4.2.3 Chloroqu<strong>in</strong>e plus proguanil<br />

agents.<br />

ACMP does not recommend <strong>the</strong> use<br />

of chloroqu<strong>in</strong>e plus proguanil <strong>for</strong><br />

<strong>travellers</strong> to sub-Saharan Africa.<br />

If no alternative is felt to be<br />

appropriate, <strong>the</strong> matter should be<br />

discussed with an expert centre<br />

(see chapter 9).<br />

4.2.4 Mefloqu<strong>in</strong>e<br />

Mode of action<br />

Mefloqu<strong>in</strong>e’s mode of action has not<br />

been determ<strong>in</strong>ed, but is thought to be<br />

unrelated to that of chloroqu<strong>in</strong>e and not<br />

to <strong>in</strong>volve an anti-folate action. It acts as<br />

a suppressive prophylactic.<br />

Efficacy<br />

The protective efficacy of mefloqu<strong>in</strong>e is<br />

approximately 90% <strong>in</strong> Africa 31 . At <strong>the</strong><br />

present time, significant resistance of<br />

P. falciparum to mefloqu<strong>in</strong>e is a problem<br />

only <strong>in</strong> some areas of south-East Asia 32 ,<br />

but is reported sporadically <strong>from</strong> <strong>the</strong><br />

Amazon bas<strong>in</strong>.<br />

Side-effects<br />

Attention has focused on<br />

neuropsychiatric problems with<br />

mefloqu<strong>in</strong>e prophylaxis. Increased<br />

moderate problems have been found<br />

especially <strong>in</strong> women us<strong>in</strong>g mefloqu<strong>in</strong>e<br />

when compared with those receiv<strong>in</strong>g<br />

doxycycl<strong>in</strong>e, or atovaquone plus<br />

proguanil, but not those tak<strong>in</strong>g<br />

chloroqu<strong>in</strong>e plus proguanil 33 but <strong>the</strong>re<br />

is no evidence that mefloqu<strong>in</strong>e use<br />

<strong>in</strong>creases <strong>the</strong> risk of first-time diagnosis<br />

of depression 34 and no association<br />

between mefloqu<strong>in</strong>e prescriptions and<br />

hospitalisation 35 . Overall, mefloqu<strong>in</strong>e<br />

rema<strong>in</strong>s an important prophylactic agent<br />

which is tolerated by <strong>the</strong> majority of<br />

<strong>travellers</strong> who take it 36 .<br />

Interactions<br />

Drugs: Mefloqu<strong>in</strong>e antagonises <strong>the</strong><br />

anticonvulsant effect of antiepileptics<br />

and <strong>in</strong>teracts with a number of cardiac<br />

drugs.<br />

Contra<strong>in</strong>dications<br />

Mefloqu<strong>in</strong>e prophylaxis is contra<strong>in</strong>dicated<br />

<strong>in</strong> those with a current or previous<br />

history of depression, neuropsychiatric<br />

disorders or epilepsy; or with<br />

hypersensitivity to qu<strong>in</strong><strong>in</strong>e.<br />

Cautions<br />

Pregnancy; (see Chapter 6,special<br />

groups) breast-feed<strong>in</strong>g; (see Chapter 6,<br />

special groups) cardiac conduction<br />

disorders. Not recommended <strong>in</strong> <strong>in</strong>fants<br />

under 3 months (5kg).<br />

Can mefloqu<strong>in</strong>e be taken by those<br />

who plan to undertake underwater<br />

div<strong>in</strong>g?<br />

If <strong>the</strong> <strong>in</strong>dividual tolerates mefloqu<strong>in</strong>e<br />

prophylaxis, <strong>the</strong>re is no evidence that<br />

<strong>the</strong>y cannot physically per<strong>for</strong>m<br />

underwater div<strong>in</strong>g. However, it should be<br />

noted that some sub-aqua centres do not<br />

permit those tak<strong>in</strong>g mefloqu<strong>in</strong>e to dive.
