Services furnished to a minor by parent(s), stepparent(s) or legal guardian.<br />
Services furnished to a participant by the participant’s spouse.<br />

Section 10.8: Community Transition (currently under CIHW only)<br />
Service Definition<br />
<br />
Community Transition services include reasonable, one-time set-up expenses for individuals who<br />
make the transition from an institution to their own home in the community and will not be<br />
reimbursable on any subsequent move.<br />
<br />
Note: “Own home” is defined as any dwelling, including a house, an apartment, a condominium, a<br />
trailer, or other lodging that is owned, leased, or rented by the individual and/ or the individual’s<br />
guardian or family, or a home that is owned and/ or operated by the agency providing supports.<br />
Items purchased through Community Transition services are the property of the individual receiving<br />
the service, and the individual should take the property with him or her in the event of a move to<br />
another residence, even if the residence from which he or she is moving is owned by a provider<br />
agency. Nursing Facilities are not reimbursed for Community Transition services because those<br />
services are part of the per diem.<br />
Reimbursable Activities<br />
Reimbursable activities include the following:<br />

Security deposits that are required to obtain a lease on an apartment or home<br />
Essential furnishings and moving expenses required to occupy and use a community domicile<br />
including a bed, table or chairs, window coverings, eating utensils, food preparation items, and bed<br />
or bath linens<br />
Set-up fees or deposits for utility or service access including telephone, electricity, heating, and<br />
water<br />
Health and safety assurances including pest eradication, allergen control, or one-time cleaning prior<br />
to occupancy<br />
When the individual is receiving Residential Habilitation and Support, Structured Family Caregiving services,<br />
or Community-Based Habilitation - Individual services under the CIH <strong>Waiver</strong>, the Community Transition<br />
Supports service is included in the Cost Comparison Budget.<br />
Service Standards<br />
Community Transition services must be reflected in the ISP and the Cost Comparison Budget (POC/CCB) of<br />
the individual.<br />
