Refund Policy: This trip is NON-REFUNDABLE; doctor’s certificates will not be accepted. You will<br />
not receive a refund if you are denied bus entrance due to a positive COVID-19 screening and/or do not<br />
present Ontario Proof of Vaccination* with QR code and government-<strong>issue</strong>d identification. Should there be<br />
a mandatory provincial shutdown due to COVID-19 or GWSA cancels the trip due to lack of participation,<br />
all participants will receive a full refund to their method of payment.<br />
Don’t Wait! That means booking your trip before the trip must be cancelled due to low participation.<br />
It takes time to put these trips together and we need your commitment to make these trips happen! The<br />
Travel Committee must give the number of bookings by Friday, April 1, and, if there are not enough<br />
reservations at that time, the trip will be cancelled!<br />
Parking: Please do not park your vehicles in the Evergreen parking lot while you are on a day trip.<br />
Vehicles must be left at the Carousel parking area in Riverside Park where you will board the bus. This<br />
allows for other members to have a place to park when they come throughout the day.<br />
Accessibility: (1) Please inquire about accessible seating prior to booking your ticket. As with all<br />
buses, accessible seating is limited. You can help us ensure that we use this seating as efficiently as possible<br />
by providing information about your assistive device. Please note that: Travellers who will be using a<br />
powered wheelchair on trips are advised to note this when registering. While our trips are accessible,<br />
seating for a powered wheelchair on the bus is limited and will be spoken for on a first-come first-serve<br />
basis. We cannot accommodate scooters on the buses. Travellers who can transfer to a bus seat may have<br />
their collapsible, manual wheelchair or walker stored in the cargo area of the bus if required.<br />
Lift Limit: 500 lbs maximum weight (wheelchair and person combined) Lift Width: 32 inches<br />
(2) Travellers with a disability, who require assistance during the trip, must be accompanied by a person<br />
who is fully capable of rendering such assistance (this person must be an adult - cost for this person will be<br />
the price posted).<br />
PROCEDURES FOR BOOKING THIS TRIP: All participants will be required to have a current<br />
GWSA membership and present their Ontario Proof of Vaccination with QR code on it along with<br />
government-<strong>issue</strong>d identification showing name and date of birth to board the bus, and to gain entrance to<br />
establishments on the trip. If your membership needs renewal, please see the front desk staff at the<br />
Centre. The contact list is available on the GWSA website located on the Trips, Travel and Tours page. If<br />
you are unable to print this form, copies will be available from a Travel Committee member the day of the<br />
trip.<br />
Scanning and verifying the QR code through the Verify Ontario app is now the accepted proof of<br />
vaccination or medical exemption - the QR code may be scanned either from the member’s electronic<br />
device or from their paper printout that includes a QR code - vaccination receipts without QR codes will<br />
not be accepted. * The GWSA IPP Access Card can no longer be accepted as proof of vaccination. Please<br />
see the Trip, Travel and Tours page on the GWSA website for specific instructions.<br />
