7.1 Children<br />

G UIDELINES FOR M ALARIA P REVENTION IN<br />

T RAVELLERS FROM THE U NITED K INGDOM<br />

Children are at particular risk of severe and fatal <strong>malaria</strong>; <strong>the</strong>re<strong>for</strong>e, parents are<br />

advised aga<strong>in</strong>st tak<strong>in</strong>g <strong>in</strong>fants and young children to malarious areas.<br />

If travel is unavoidable, <strong>in</strong>fants and children should be well protected aga<strong>in</strong>st<br />

mosquito bites and receive appropriate <strong>malaria</strong> chemoprophylaxis.<br />

It is important that <strong>the</strong> child’s carers understand <strong>the</strong> importance of try<strong>in</strong>g to ensure<br />

that <strong>the</strong> child properly completes <strong>the</strong> full course of prophylactic medication.<br />

Parents should supervise children’s chemoprophylaxis, as some regimens can be<br />

difficult even <strong>for</strong> adults to follow.<br />

Parents must be cautious not to exceed maximum recommended doses, s<strong>in</strong>ce<br />

anti<strong>malaria</strong>ls can be particularly toxic to children.<br />

Paediatric doses of anti<strong>malaria</strong>ls <strong>for</strong> prophylaxis are shown <strong>in</strong> tables 4-6 <strong>in</strong> chapter 4:<br />

• Chloroqu<strong>in</strong>e: Take care to ensure<br />

that tablets are actually swallowed,<br />

as <strong>the</strong>y have a bitter taste. Sweetened<br />

chloroqu<strong>in</strong>e syrup is available. Store<br />

safely away <strong>from</strong> children s<strong>in</strong>ce an<br />

overdose can be fatal.<br />

• Proguanil: as <strong>for</strong> adults more effective<br />

when taken with chloroqu<strong>in</strong>e<br />

although chloroqu<strong>in</strong>e resistance is<br />

present <strong>in</strong> many areas. Difficult to use<br />

<strong>for</strong> children s<strong>in</strong>ce proguanil is only<br />

available <strong>in</strong> adult <strong>for</strong>mulations and,<br />

dependent on <strong>the</strong> weight of <strong>the</strong><br />

child, <strong>the</strong> adult-dose tablets must be<br />

broken and powdered <strong>in</strong>to food.<br />

• Chloroq<strong>in</strong>e plus proguanil: see<br />

<strong>in</strong>dividual agents above.<br />

• Mefloqu<strong>in</strong>e: Problem <strong>in</strong> adm<strong>in</strong>ister<strong>in</strong>g<br />

correct dosage because <strong>the</strong>re is<br />

currently no suspension available and<br />

adult-dose tablets must be broken.<br />

• Doxycycl<strong>in</strong>e: Only licensed <strong>in</strong> <strong>the</strong> UK<br />

<strong>for</strong> children over <strong>the</strong> age of 12 years<br />

due to <strong>the</strong> bone damag<strong>in</strong>g effects of<br />

<strong>the</strong> drug. This age limit varies<br />

between countries; tablets should<br />

be swallowed whole and must not<br />

be crushed.<br />

• Atovaquone/proguanil: Paediatric<br />

tablets are available <strong>in</strong> <strong>the</strong> UK <strong>for</strong><br />

<strong>malaria</strong> prophylaxis <strong>in</strong> children <strong>from</strong><br />

11 kg upwards. The tablets are a<br />

quarter of <strong>the</strong> strength of adult<br />

tablets and can be crushed if<br />

necessary <strong>for</strong> ease of adm<strong>in</strong>istration.<br />
