The impact of a low-cost telehealth intergenerational program<br />

with community-dwelling older adults will be discussed. The<br />

purpose of the program is to raise awareness of wellness within<br />

skyrocketing healthcare costs, and reduce 30-day hospital<br />

readmission and ER visits. Information to replicate the study will<br />

be provided.<br />

for individuals with dementia residing in six Wisconsin-based<br />

Assisted Living Communities. The study was conducted by<br />

University of Wisconsin-Madison, Center for Health Systems<br />

Research and Analysis.<br />

and its 15 diverse cities embarked on a regional initiative to guide<br />

individual cities and the county to join the World Health Organization<br />

Network of Age Friendly cities. Learn how the multidisciplinary<br />

team developed a community organizing model engaging<br />

seniors, stakeholders and elected officials in the process.<br />

A common pitfall of evidence-based interventions is the<br />

difficulty of integrating them into everyday practice. We will<br />

describe the implementation of the Adult Day Service Plus<br />

<strong>Program</strong> in two centers in which the Normalization Process<br />

Theory served as a framework for planning, adaptation, integration<br />

and evaluation. Emphasis will focus on processes necessary<br />

to embed this multifaceted care management program into<br />

routinized practice.<br />
