a New Quality Blueprint for Action<br />

Up to one out of every two older Americans is at risk for<br />
malnutrition, yet there is a gap in the delivery of malnutrition<br />
care for older adults. The Defeat Malnutrition Today coalition<br />
has worked with other partners to develop a new national<br />
blueprint for action, which outlines potential actions to close the<br />
gap and improve health outcomes for older adults by addressing<br />
malnutrition care across the continuum of acute, post-acute and<br />
community settings. The blueprint also provides suggested<br />
strategies for policymakers, organizations, healthcare providers,<br />
patients and caregivers to address malnutrition. This session<br />
gives an overview of the blueprint and details how participants<br />
can integrate and align specific strategies with their own<br />
organization/agency agendas.<br />

lens with the WHO Age-Friendly Community framework<br />

and building collective impact without domination by one<br />
community approach.<br />

Elder abuse response is multifaceted. Elders can’t be left in or<br />
returned to their homes where perpetrators reside. This workshop<br />
will focus on how the shelter model leverages resources to fill<br />
critical service gaps for victims. We will examine the missing links<br />
to elder safety, including holistic faith and community-based<br />
coalitions and programs, and elder sheltering.<br />

housing stock and creating homes that are safe and healthy for<br />
aging. Aging network professionals will obtain tools to engage in<br />
making more affordable, accessible and appropriate housing<br />
options available at the local level and ensuring that housing is<br />
connected to the community so that people can continue to<br />
thrive as they age.<br />
