The impact of a low-cost telehealth intergenerational program<br />
with community-dwelling older adults will be discussed. The<br />
purpose of the program is to raise awareness of wellness within<br />
skyrocketing healthcare costs, and reduce 30-day hospital<br />
readmission and ER visits. Information to replicate the study will<br />
be provided.<br />

for individuals with dementia residing in six Wisconsin-based<br />

Adult Day Service Plus <strong>Program</strong><br />

A common pitfall of evidence-based interventions is the<br />
difficulty of integrating them into everyday practice. We will<br />
describe the implementation of the Adult Day Service Plus<br />
<strong>Program</strong> in two centers in which the Normalization Process<br />
Theory served as a framework for planning, adaptation, integration<br />
and evaluation. Emphasis will focus on processes necessary<br />
to embed this multifaceted care management program into<br />
routinized practice.<br />
