Documentation Standards<br />
Documentation requirements include maintaining receipts for all expenditures, showing the amount and<br />
what item or deposit was covered.<br />
Limitations<br />
The following limitations apply to Community Transition Services:<br />
Community Transition Services are limited to one-time set-up expenses, up to $1,000.<br />
Activities Not Allowed<br />
The following activities are not allowed under Community Transition services:<br />

Apartment or housing rental expenses<br />
Food<br />
Appliances<br />
Diversional or recreational items such as hobby supplies<br />

Service Definition<br />
Electronic Monitoring/Surveillance System and On-Site Response includes the provision of oversight and<br />
monitoring within the residential setting of adult waiver participants through off-site electronic surveillance.<br />
Also included is the provision of stand-by intervention staff prepared for prompt engagement with the<br />
participants and/or immediate deployment to the residential setting.<br />
Reimbursable Activities<br />
Reimbursable activities include the following:<br />

Electronic Monitoring/Surveillance System and On-Site Response may be installed in residential<br />
settings in which all residing adult participants, their guardians, and their support teams request<br />
such surveillance and monitoring in place of on-site staffing.<br />
Use of the system may be restricted to certain hours through the ISPs of the participants involved.<br />
Service Standards<br />
To be reimbursed for operating an electronic monitoring and surveillance system, a provider must adhere to<br />
