This workshop will provide an overview on caregiver burden and<br />

practical applications. Caregiver burden will be explored through<br />

the lens of stress theory, role theory, grounded optimism, grief,<br />

communication skills and coping skills. The need for help and<br />

support that individual and family interventions provide may<br />

improve the quality of life for the caregiver, which in turn may<br />

improve the quality of life for the care recipient.<br />

feuds that result from caring for an aging parent. Explore the<br />

family harmony and better care for their loved one.<br />

WE527 Legal Issues with Family Caregivers<br />

There are legal standards that govern all actions taken by a<br />

caregiver, even if the caregiver is a family member. Other<br />

interested family members, such as siblings who are not acting<br />

as the day-to-day caregivers, are generally allowed to sue the<br />

caregiver for a variety of infractions, especially regarding money<br />

or property that would have passed to those family members<br />

upon the death of the informed individual. This session will<br />

highlight some of the legal rules that need to be considered.<br />

Low health literacy is associated with a risk of poor health<br />

outcomes. People most at risk experience multiple health<br />

disparities, including access to care, effective communication<br />

with healthcare providers, and decreased treatment adherence.<br />

Attendees will become familiar with the impact of health<br />

literacy on health outcomes and learn evidence-based strategies<br />

to improve communication and understanding of illness and<br />

treatment options through shared decision-making techniques.<br />

WE531 Filling the Gap in Oral Health for Older<br />

Adults: A Community Guide to <strong>Program</strong><br />

Implementation<br />

The Administration for Community Living and the Office on<br />

Women’s Health are collaborating to identify and promote<br />

community-based oral health programs for older adults in the<br />

United States. The final product will be a public website with<br />

two main components: a searchable database of communitybased<br />

oral health programs and the Community Guide to Adult<br />

Oral Health <strong>Program</strong> Implementation, which provides program<br />

development resources to communities designing oral health<br />

programs for older adults.<br />

Case Management <strong>Program</strong> Since 1995<br />

The Community Case Management (CCM) program consistently<br />

demonstrates a reduction in readmissions and ED visits.<br />

Data is tracked and published in an annual report. This session<br />

will include a discussion of the CCM program including the<br />

tools used to assess and identify patient/family needs,<br />

measurements of data gathered, referral sources, admission<br />

criteria, caseload, process for providing interventions, reasons<br />

for referrals, strengths of the program, and patient/provider<br />

satisfaction surveys.<br />

WE535 The Gap in Meal Services for Seniors<br />

Living With Chronic or Life-Altering Illness<br />

This session will review the gap in meal and nutrition services<br />

between our country’s response to malnutrition and food<br />

insecurity for individuals with serious illnesses regardless of age.<br />

We will also consider how medical nutrition therapy services<br />

that are accessed as a core part of the healthcare system can<br />

support an aging population that is also dealing with the onset<br />

of chronic or life-altering illnesses.<br />
