br />

malarious area.<br />

4.2.5 Doxycycl<strong>in</strong>e<br />

Mode of action<br />

Doxycycl<strong>in</strong>e is lipophilic and acts<br />

<strong>in</strong>tracellularly, b<strong>in</strong>d<strong>in</strong>g to ribosomal<br />

mRNA and <strong>in</strong>hibit<strong>in</strong>g prote<strong>in</strong> syn<strong>the</strong>sis.<br />

It acts as a suppressive prophylactic.<br />

Efficacy<br />

Doxycycl<strong>in</strong>e is of comparable<br />

prophylactic efficacy to mefloqu<strong>in</strong>e 37 .<br />

Side-effects<br />

Doxycycl<strong>in</strong>e hydrochloride preparations<br />

have a low pH and may produce<br />

oesophagitis, especially if taken on an<br />

empty stomach and/or just be<strong>for</strong>e ly<strong>in</strong>g<br />

down. Rarely, doxycycl<strong>in</strong>e may cause<br />

photosensitivity which is mostly mild and<br />

transient 38 . Doxycycl<strong>in</strong>e is a broad<br />

spectrum antibiotic and may predispose<br />

to vag<strong>in</strong>al candidiasis.<br />

Interactions<br />

Drugs: The metabolism of doxycycl<strong>in</strong>e is<br />

accelerated by carbamazep<strong>in</strong>e and<br />

phenyto<strong>in</strong>. At present, <strong>the</strong>re are no data<br />

to support a change <strong>in</strong> <strong>the</strong> dose of<br />

doxycycl<strong>in</strong>e used <strong>for</strong> <strong>malaria</strong> prophylaxis<br />

<strong>in</strong> <strong>in</strong>dividuals tak<strong>in</strong>g one or more of<br />

<strong>the</strong>se agents. Tetracycl<strong>in</strong>es possibly<br />

enhance <strong>the</strong> anticoagulant effect of<br />

coumar<strong>in</strong>s (e.g. warfar<strong>in</strong>), and may<br />

<strong>in</strong>crease plasma concentration of<br />

ciclospor<strong>in</strong>. It may temporarily reduce<br />

<strong>the</strong> contraceptive effect of oestrogens<br />

(see FAQs <strong>in</strong> Chapter 8).<br />

Vacc<strong>in</strong>es: Possibly reduces <strong>the</strong> efficacy of<br />

oral typhoid vacc<strong>in</strong>e if given<br />

simultaneously. Should be separated by<br />

24 hours.<br />

Contra<strong>in</strong>dications<br />

Children under 12 years of age.<br />

Pregnancy and breast feed<strong>in</strong>g. Allergy to<br />

tetracycl<strong>in</strong>es.<br />
