LTSS access functions into single statewide Aging and Disability Resource Center/No Wrong Door systems to work better for<br />
consumers and their families. The AARP Scorecard project, funded by The SCAN Foundation and the Commonwealth Fund and in<br />
collaboration with the U.S. Administration for Community Living, researched promising practices to speed up the pace of change.<br />
