Learning Collaborative Lessons<br />

This session features two first-generation business acumen<br />

learning collaboratives hosted by the National Council on Aging<br />

to achieve integrated, sustainable service systems for chronic<br />

disease self-management education programs. Aging professionals<br />

from 15 states participated to obtain Medicare reimbursement<br />

for their programs. Challenges, learnings and<br />

outcomes will be highlighted, along with tools and resources.<br />

TU259 Quality Improvement for Long-Term Success<br />

Quality improvement is not an option, but a necessity for<br />

community-based care agencies as demands for their services<br />

and expertise rise. It is not enough to comply with standards, but<br />

instead CBOs must emerge as leaders in the healthcare continuum.<br />

We will discuss best practices for internal capacity building<br />

around quality, including developing a committee, standardizing<br />

processes, conducting strategic data collection and implementing<br />

a feedback system to act on measures and achieve outcomes.<br />

Coordination Role in Diverse Settings<br />

effective person-centered planning, coordination and navigation<br />

elements from both aging and disabilities networks to<br />

create a new “enhanced” coordination role with additional key<br />

competencies that inspire consumers to actively engage in<br />

their own healthcare.<br />

TU263 Benefits of Using Library Materials<br />

With Persons With Dementia<br />

Until recently, the benefits of using library materials with<br />

persons living with dementia had not yet been studied. In 2016,<br />

the National Institutes of Health/National Library of Medicine<br />

funded a target project study into the benefits of sharing the<br />

award-winning Tales & Travel library program with persons with<br />

dementia, increasing cognitive and social interactions and<br />

improving relationships with caregivers.<br />

older adults and information about innovative programming that<br />

supports this critical feature of positive aging. Presenters will<br />

discuss the meaning, measurement, importance and prevalence<br />

of purpose in aging populations and how practitioners and<br />

organizations can help individuals, even those facing serious<br />

health and financial challenges, develop purposeful engagements.<br />

advocates in motion by encouraging consumers, providers and<br />

professionals to become civically engaged. You will be equipped<br />

resources and tools to help develop a volunteer’s capacity to<br />

participate in advocacy-related activities.<br />
