affordable care options. This session explores findings from a<br />
study of how consumers identify LTSS care options, the information<br />
sources they rely upon, and how adequately consumer<br />
needs are being met.<br />

and engagement program at the AiA16 conference, the<br />
Investor Protection Institute will present program developments<br />
and resources accessible to ASA members. <strong>Program</strong><br />
developments include engagement videos developed by states<br />
and other updates on what various states are doing to implement<br />
the program.<br />

Three organizations funded by CMS/CMMI’s Health Care<br />
Innovations Award implemented robust training programs for<br />
direct care workers caring for complex/high-risk patients.<br />
Preliminary data indicate positive effects on direct care workers’<br />
reported skillset and confidence to perform their jobs. This<br />
session will describe the training programs, elaborate on<br />
preliminary outcomes, describe implementation successes and<br />
challenges, and discuss implications for replication.<br />

Medical Homes (PCMHs) and the Care of Older Adults” by the<br />
Hartford PCMH Change Agents <strong>Program</strong>. PCMHs are now<br />
recognizing the need for connecting to community-based<br />
organizations that have access to long-term service supports,<br />
self-management programs, care transitions programs and<br />
dealing with social determinant needs. The approach to partnership<br />
will be explored.<br />
