G UIDELINES FOR M ALARIA P REVENTION IN<br />

T RAVELLERS FROM THE U NITED K INGDOM<br />

suffer<strong>in</strong>g an acute attack of <strong>malaria</strong><br />

after return<strong>in</strong>g home.<br />

Pregnant women and small children are<br />

at higher risk than o<strong>the</strong>rs of suffer<strong>in</strong>g<br />

severe disease.<br />

Risk assessment and personal counsell<strong>in</strong>g<br />

is essential to warn <strong>in</strong>dividuals of <strong>the</strong> risk<br />

of suffer<strong>in</strong>g <strong>from</strong> <strong>malaria</strong>, emphasis<strong>in</strong>g<br />

avoidance measures, and <strong>the</strong> need <strong>for</strong><br />

immediate diagnosis and treatment of<br />

acute feverish illnesses<br />

7.12.1 Preventive measures appropriate<br />

to an endemic sett<strong>in</strong>g 79<br />

Bednets<br />

Bed nets and o<strong>the</strong>r personal barrier<br />

protective measures (e.g. suitable<br />

cloth<strong>in</strong>g) are very low-cost, are effective<br />

long-term, have virtually no side-effects<br />

and will also help to protect <strong>from</strong> o<strong>the</strong>r<br />

mosquito-borne <strong>in</strong>fections.<br />

Intermittent Preventive Therapy (IPT)<br />

If IPT is local policy <strong>in</strong> <strong>the</strong>ir dest<strong>in</strong>ation<br />

country to prevent <strong>malaria</strong> <strong>in</strong> pregnancy<br />

and childhood, <strong>the</strong> return<strong>in</strong>g visitor<br />

should be advised to seek medical advice<br />

on this immediately on arrival.<br />

Case management of illness<br />

People should be advised to seek<br />

medical attention immediately if ei<strong>the</strong>r<br />

<strong>the</strong>y or <strong>the</strong>ir children become feverish<br />

after repatriation <strong>in</strong> <strong>the</strong> endemic<br />

country. They should be warned that a<br />

<strong>malaria</strong> attack may be more serious<br />

because of dim<strong>in</strong>ished immunity.<br />

7.12.2 Prophylaxis<br />

Intended use<br />

The ACMP prophylaxis guidance is <strong>for</strong><br />

temporary protection <strong>for</strong> <strong>the</strong> UK traveller.<br />

This is not appropriate <strong>for</strong> <strong>in</strong>dividuals<br />

who are return<strong>in</strong>g to permanent<br />

residence <strong>in</strong> <strong>the</strong>ir country of orig<strong>in</strong>.<br />

Exception <strong>for</strong> pregnant women and<br />

young children<br />

A limited period of prophylaxis of four to<br />

six weeks <strong>for</strong> pregnant women and<br />

young children may be appropriate <strong>in</strong><br />

some circumstances, to allow <strong>the</strong>m to<br />

settle and arrange <strong>for</strong> future healthcare<br />

after arrival <strong>in</strong> <strong>the</strong> endemic country.<br />

Standby treatment<br />

Offer<strong>in</strong>g standby treatment is<br />

<strong>in</strong>appropriate where <strong>the</strong>re are likely to be<br />

health services to diagnose and manage<br />

<strong>malaria</strong>.
