Intended Use<br />
Enzyme immunoassay for the qualitative and quantitative detection of antibodies to hepatitis B<br />
surface antigen (<strong>HBs</strong>Ag) in serum and plasma.<br />
The enzyme immunoassay is processed using the BEP ® <strong>II</strong>, BEP ® <strong>II</strong>I and BEP ® 2000 ELISA<br />
processors. The test was developed for testing individual samples, not for pooled samples. The<br />
product is for in vitro diagnostic use only. The product is for in vitro diagnostic use only.<br />
Summary and Explanation<br />
Like hepatitis B surface antigen (<strong>HBs</strong>Ag), the anti-<strong>HBs</strong> antibody directed against the surface<br />
protein is an important parameter for the diagnosis of infection with hepatitis B virus (HBV) 1,2 .<br />
During the incubation period and in the acute phase of HBV infection, antibodies to <strong>HBs</strong>Ag are<br />
undetectable. In 90% of all cases these anti-<strong>HBs</strong> antibodies, which provide immunity, do not<br />
occur until late in convalescence approx. 3 to 4 months after the onset of the disease, when<br />
circulating <strong>HBs</strong>Ag can no longer be detected 3, 4 .<br />
This seroconversion, i.e. the transition from anti-<strong>HBs</strong> negative to positive, represents a very<br />
reliable parameter for diagnosing a past HBV infection, especially since approx. 10% of acutely<br />
infected patients in whom no <strong>HBs</strong>Ag can be detected in the early phase later become positive for<br />
anti-<strong>HBs</strong>. The anti-<strong>HBs</strong> test is therefore also suitable for the diagnosis for subclinical HBV<br />
infections 3, 4 .<br />
As a positive result for anti-<strong>HBs</strong> is indicative of past exposure to this antigen - either by HBV<br />
infection or by vaccination, the most important applications of the anti-<strong>HBs</strong> test are as follows:<br />
a) assessment of convalescence and, to a certain extent, also prognosis in patients infected with<br />
HBV (follow-up),<br />
b) serological investigations in the context of vaccination programmes (screening and<br />
immunization check-ups),<br />
c) epidemiological studies.<br />
Besides the qualitative detection of anti-<strong>HBs</strong>, the quantitative evaluation of anti-<strong>HBs</strong> has gained<br />
its own relevance with regard to the aspect of active immunization. According to a<br />
recommendation of the WHO, a person vaccinated with a hepatitis vaccine can be assumed to<br />
be protected against the infection if an anti-<strong>HBs</strong> concentration of > 10 IU/L can be detected in the<br />
serum or plasma. Booster vaccinations in time are recommended to guarantee that the values<br />
are not below this limit 5, 6, 7 .<br />
Patients found to have anti-<strong>HBs</strong> values < 100 IU/L after the completion of basic immunization<br />
require booster vaccination within one year (Recommendations of the Standing Vaccination<br />
Comittee of the Robert Koch Institute in Germany, October 1995).<br />
The necessity of a quantitative assessment is furthermore underlined by the fact that the<br />
duration of immunity is proportional to the attained levels of anti-<strong>HBs</strong>.<br />
Principle of the Method<br />
Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong> is a one-step assay based on the sandwich principle. The antigen used<br />
for the solid phase and conjugate is human <strong>HBs</strong>Ag.<br />
Peroxidase-labelled <strong>HBs</strong>Ag binds to the <strong>HBs</strong>-specific antibodies contained in the sample. These<br />
bind to the <strong>HBs</strong>Ag bound to the surface of the microtitration plate (antigen sandwich).<br />
The unbound constituents are removed by washing and the bound enzyme activity of the<br />
conjugate is then determined. The enzymatic conversion of substrate and chromogen results in<br />
a blue colour. The reaction is terminated by the addition of Stopping Solution POD, producing a<br />
yellow colour. The intensity of the colour is proportional to the concentration of antibody in the<br />
sample. The results are quantified by calculation using the α-method or by comparison with a<br />
standard curve.<br />

Warnings and Precautions<br />
1. For in vitro diagnostic use.<br />
2. Each individual blood donation for use in manufacture of the controls of Enzygnost * <strong>Anti</strong>-<strong>HBs</strong><br />
<strong>II</strong> is tested for <strong>HBs</strong>Ag, anti-HCV, anti-HIV1 and anti-HIV2. Only donations with negative<br />
findings are used for manufacture.<br />
Nevertheless, since absence of infectious agents cannot be proven, all materials obtained<br />
from human blood should be handled with due care, observing the precautions recommended<br />
for biohazardous material 8 .<br />
3. The <strong>HBs</strong>Ag used in the manufacture of the test plates and POD conjugate was isolated from<br />
<strong>HBs</strong>Ag-positive human plasmas, which were found to be negative concerning <strong>Anti</strong>-HCV, <strong>Anti</strong>-<br />
HIV1 and <strong>Anti</strong>-HIV2, to a high degree of purity and subjected to a recognized inactivation<br />
process.<br />
4. It is advisable to wear protective gloves throughout the entire test procedure.<br />
5. For disposal, it is recommended that solid infectious materials should be autoclaved for at<br />
least one hour at +121 °C. All aspirated liquids should be collected in two receptacles<br />
connected in series, which should both contain a disinfectant suitable for inactivating<br />
pathogenic human viruses. The concentrations and times specified by the manufacturer must<br />
be observed.<br />
Preparation of the Reagents<br />
Bring all the reagents and samples to +15 to +25 °C before beginning the test (without removing<br />
the test plate from the container).<br />
Test strips not needed for the test must be removed from the holder and stored for later use (see<br />
Table 1)<br />
For each test plate, dilute 20 mL of Washing Solution POD to 400 mL with distilled or deionized<br />
water.<br />
For each test plate, dilute 1 mL of Chromogen TMB with 10 mL of Buffer/Substrate TMB in<br />
the empty bottle supplied with the kit (= Working Chromogen Solution) and store protected from<br />
light. After use rinse the bottle thoroughly with distilled water.<br />
For technical reasons (overage) it is not permissible to transfer the contents of the Chromogen<br />
TMB vial into the vial of Buffer/Substrate TMB.<br />
Prediluting the samples: For the quantitative test, samples expected to contain anti-<strong>HBs</strong><br />
concentrations of ≥ 100 IU/L should be diluted as follows:<br />
up to 1000 IU/L: Dilute 1:10 (e.g. 20 µL sample + 180 µL <strong>Anti</strong>-<strong>HBs</strong> Serum, negative)<br />
up to 10,000 IU/L: Dilute 1:100 (e.g. 20 µL sample + 180 µL <strong>Anti</strong>-<strong>HBs</strong> Serum, negative;<br />
20 µL thereof + 180 µL <strong>Anti</strong>-<strong>HBs</strong> Serum, negative)<br />
up to 100,000 IU/L: Dilute1:1000 (e.g. 20 µL sample + 180 µL <strong>Anti</strong>-<strong>HBs</strong> Serum, negative;<br />
20 µL thereof + 180 µL <strong>Anti</strong>-<strong>HBs</strong> Serum, negative;<br />
20 µL thereof + 180 µL <strong>Anti</strong>-<strong>HBs</strong> Serum, negative).<br />
If the quantitative evaluation of the test is performed using serial dilutions, the following dilutions<br />
of the <strong>Anti</strong>-<strong>HBs</strong> Serum 100 must be additionally prepared with the <strong>Anti</strong>-<strong>HBs</strong> Serum, negative<br />
(does not apply if evaluation is by the α-method):<br />
50 IU/L: 1:2 dilution (e.g. 200 µL <strong>Anti</strong>-<strong>HBs</strong> Serum 100 + 200 µL <strong>Anti</strong>-<strong>HBs</strong> Serum, negative)<br />
25 IU/L: 1:4 dilution (e.g. 150 µL of the 50 IU/L Serum + 150 µL <strong>Anti</strong>-<strong>HBs</strong> Serum, negative)<br />
10 IU/L: 1:10 dilution (e.g. 50 µL <strong>Anti</strong>-<strong>HBs</strong> Serum 100 + 450 µL <strong>Anti</strong>-<strong>HBs</strong> Serum, negative)<br />

Storage and Stability<br />
Stored unopened at +2 to +8 °C, all components of the Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong> kit may be used up<br />
to the dates of expiry given on the labels.<br />
Once opened or diluted ready for use, the details given in Table 1 in the Appendix apply.<br />

Equipment required:<br />
BEP ® <strong>II</strong>: For automatic dispensing of reagent and washing<br />
BEP ® <strong>II</strong>I: For automatic processing of the test after dispensing the samples as well as for<br />
evaluation<br />
BEP ® 2000: For fully automatic processing and evaluation of the test<br />
Pipettes: piston-type pipettes: 25, 100 and 1000 µL.<br />
Incubator: Covered water bath (+37 ± 1 °C) or comparable incubation methods<br />
All the equipment used in the test must have been validated.<br />
Specimens<br />
Suitable specimens are individual samples (human sera or EDTA/ heparinized/citrated plasma)<br />
obtained by standard laboratory techniques.<br />
The samples should be stored for not more 3 days at 2 - 8 ° C. If the samples are to be stored for<br />
a longer period of time, they must be frozen.<br />
Procedure<br />
Procedure using the BEP ® <strong>II</strong><br />
1. Assay scheme: Ascertain the number of wells required (= number of test samples plus 6<br />
wells for the controls).<br />
2. Dispense samples:<br />
Qualitative Test/Quantitative Test (α - method):<br />
Into 4 wells (A1 to D1) pipette 100 µL/well of <strong>Anti</strong>-<strong>HBs</strong> Serum, negative, and into one well (E1)<br />
100 µL of <strong>Anti</strong>-<strong>HBs</strong> Serum 100. Fill each of the subsequent wells with 100 µL/well of sample.<br />
At the end of the series/plate, fill one further well with 100 µL of <strong>Anti</strong>-<strong>HBs</strong> Serum 100.<br />
Perform the pipetting steps swiftly within 15 min per test plate.<br />
Important: It is NOT permitted to pipette the <strong>Anti</strong>-<strong>HBs</strong> Serum 100 into the assigned wells<br />
first and then to add the samples into the "gaps" between the wells.<br />
Quantitative Test (standard series):<br />
Pipette into 4 wells 100 µL/well of <strong>Anti</strong>-<strong>HBs</strong> Serum, negative, and then fill the 100, 50, 25, 10<br />
and 5 IU/L dilutions of the <strong>Anti</strong>-<strong>HBs</strong> Serum into 2 wells each at 100 µL/well. Fill the following<br />
wells with 100 µL/well of sample (prediluted if required).<br />
Immediately after dispensing the samples continue by dispensing the conjugate.<br />
3. Dispense conjugate: Pipette 25 µL of the <strong>HBs</strong>Ag/POD Conjugate (<strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong>) into each<br />
well.<br />
Then seal with foil and, immediately after completion of the conjugate dispensing step, place<br />
the plate into the incubator.<br />
4. Incubate: Incubate at 37 ± 1 °C for 60 ± 2 min. Then proceed immediately to the wash step.<br />
5. Wash: Remove the foil, aspirate all the wells and wash 4 times with approx. 0.3 mL/well of<br />
washing solution. After completing the wash cycles proceed immediately to the substrate<br />
dispensing step in order to prevent drying out of the wells.<br />
6. Dispense substrate: Pipette 100 µL of the Working Chromogen Solution into each well and<br />
seal the plate with fresh foil.<br />
7. Incubate: Incubate protected from light at +15 to +25 °C for 30 ± 2 min.<br />
8. Stop reaction: Remove foil, and add 100 µL of Stopping Solution POD to each well, keeping<br />
to the same timing as in Step 6.<br />
9. Read: Read at 450 nm within one hour. The recommended wavelength for the reference<br />
measurement is 650 nm (where appropriate, between 615 and 690 nm).<br />

Test procedure using the BEP ® <strong>II</strong>I<br />
Before using the BEP ® <strong>II</strong>I, prepare the test plates and sample dispensing steps (Section 1 and 2<br />
at "Test procedure with the BEP ® <strong>II</strong>"). Immediately afterwards place the uncovered test plates<br />
(i.e. not covered with adhesive foil) into the BEP ® <strong>II</strong>I. Note that partially filled test plates need to<br />
be made up to at least half plates (6 test strips) by adding "water-filled strips". The test is then<br />
processed fully automatically (see BEP ® <strong>II</strong>I instruction manual).<br />
The settings for the incubation times in the BEP ® <strong>II</strong>I software may differ from the times on the<br />
BEP ® <strong>II</strong> for technical reasons (system speed) but have been validated for Enzygnost * on the<br />
BEP ® <strong>II</strong>I.<br />
Test procedure using the BEP ® 2000<br />
The sample dispensing steps and subsequent processing of the test are performed fully automatically<br />
by the analyzer (see BEP ® 2000 instruction manual).<br />
Validation of the test<br />
The individual values of the absorbances of the control sera are used for calculating the mean<br />
values if:<br />
–0.010 ≤ A neg ≤ 0.120<br />
A <strong>Anti</strong>-<strong>HBs</strong> Serum 100 ≥ 0.7<br />
If one of the four absorbance values of the negative controls is outside the specification, this<br />
value can be neglected.<br />
Both absorbance values of the <strong>Anti</strong>-<strong>HBs</strong> Serum 100 must comply with the specification.<br />
If these conditions are not fulfilled, the test must be repeated.<br />
For the quantitative test the following validation criteria apply:<br />
If serial dilutions are used:<br />
A neg ≤ A standard 5 IU/L<br />
If the α - method is used:<br />
The individual absorbance readings for the <strong>Anti</strong>-<strong>HBs</strong> Serum 100 are used to calculate the mean, if:<br />
lower margin < A <strong>Anti</strong>-<strong>HBs</strong> Serum 100 < upper margin<br />
The values for the lower and upper margins are given in the enclosed table of values.<br />
Both absorbance values of the <strong>Anti</strong>-<strong>HBs</strong> Serum 100 must fulfil the specification.<br />
In addition, the individual values must not differ by more than 20% from the mean of these two values.<br />
If these conditions are not met, the test can only be evaluated qualitatively.<br />
Evaluation<br />
The evaluations are performed automatically if using the BEP ® 2000 and BEP ® <strong>II</strong>I. Please<br />
consult the relevant instruction manuals. The following sections apply if the measurements are<br />
carried out without using a software.<br />
Qualitative test:<br />
Calculate the mean absorbance of the negative controls, then calculate the cut-off limit by<br />
adding 0.08:<br />

The test samples are classed as follows based on the criteria of the test:<br />
1. A sample < cut-off = <strong>Anti</strong>-<strong>HBs</strong> negative<br />
2. A sample ≥ cut-off = <strong>Anti</strong>-<strong>HBs</strong> positive<br />

Quantitative test (α - method):<br />
A correction factor is calculated for each individual processed test plate and used to correct the<br />
absorbance reading of each sample (measurement correction). The corrected readings are then<br />
used to calculate the antibody activity (see "Calculation of the results").<br />
Measurement correction<br />
The correction factor is calculated from the following formula:<br />

The nominal value is given in the enclosed table of values. The absorbance readings of the test<br />
samples are multiplied by the correction factor. The corrected readings obtained are then used to<br />
calculate the results.<br />
If several test plates are run, the correction factor has to be determined for each plate<br />
separately and used to correct the readings for the appropriate plate.<br />
Calculation of the results<br />
To calculate the antibody activities the corrected readings of the samples are used in the<br />
following formula (α-method):<br />
log 10 mIU/L = α .A β<br />
where α and β and are lot-dependent constants given in the enclosed table of values. The<br />
antibody activity result in "IU/L" relates to the WHO International HBV Reference Serum.<br />
Important: DO NOT use the following in the formula:<br />
- corrected readings < cut-off<br />
- uncorrected readings > 2.5<br />

If the test sample was prediluted (e.g. 1:11) for the test, then the calculated antibody activity (not<br />
the signal reading) must be multiplied by 11.<br />
Quantitative test (standard series)<br />
For the quantitative evaluation, calculate the mean absorbances from each pair of wells in the<br />
standard series (5, 10, 25, 50 and 100 IU/L) and use these values to establish a reference curve<br />
on double logarithmic paper. Abscissa: <strong>Anti</strong>-<strong>HBs</strong> concentration 5 to 100 IU/L. Ordinate:<br />
Absorbance 0.05 to 2.0. The anti-<strong>HBs</strong> concentrations of the samples can then be read from the<br />
reference curve on the basis of their absorbances.<br />
If the samples were used in diluted form, the concentration read from the reference curve must<br />
be multiplied by the appropriate dilution factor.<br />

Limitations of the Procedure<br />
1. <strong>Anti</strong>coagulants such as heparin, EDTA and citrate do not interfere with the test.<br />
2. Samples that are lipaemic, haemolytic, icteric or contain rheumatoid factors do not impair the<br />
test results.<br />
3. In the case of samples from pregnant women, no interference with the test result was<br />
observed.<br />
4. Samples containing the following potential sources of interference were checked in the test:<br />
<strong>HBs</strong> antigen, ANA as well as antibodies to HCV, HAV, HBC, HBe, EBV and CMV. With the<br />
samples used, no interference was observed with the test result.<br />
5. No interferences were observed with heat-treated samples (30 min, 56 °C).<br />
6. Serum from insufficiently coagulated blood, contain sodium azide or microbial contamination<br />
should not be used. Any particulate components in the sample (e.g. fibrin clots) should be<br />
removed before the test.<br />
7. When using thawed samples, ensure good homogenization of the material prior to use.<br />
8. The pack contains a matched set of reagents. Reagents must not be interchanged with other<br />
kits unless the reagents are from an identical lot (i.e. they must display the required 6-digit lot<br />
numbers printed on the pack and given in the enclosed barcode table). Washing Solution<br />
POD, Stopping Solution POD and Working Chromogen Solution are exceptions to this<br />
requirement. Note that the Working Chromogen Solution must first be prepared from<br />
matched components (do not use Chromogen TMB and Buffer/Substrat TMB from kits with a<br />
different number).<br />
9. Buffer/Substrate TMB, the Working Chromogen Solution and the Stopping Solution POD<br />
must not be allowed to come into contact with heavy metal ions or oxidizing substances (do<br />
not use pipettes with metal parts in contact with the liquid!). Do not perform the substrate<br />
reaction in the proximity of disinfectants containing hypochlorite. If the Working Chromogen<br />
Solution has spontaneously developed a blue colour before transferral into the test plate, this<br />
indicates that the solution is contaminated; in such cases prepare a fresh solution in a clean<br />
container. Skin contact with the above-mentioned solutions is to be avoided.<br />
10. The control sera have been prepared from native human sera. As a result, turbidity may<br />
occur but this has no effect on the test result.<br />
11. The test plate should be protected from vibration during the incubation phase (e.g. placed on<br />
a secured floatation aid, or in a non-circulating water bath); the wells of the plate must be in<br />
contact with the thermostated water. If stabilizers are used to prevent microbial contamination<br />
of the water, care must be taken that neither the surface of the test plate nor the wells come<br />
into contact with these solutions since such contamination can lead to unspecific reactions.<br />
12. With highly reactive samples the dye may precipitate during the stopping reaction. This does<br />
not interfere with the photometric evaluation.<br />
13. Dade Behring has validated use of these reagents on various analyzers to optimize product<br />
performance and meet product specifications. User defined modifications are not supported<br />
by Dade Behring as they may affect performance of the system and assay results. It is the<br />
responsibility of the user to validate modifications to these instructions or use of the reagents<br />
on analyzers other than those included in Dade Behring Application Sheets or these<br />
instructions for use.<br />

Specific Performance Characteristics<br />
Sensitivity and Specificity<br />
The results of the sensitivity and specificity studies are shown in Tables 2 + 3 (Appendix).<br />
The diagnostic sensitivity was established by investigating 515 anti-<strong>HBs</strong>-positive samples, and<br />
the sensitivity result was 100%. The reactivity of the test in respect to seroconversion samples /<br />
vaccination profiles was investigated using 41 profiles. Here it was found that, in detecting<br />
seroconversions/vaccination profiles, Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong> exhibited a degree of sensitivity<br />
comparable with other tests.<br />
The analytical sensitivity of the test was established to be < 8 IU/L which was determined by<br />
quantitative evaluation (standard curve) using the WHO Reference Preparation.<br />
For establishing the specificity of the test, a total of 4736 anti-<strong>HBs</strong>-negative blood donor samples<br />
were investigated and the result was 99.5 to 99.7%. Different values may be obtained in relation<br />
to sample population, test procedure, and other factors.<br />
Reproducibility<br />
The results of the intra-/inter-assay reproducibility study are listed in Table 4 (in the Appendix).<br />
These are exemplary data. Differing values may be well obtained depending on various factors<br />
such as procedure, etc.<br />

Anwendungsbereich<br />
Enzymimmunoassay zum qualitativen und quantitativen Nachweis von <strong>Anti</strong>körpern gegen Hepatitis<br />
B surface <strong>Anti</strong>gen (<strong>HBs</strong>Ag) in Serum und Plasma.<br />
Die Abarbeitung des Enzymimmunoassays erfolgt mit den ELISA Prozessoren BEP ® <strong>II</strong>, BEP ® <strong>II</strong>I<br />
und BEP ® 2000. Der Test wurde entwickelt für die Untersuchung von Einzelproben, nicht von<br />
gepoolten Proben. Das Produkt darf nur für in vitro diagnostische Zwecke angewendet werden.<br />
Diagnostische Bedeutung<br />
Ebenso wie das Hepatitis B surface <strong>Anti</strong>gen (<strong>HBs</strong>Ag) ist auch der gegen das Oberflächenprotein<br />
gerichtete <strong>Anti</strong>körper <strong>Anti</strong>-<strong>HBs</strong> ein wichtiger diagnostischer Parameter einer Infektion mit dem<br />
Hepatitis B Virus (HBV) 1, 2 .<br />
Während der Inkubationszeit und der akuten Phase einer HBV-Infektion sind <strong>Anti</strong>körper gegen das<br />
<strong>HBs</strong>Ag nicht nachweisbar. Die Immunität verleihenden <strong>Anti</strong>körper <strong>Anti</strong>-<strong>HBs</strong> erscheinen in 90 %<br />
aller Fälle erst in der späten Rekonvaleszenz ca. 3 bis 4 Monate nach Erkrankungsbeginn, wenn<br />
zirkulierendes <strong>HBs</strong>Ag bereits nicht mehr nachweisbar ist 3, 4 .<br />
Diese Serokonversion, d.h. der Übergang von <strong>Anti</strong>-<strong>HBs</strong>-negativ zu -positiv, stellt einen recht<br />
zuverlässigen Parameter für eine überstandene HBV-Infektion dar, zumal auch ca. 10 % von<br />
akut infizierten Patienten, bei denen in der Frühphase kein <strong>HBs</strong>Ag nachweisbar ist, später <strong>Anti</strong>-<br />
<strong>HBs</strong>-positiv werden. Daher eignet sich der <strong>Anti</strong>-<strong>HBs</strong>-Nachweis auch zur Diagnose von<br />
inapparent verlaufenden HBV-Infektionen 3, 4 .<br />
Da ein positiver Nachweis von <strong>Anti</strong>-<strong>HBs</strong> auf eine vorausgegangene Exposition mit diesem <strong>Anti</strong>gen<br />
entweder durch eine HBV-Infektion oder in Form eines Impfstoffes hinweist, ergeben sich<br />
als wichtigste Anwendungen der <strong>Anti</strong>-<strong>HBs</strong>-Bestimmung die folgenden Untersuchungen:<br />
a) Beurteilung der Rekonvaleszenz und in gewissem Umfang auch Prognose bei HBV-infizierten Patienten<br />
(Verlaufskontrolle).<br />
b) Serologische Untersuchungen im Rahmen von Impfprogrammen (Screening und Immunisierungskontrolle).<br />
c) Epidemiologische Erhebungen.<br />
Neben dem qualitativen <strong>Anti</strong>-<strong>HBs</strong>-Nachweis hat die quantitative Ermittlung von <strong>Anti</strong>-<strong>HBs</strong> unter<br />
dem Gesichtspunkt der aktiven Immunisierung eine eigene Bedeutung erlangt. Gemäß einer<br />
Empfehlung der WHO kann von einem Infektionsschutz nach einer Schutzimpfung dann ausgegangen<br />
werden, wenn sich im Serum oder Plasma eine <strong>Anti</strong>-<strong>HBs</strong>-Konzentration von > 10 IU/l<br />
nachweisen läßt. Es werden rechtzeitige Auffrischungsimpfungen empfohlen, um sicherzustellen,<br />
daß dieser Grenzwert nicht unterschritten wird 5, 6, 7 .<br />
Bei <strong>Anti</strong>-<strong>HBs</strong> Werten < 100 IU/l nach der Grundimmunisierung ist, gemäß den Impfempfehlungen<br />
der Ständigen Impfkommision des Robert-Koch-Institutes (Deutschland) (STIKO, Stand Oktober<br />
1995), eine Auffrischimpfung innerhalb eines Jahres indiziert.<br />
Unterstrichen wird die Notwendigkeit einer Quantifizierung ferner durch die Tatsache, daß die<br />
Dauer des Impfschutzes proportional zur erreichten <strong>Anti</strong>-<strong>HBs</strong>-Konzentration nach Impfung ist.<br />
Prinzip der Methode<br />
Bei Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong> handelt es sich um einen nach dem Sandwich-Prinzip aufgebauten<br />
Einschrittassay. Als Festphasen- und Konjugatantigen wird humanes <strong>HBs</strong>Ag eingesetzt.<br />
Peroxidase-markiertes <strong>HBs</strong>Ag bindet sich an die in der Probe enthaltenen <strong>HBs</strong>-spezifischen<br />
<strong>Anti</strong>körper. Diese binden sich an das an der Oberfläche der Mikrotitrationsplatte gebundene<br />
<strong>HBs</strong>Ag (<strong>Anti</strong>gen-Sandwich).<br />
Nach Entfernung der ungebundenen Bestandteile durch Waschen wird die gebundene Enzymaktivität<br />
des Konjugates bestimmt. Die enzymatische Umsetzung von Substrat und Chromogen<br />
(blaue Farbreaktion) wird durch Zusatz von Stopplösung POD unterbrochen (gelbe<br />
Farbreaktion). Die Farbintensität ist der in der Probe vorhandenen <strong>Anti</strong>körperkonzentration proportional.<br />
Die Quantifizierung erfolgt durch Berechnung nach der α-Methode oder durch Vergleich<br />
mit einer Standardkurve.<br />

Warnungen und Vorsichtsmaßnahmen<br />

Für die Herstellung wurden nur Spenden mit negativem Befund verwendet.<br />
Unabhängig davon sollten alle aus menschlichem Blut gewonnenen Materialien wegen nie<br />
auszuschließender Gefährdung durch Krankheitserreger mit angemessener Sorgfalt unter<br />
Einhaltung der bei Biogefährdung empfohlenen Sicherheitsmaßnahmen gehandhabt werden<br />

3. Das zur Herstellung der Testplatten und des POD-Konjugates verwendete <strong>HBs</strong>Ag wurde aus<br />
<strong>HBs</strong>Ag-positiven Human-Plasmen, die negativ hinsichtlich <strong>Anti</strong>-HCV, <strong>Anti</strong>-HIV1 und <strong>Anti</strong>-<br />
HIV2 befundet wurden, in hoher Reinheit isoliert und einem anerkannten<br />
Inaktivierungsverfahren unterworfen.<br />
4. Das Tragen von Untersuchungshandschuhen während der gesamten Testdurchführung wird<br />
angeraten.<br />
5. Für die Entsorgung fester infektiöser Materialien empfiehlt sich eine Autoklavierung von mindestens<br />
1 Stunde bei +121 °C. Alle abgesaugten Lösungen sind in zwei hintereinandergeschalteten<br />
Vorlagen zu sammeln. Die Vorlagen sollten ein Desinfektionsmittel enthalten, das<br />
geeignet ist, human-pathogene Viren zu inaktivieren. Die vom Hersteller angegebenen Konzentrationen<br />
und Einwirkungszeiten müssen beachtet werden.<br />
Vorbereitung der Reagenzien<br />
Alle Reagenzien und Proben vor Testbeginn auf +15 bis +25 °C erwärmen. Dabei die Testplatte<br />
nicht dem Behältnis entnehmen.<br />
Für die Testdurchführung nicht benötigte Riegel dem Halterahmen entnehmen und für die spätere<br />
Verwendung lagern (siehe Tabelle 1).<br />
Je Testplatte 20 ml Waschlösung POD mit destilliertem oder entionisiertem Wasser auf 400 ml<br />
verdünnen.<br />
Je Testplatte 1 ml Chromogen TMB mit 10 ml Puffer/Substrat TMB in der mitgelieferten Leerflasche<br />
verdünnen (Chromogen-Gebrauchslösung) und verschlossen unter Lichtschutz aufbewahren.<br />
Flasche nach Gebrauch sorgfältig mit destilliertem Wasser spülen.<br />
Wegen technisch bedingter Überfüllung ist das Überführen des Flascheninhalts von Chromogen<br />
TMB in die Abfüllung Puffer/Substrat TMB nicht zulässig.<br />
Vorverdünnung der Proben: Für die quantitative Testdurchführung sollten die Proben bei zu<br />
erwartenden <strong>Anti</strong>-<strong>HBs</strong>-Konzentrationen von ≥ 100 IU/l wie folgt verdünnt werden:<br />

20 µl + 180 µl <strong>Anti</strong>-<strong>HBs</strong>-Serum, negativ).<br />
Wenn die quantitative Testdurchführung mit einer Standardreihe durchgeführt wird, müssen von<br />
dem <strong>Anti</strong>-<strong>HBs</strong>-Serum 100 zusätzlich folgende Verdünnungen mit dem <strong>Anti</strong>-<strong>HBs</strong>-Serum, negativ<br />
hergestellt werden (gilt nicht für die Auswertung mit α-Methode):<br />

Haltbarkeit und Lagerungsbedingungen<br />
Ungeöffnet sind alle Bestandteile der Kombinationspackung Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong> bei einer<br />
Lagertemperatur von +2 bis +8 °C bis zu den auf den Etiketten angegebenen Daten verwendbar.<br />
Die Haltbarkeit und Lagerbedingungen der geöffneten bzw. gebrauchsverdünnten Reagenzien<br />
sind der Tabelle 1 in der Anlage zu entnehmen.<br />

Untersuchungsmaterial<br />
Zur Untersuchung können Einzelproben (Human-Seren oder -EDTA/Heparin/Citrat-Plasmen) verwendet<br />
werden, welche nach Standard-Labortechniken entnommen wurden. Die Proben sollen<br />
maximal 3 Tage bei 2 - 8 °C gelagert werden. Zur längeren Lagerung sind die Proben einzufrieren.<br />
Testdurchführung<br />
Testdurchführung mit BEP ® <strong>II</strong><br />
1. Ansatzschema: Benötigte Anzahl der Vertiefungen feststellen (= Anzahl der zu untersuchenden<br />
Proben plus 6 Vertiefungen für die Kontrollen).<br />
2. Proben-Dosierung:<br />
Qualitativer Test/Quantitativer Test (α - Methode):<br />
In 4 Vertiefungen (A1 bis D1) je 100 µl <strong>Anti</strong>-<strong>HBs</strong>-Serum, negativ, in eine Vertiefung (E1) 100 µl<br />
<strong>Anti</strong>-<strong>HBs</strong>-Serum 100, in die folgenden Vertiefungen je 100 µl Probe und am Ende der Serie<br />
bzw. Testplatte noch einmal 100 µl <strong>Anti</strong>-<strong>HBs</strong>-Serum 100 einpipettieren.<br />
Die Pipettiervorgänge müssen zügig innerhalb von 15 min pro Testplatte durchgeführt werden.<br />
Wichtig: Es ist unzulässig, zunächst das <strong>Anti</strong>-<strong>HBs</strong>-Serum 100 in die beiden vorgesehenen<br />
Positionen zu pipettieren und anschließend die Proben "dazwischenzuschieben".<br />
Quantitativer Test (Standardreihe):<br />
In 4 Vertiefungen je 100 µl <strong>Anti</strong>-<strong>HBs</strong>-Serum, negativ und anschließend in jeweils 2 Vertiefungen<br />
je 100 µl des <strong>Anti</strong>-HBS-Serums entsprechend 100, 50, 25, 10 und 5 IU/l pipettieren. In die<br />
folgenden Vertiefungen je 100 µl Probe (evtl. nach Bedarf vorverdünnt) dosieren.<br />
Die Konjugat-Dosierung unmittelbar nach Beendigung der Proben-Dosierung anschließen.<br />
3. Konjugat-Dosierung: In jede Vertiefung 25 µl des <strong>HBs</strong>Ag/POD-Konjugates (<strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong>) einfüllen.<br />
Anschließend mit Folie abkleben und die Platte unmittelbar nach Beendigung der<br />
Konjugat-Dosierung in den Inkubator stellen.<br />
4. Inkubation: 60 ± 2 min bei 37 ± 1 °C inkubieren, Waschvorgang unmittelbar anschließen.<br />
5. Waschen: Folie abziehen, alle Vertiefungen aussaugen und mit je ca. 0,3 ml Waschlösung<br />
4mal waschen. Nach Abschluß der Waschschritte die Substrat-Dosierung sofort anschließen,<br />
um ein Eintrocknen zu vermeiden.<br />
6. Substrat-Dosierung: In jede Vertiefung 100 µl der Chromogen-Gebrauchslösung einfüllen,<br />
Platte mit neuer Folie abkleben.<br />
7. Substrat-Inkubation: 30 ± 2 min bei +15 bis +25 °C lichtgeschützt inkubieren.<br />
8. Stoppreaktion: Die Folie entfernen. Je Vertiefung 100 µl Stopplösung POD zugeben, dabei<br />
den gleichen Zeittakt wie bei Punkt 6. einhalten.<br />
9. Messung: Innerhalb einer Stunde bei 450 nm photometrieren. Als Wellenlänge der Referenzmessung<br />
wird 650 nm (ggf. zwischen 615 und 690 nm) empfohlen.<br />
Testdurchführung mit BEP ® <strong>II</strong>I<br />
Bei der Abarbeitung mit BEP ® <strong>II</strong>I müssen die Testplatten einschließlich der Probendosierung<br />
(Punkt 1 und 2 der "Testduchführung BEP ® <strong>II</strong>") vorbereitet werden. Unmittelbar im Anschluss<br />
daran werden die Testplatten offen, d. h. nicht mit Folie abgeklebt, in den BEP ® <strong>II</strong>I eingegeben.<br />
Dabei ist zu beachten, dass teilbestückte Testplatten mit "Wasserriegeln" auf mindestens halbe<br />
Testplatten (6 Testriegel) zu ergänzen sind.Die anschließende Testabarbeitung erfolgt vollautomatisch<br />
(siehe BEP ® <strong>II</strong>I-Bedienungsanleitung).<br />
Die in der BEP ® <strong>II</strong>I Software eingestellten Inkubationszeiten können auf Grund der technischen<br />
Rahmenbedingungen (Gerätetaktung) von denen der BEP ® <strong>II</strong> Prozessierung abweichen, sind<br />
jedoch in der Kombination BEP ® <strong>II</strong>I/Enzygnost * validiert worden.<br />
Testdurchführung mit BEP ® 2000<br />
Die Probendosierung und anschließende Testabarbeitung erfolgt vollautomatisch im Gerät (siehe<br />
BEP ® 2000-Bedienungsanleitung).<br />

Testvalidierung<br />
Die Einzelwerte der Extinktionen für die Kontroll-Sera werden zur Berechnung der Mittelwerte<br />
eingesetzt, wenn:<br />

E <strong>Anti</strong>-<strong>HBs</strong>-Serum 100 ≥ 0,7<br />
Von den Extinktionswerten der negativen Kontrollen kann ein außerhalb der Spezifikation liegender<br />
Wert vernachlässigt werden. Die Extinktionswerte des <strong>Anti</strong>-<strong>HBs</strong>-Serum 100 müssen beide<br />
die Spezifikation erfüllen. Werden diese Bedingungen nicht erfüllt, ist der Test zu wiederholen.<br />
Für den quantitativen Test gelten folgende Validierungskriterien:<br />
Bei Verwendung der Standardreihe:<br />

Bei Verwendung der α - Methode:<br />
Die Einzelwerte der Extinktionen für das <strong>Anti</strong>-<strong>HBs</strong>-Serum 100 werden zur Berechnung des Mittelwertes<br />
eingesetzt wenn:<br />
untere Toleranzgrenze < E <strong>Anti</strong>-<strong>HBs</strong>-Serum 100 < obere Toleranzgrenze<br />
Die Werte für die untere und obere Toleranzgrenze sind in der beiliegenden Wertetabelle angegeben.<br />
Beide Extinktionswerte des <strong>Anti</strong>-<strong>HBs</strong>-Serum 100 müssen die Spezifikation erfüllen.<br />
Zusätzlich dürfen die Einzelwerte maximal 20 % vom Mittelwert dieser beiden Bestimmungen<br />
abweichen.<br />
Werden diese Bedingungen nicht erfüllt, ist der Test nur qualitativ auswertbar.<br />
Testauswertung<br />
Die Auswertungen erfolgen mit BEP ® 2000 und BEP ® <strong>II</strong>I automatisch. Bitte dazu die Bedienungsanleitungen<br />
heranziehen. Die nachfolgenden Kapitel sind bei Auswertung ohne Softwareunterstützung<br />
zu beachten.<br />
Qualitativer Test:<br />
Aus den Extinktionswerten der negativen Kontrollen wird der Mittelwert gebildet.<br />
Zur Grenzwertberechnung wird zum Extinktionsmittelwert der negativen Kontrollen ein Wert von<br />
0,08 addiert: – E neg. + 0,08 = Grenzwert (cut off)<br />
Nach den Kriterien des Tests werden die Untersuchungsproben wie folgt klassifiziert:<br />
1. E Probe < cut off = <strong>Anti</strong>-<strong>HBs</strong>-negativ<br />
2. E Probe ≥ cut off = <strong>Anti</strong>-<strong>HBs</strong>-positiv<br />
Quantitativer Test (α - Methode):<br />
Für jeden einzelnen Testplattenansatz wird ein Korrekturfaktor ermittelt, mit dem der gemessene<br />
Extinktionswert jeder Probe korrigiert wird (Meßwertkorrektur). Die korrigierten Meßwerte werden<br />
anschließend zur Berechnung der <strong>Anti</strong>körperaktivität eingesetzt (Ergebnisberechnung).<br />
Meßwertkorrektur<br />
Der Korrekturfaktor wird nach folgender Formel ermittelt:<br />

Bei mehreren Testplatten ist der Korrekturfaktor für jede Platte neu zu ermitteln und für die<br />
entsprechende Korrekturrechnung einzusetzen.<br />
Ergebnisberechnung<br />
Die korrigierten Meßwerte der Untersuchungsproben werden zur Berechnung der <strong>Anti</strong>körperaktivität<br />
in die nachfolgende Formel eingesetzt (α - Methode):<br />

Die Angabe der <strong>Anti</strong>körperaktivität in „IU/l“ bezieht sich auf das internationale HBV-<br />
Referenzserum der WHO.<br />

Wurde die Untersuchungsprobe zur Bewertung vorverdünnt (z. B. 1:11), so ist die errechnete<br />
<strong>Anti</strong>körperaktivität (nicht das Meßsignal) mit 11 zu multiplizieren.<br />
Quantitativer Test (Standardreihe):<br />
Zur quantitativen Auswertung werden die Extinktionsmittelwerte aus den Doppelbestimmungen<br />
für die Standardreihe mit 5, 10, 25, 50 und 100 IU/l gebildet, und auf doppellogarithmischem<br />
Papier wird eine Bezugskurve erstellt. Abszisse: <strong>Anti</strong>-<strong>HBs</strong> Konzentration 5 bis 100 IU/l. Ordinate:<br />
Extinktion 0,05 bis 2,0. Aus der Bezugskurve werden anhand der einzelnen Extinktionen die<br />
<strong>Anti</strong>-<strong>HBs</strong> Konzentrationen der Proben abgelesen.<br />
Wurden die Proben verdünnt eingesetzt, so muß die auf der Bezugskurve abgelesene Konzentration<br />
mit dem Verdünnungsfaktor multipliziert werden.<br />
Einschränkungen der Testdurchführung<br />
1. <strong>Anti</strong>koagulantien wie Heparin, EDTA und Citrat beeinflussen das Testergebnis nicht.<br />
2. Lipämische, rheumafaktorhaltige, hämolytische oder ikterische Proben führen zu keiner<br />
Testbeeinträchtigung.<br />
3. Mit Proben von Schwangeren wurde keine Beeinflussung des Testergebnisses beobachtet.<br />
4. Proben mit folgenden potentiell störenden Substanzen wurden mit dem Test überprüft: <strong>HBs</strong>-<br />
<strong>Anti</strong>gen, ANA sowie <strong>Anti</strong>körper gegen HCV, HAV, HBC, HBe, EBV und CMV. Mit den eingesetzten<br />
Proben wurde keine Beeinflussung des Testergebnisses beobachtet.<br />
5. Bei hitzebehandelten Proben (30 min, 56 °C) wurden keine Störungen beobachtet.<br />
6. Ungenügend geronnene Seren, natriumazidhaltiges Probenmaterial und mikrobiell kontaminierte<br />
Untersuchungsproben sollten nicht eingesetzt werden. Eventuell vorhandene partikuläre<br />
Komponenten (z. B. Fibrin-Gerinnsel) sollten vor der Testdurchführung entfernt werden.<br />
7. Bei aufgetauten Proben ist auf eine gute Homogenisierung des Materials zu achten.<br />

8. Die Reagenzien (ausgenommen Waschlösung POD, Stopplösung POD und die aus den<br />
chargengebundenen Reagenzien Chromogen TMB und Puffer/Substrat TMB hergestellte<br />
Chromogen-Gebrauchslösung) sind nur chargengebunden zu verwenden, d.h. nur in der<br />
Kombination der einzelnen 6-ziffrigen Chargen-Bezeichnungen, die auf der Packung aufgedruckt<br />
bzw. der separat beigepackten Barcode-Tabelle zu entnehmen sind.<br />
9. Puffer/Substrat TMB, Chromogen-Gebrauchslösung und Stopplösung POD dürfen nicht in<br />
Kontakt mit Schwermetallionen oder oxidierenden Substanzen kommen (keine Pipetten mit<br />
flüssigkeitsführenden Metallteilen verwenden). Die Substratreaktionen nicht in der Nähe von<br />
Hypochlorit-haltigen Desinfektionsmitteln durchführen. Eine spontane Blaufärbung der<br />
Chromogen-Gebrauchslösung vor der Übertragung in die Testplatte deutet auf Kontamination<br />
hin; eine frische Lösung ist in einem sauberen Gefäß anzusetzen. Hautkontakt mit den<br />
vorgenannten Lösungen ist zu vermeiden.<br />
10. Die Kontrollsera sind unter Verwendung nativer Humansera hergestellt. Daher können Trübungen<br />
auftreten, die das Testergebnis jedoch nicht beeinflussen.<br />
11. Die Testplatte soll während der Inkubation ruhig liegen (z.B. auf fixierter Schwimmhilfe oder<br />
im nicht zirkulierenden Wasserbad); die Kavitäten sind dabei in Kontakt mit dem temperierten<br />
Wasser. Werden Stabilisatoren zur Verhinderung der Verkeimung des Wassers verwendet,<br />
so ist sorgfältig darauf zu achten, daß weder die Testplattenoberfläche noch die Näpfchen<br />
mit diesen Lösungen in Kontakt kommen, da dadurch unspezifische Reaktionen hervorgerufen<br />
werden könnten.<br />
12. Bei stark reaktiven Proben kann bei der Stoppreaktion der Farbstoff ausfallen. Die photometrische<br />
Auswertung wird dadurch nicht beeinflußt.<br />
13. Dade Behring hat den Einsatz dieser Reagenzien auf verschiedenen Analysengeräten auf<br />
optimale Produktleistung und Einhaltung der Produktspezifikationen überprüft. Vom Benutzer<br />
vorgenommene Änderungen werden von Dade Behring nicht unterstützt, da sie die Leistung<br />
des Systems und die Testergebnisse beeinflussen können. Es liegt in der Verantwortung<br />
des Benutzers, Änderungen an diesen Anleitungen oder die Verwendung dieser Reagenzien<br />
auf anderen als in den Applikationsvorschriften von Dade Behring oder diesen Gebrauchsanweisungen<br />
genannten Analysengeräten zu validieren.<br />
14. Resultate dieses Tests sollten stets in Verbindung mit der Vorgeschichte des Patienten, dem<br />
klinischen Bild und anderen Untersuchungsergebnissen interpretiert werden.<br />
Leistungsmerkmale des Tests<br />
Sensitivität und Spezifität<br />
Die Ergebnisse zur Prüfung der Sensitivität und Spezifität sind in den Tabellen 2 + 3 (im Anhang)<br />
zusammengefaßt.<br />
Bei der Ermittlung der diagnostischen Sensitivität wurden insgesamt 515 <strong>Anti</strong>-<strong>HBs</strong>-positive Proben<br />
untersucht und eine Sensitivität von 100 % ermittelt. Die Reaktivität des Testes bei<br />
Serokonversionsproben/Impfverläufen wurde anhand von 41 Verläufen untersucht. Dabei zeigte<br />
sich, daß Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong> eine Sensitivität bezüglich der Erkennung von Serokonversionen/<br />
Impfverläufen aufweist, die der Empfindlichkeit vergleichbarer Teste entspricht.<br />
Die analytische Sensitivität wurde bei quantitativer Auswertung (Standardkurve) am WHO-Referenz-Präparat<br />
mit < 8 IU/l ermittelt.<br />
Bei der Ermittlung der Spezifität wurden insgesamt 4736 <strong>Anti</strong>-<strong>HBs</strong>-negative Blutspendeproben<br />
untersucht und eine Spezifität von 99,5 bis 99,7 % ermittelt. Bedingt durch das Untersuchungskollektiv,<br />
die Testdurchführung u.a. sind abweichende Werte möglich.<br />
Reproduzierbarkeit<br />
Die Ergebnisse zur Intra/Inter-assay-Reproduzierbarkeit sind in der Tabelle 4 (im Anhang) zusammengefaßt.<br />
Es handelt sich hierbei um beispielhaft ermittelte Daten. Abhängig von der Testdurchführung<br />
u.a. sind durchaus abweichende Werte möglich.<br />

Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong><br />
Domaine d’utilisation<br />
Test immunoenzymatique qui permet la mise en évidence qualitative et quantitative des<br />
anticorps anti-antigène de surface de l’hépatite B (Ag<strong>HBs</strong>) dans le sérum ou le plasma.<br />
Le test immunoenzymatique s’effectue à l’aide des ELISA Processors BEP ® <strong>II</strong>, BEP ® <strong>II</strong>I et BEP ®<br />
2000. Il a été mis au point pour l’analyse d’échantillons individuels et non pas d’échantillons<br />
poolés. Les réactifs ne peuvent être utilisés qu’à des fins de diagnostic in vitro.<br />
Intérêt diagnostique<br />
Tout comme l’antigène de surface de l’hépatite B (Ag<strong>HBs</strong>), l’anticorps anti-<strong>HBs</strong> dirigé contre la<br />
protéine de surface constitue également un marqueur biologique important de l’infection par le<br />
virus de l’hépatite B (HBV) 1, 2 .<br />
Pendant le temps d’incubation et la phase aiguë d’une infection à HBV, les anticorps anti-<strong>HBs</strong> ne<br />
peuvent être révélés. Les anticorps anti-<strong>HBs</strong> qui confèrent l’immunité n’apparaissent dans 90%<br />
des cas que dans la période de convalescence tardive, env. 3 à 4 mois après le début de la<br />
maladie, alors que l’Ag<strong>HBs</strong> circulant ne peut déjà plus être mis en évidence 3, 4 .<br />
Cette séroconversion, c’est-à-dire le passage d’un état anti-<strong>HBs</strong>-négatif à un état anti-<strong>HBs</strong>positif,<br />
constitue un paramètre très fiable pour confirmer la guérison d’une infection à HBV, et ce<br />
d’autant plus qu’env. 10% des patients infectés de façon aiguë et chez qui l’Ag<strong>HBs</strong> n’a pu être<br />
mis en évidence dans la phase précoce, deviennent plus tard anti-<strong>HBs</strong>-positifs. Aussi la mise en<br />
évidence d’anti-<strong>HBs</strong> permet-elle aussi le diagnostic d’infections à HBV asymptomatiques 3, 4 .<br />
Dans la mesure où la mise en évidence d’anti-<strong>HBs</strong> indique un contact ancien avec cet antigène,<br />
soit par une infection à HBV soit par un vaccin, voici les principales indications d’un dosage<br />
d’anti-<strong>HBs</strong> :<br />
a) évaluation de la convalescence et, dans certains cas, pronostic pour les patients infectés par<br />
l’HBV (contrôle d’évolution),<br />
b) études sérologiques dans le cadre de programmes de vaccination (dépistage et contrôle<br />
d’immunisation),<br />
c) sondages épidémiologiques.<br />
Outre la mise en évidence qualitative de l’anti-<strong>HBs</strong>, son dosage quantitatif a acquis un intérêt<br />
particulier pour l’immunisation active. Conformément à une recommandation de l’OMS, une personne<br />
vaccinée contre l’hépatite peut être considérée comme protégée d’une infection si son taux<br />
sérique ou plasmatique d’anti-<strong>HBs</strong> est > 10 UI/ml. Il est recommandé d’effectuer des injections de<br />
rappel suffisamment tôt pour ne pas passer en dessous de cette valeur-seuil (10 UI/l) 5, 6, 7 .<br />
Les patients dont les valeurs d’anti-<strong>HBs</strong> sont trouvées < 100 UI/l après l’immunisation de base<br />
doivent subir une injection de rappel dans l’année qui suit (recommandations de la Commission<br />
permanente de Vaccination de l’Institut Paul-Ehrlich, Allemagne, Octobre 1995).<br />
La nécessité d’une quantification est également renforcée par le fait que la durée de la<br />
protection est proportionnelle à la concentration d’anti-<strong>HBs</strong> mesurée après la vaccination.<br />
Principe de la méthode<br />
L’Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong> est un test en une étape basé sur le principe sandwich. L’antigène de la<br />
phase solide et l’antigène du conjugué sont de l’Ag<strong>HBs</strong> humain.<br />
L’Ag<strong>HBs</strong> marqué à la peroxydase se lie aux anticorps anti-<strong>HBs</strong>-spécifiques présents dans<br />
l’échantillon. Ceux-ci se lient à l’Ag<strong>HBs</strong> fixé à la surface de la plaque de microtitration (antigène<br />
sandwich).<br />
Après élimination par lavage des éléments non liés, on mesure l’activité enzymatique liée du<br />
conjugué. La transformation enzymatique du substrat et du chromogène (réaction colorée bleue)<br />
est interrompue par l’addition de la Solution d’arrêt POD (coloration jaune). L’intensité de la<br />
coloration est proportionnelle à la concentration d’anticorps dans l’échantillon. La quantification<br />
se fait par un calcul selon l’α-méthode ou par comparaison avec une courbe d’étalonnage.<br />

Mises en garde et précautions d’emploi<br />
1. Ne doit être employé que pour un usage in vitro<br />
2. Tout don de sang prévu pour la préparation des contrôles de l’Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong> est<br />
soumis à une recherche de l'Ag<strong>HBs</strong>, d’anti-VHC, d’anti-VIH1 et d’anti-VIH2. Seuls les dons<br />
trouvés négatifs sont utilisés.<br />
Indépendamment de cela, toute préparation obtenue à partir de sang humain doit être<br />
manipulée avec les précautions nécessaires en cas de risque biologique, dans la mesure où<br />
on ne peut exclure totalement tout risque d’infection 8 .<br />
3. L'Ag<strong>HBs</strong> hautement purifié utilisé pour la préparation des plaques-tests et du Conjugué POD<br />
est isolé à partir de plasmas humains positifs en Ag<strong>HBs</strong>, et trouvés négatifs en anti-VHC, anti-<br />
VIH1 et anti-VIH2. Il est ensuite inactivé selon un procédé reconnu.<br />
4. Il est recommandé de porter des gants de protection pendant toute la réalisation du test.<br />
5. Pour décontaminer le matériel de laboratoire infecté, l’autoclaver pendant au moins 1 heure à<br />
+121°C. Toutes les solutions aspirées doivent être récoltées dans deux récipients reliés l’un à<br />
l’autre et contenant chacun un désinfectant spécialement conçu pour inactiver les virus<br />
pathogènes pour l’homme. Respecter les concentrations et temps d’incubation indiqués par<br />
le fabricant.<br />
Préparation des réactifs<br />
Porter tous les réactifs et échantillons à +15/+25°C avant le début du test, sans sortir la plaquetest<br />
de son emballage.<br />
Sortir du cadre les barrettes non utilisées dans le test, et les conserver pour un usage ultérieur<br />
(cf. Tabl. 1).<br />
Pour une plaque, diluer 20 ml de solution de lavage POD avec de l’eau distillée ou désionisée<br />
en complétant à 400 ml.<br />
Pour une plaque, diluer 1 ml de Chromogène TMB avec 10 ml de Tampon/substrat TMB dans<br />
le flacon plastique vide inclus dans le coffret (solution d’emploi du chromogène) et la conserver<br />
dans le flacon fermé à l’abri de la lumière. Après emploi, bien rincer le flacon avec de l’eau<br />
distillée.<br />
Pour des raisons de capacité de flacon, il est impossible de verser la totalité du contenu d’un<br />
flacon de Chromogène TMB dans un flacon de Tampon/substrat TMB.<br />
Prédilution des échantillons : pour une réalisation quantitative du test, et si on s’attend à des<br />
concentrations d’anti-<strong>HBs</strong> ≥ 100 UI/l, diluer les échantillons comme suit :<br />
Jusqu’à 1 000 UI/l : dilution au 1/10 (par ex. 20 µl d’échantillon + 180 µl de Sérum <strong>Anti</strong>-<strong>HBs</strong> négatif).<br />
Jusqu’à 10 000 UI/l : dilution au 1/100 (par ex. 20 µl d’échantillon + 180 µl de Sérum <strong>Anti</strong>-<strong>HBs</strong> négatif;<br />
puis 20 µl de cette dilution + 180 µl de Sérum <strong>Anti</strong>-<strong>HBs</strong> négatif).<br />
Jusqu’à 100 000 UI/l : dilution au 1/1000 (par ex. 20 µl d’échantillon + 180 µl de Sérum <strong>Anti</strong>-<strong>HBs</strong> négatif;<br />
puis 20 µl de cette dilution + 180 µl de Sérum <strong>Anti</strong>-<strong>HBs</strong> négatif;<br />
puis 20 µl de cette dilution + 180 µl de Sérum <strong>Anti</strong>-<strong>HBs</strong> négatif).<br />
Si la réalisation quantitative du test est faite à partir d’une série standard, préparer les dilutions<br />
complémentaires suivantes du Sérum <strong>Anti</strong>-<strong>HBs</strong> 100 à l’aide du Sérum anti-<strong>HBs</strong> négatif (ne pas<br />
le faire pour une exploitation selon l’α-méthode) :<br />
50 UI/l: dilution au 1/2 (par ex. 200 µl de Sérum anti-<strong>HBs</strong> 100 + 200 µl de Sérum anti-<strong>HBs</strong> négatif)<br />
25 UI/l: dilution au 1/4 (par ex. 150 µl des 50 UI/l du Sérum + 150 µl de Sérum anti-<strong>HBs</strong> négatif)<br />

Echantillons à tester<br />
Utiliser des échantillons (sérums humains ou plasmas citratés, héparinés ou prélevés sur EDTA)<br />
obtenus selon les techniques standard des laboratoires. Les échantillons peuvent être<br />
conservés 3 jours maximum à +2/+8°C. Pour une conservation plus longue, les congeler.<br />
Réalisation du test<br />
Réalisation du test sur le BEP ® <strong>II</strong><br />
1. Schéma de distribution : déterminer le nombre de cupules nécessaires (= nombre<br />
d’échantillons à tester, plus 6 cupules pour les contrôles).<br />
2. Distribution des contrôles et échantillons :<br />
Test qualitatif/Test quantitatif (α - méthode) :<br />
Distribuer dans 4 cupules (A1 à D1) 100 µl de Sérum <strong>Anti</strong>-<strong>HBs</strong> négatif, dans 1 cupule (E1)<br />
100 µl de Sérum <strong>Anti</strong>-<strong>HBs</strong> 100, dans les cupules suivantes 100 µl de chacun des échantillons,<br />
puis en fin de série ou en fin de plaque de nouveau 100 µl de Sérum <strong>Anti</strong>-<strong>HBs</strong> 100.<br />
Les pipetages doivent être enchaînés, et ne pas durer plus de 15 min par plaque.<br />
Important : il n’est pas possible de distribuer d’abord le Sérum <strong>Anti</strong>-<strong>HBs</strong> 100 dans les 2<br />
cupules prévues, puis les échantillons entre celles-ci.<br />
Test quantitatif (chaque concentration du standard) :<br />
distribuer 100 µl de Sérum anti-<strong>HBs</strong> négatif dans 4 cupules, 100 µl de chaque dilution du<br />
sérum correspondant à 100, 50, 25, 10 et 5 UI/l dans 2 cupules, puis 100 µl de chacun des<br />
échantillons (éventuellement prédilués) dans les cupules suivantes.<br />
Enchaîner la distribution du conjugué immédiatement après celle des échantillons.<br />
3. Distribution du conjugué : distribuer dans chaque cupule 25 µl de Conjugué Ag<strong>HBs</strong>/POD<br />
(<strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong>).<br />
Couvrir ensuite d’une feuille adhésive et placer la plaque dans l’incubateur immédiatement<br />
après la distribution du conjugué.<br />
4. Incubation : laisser incuber 60 ± 2 min à +37 ±1°C, et enchaîner immédiatement le<br />
processus de lavage.<br />
5. Lavage : retirer la feuille adhésive, aspirer le contenu de toutes les cupules, et distribuer dans<br />
chacune d’elles env. 0,3 ml de Solution de lavage. Laver 4 fois. Dès la fin du processus de<br />
lavage, distribuer immédiatement le substrat afin d’éviter tout dessèchement.<br />
6. Distribution du substrat : ajouter dans chaque cupule 100 µl de la solution d’emploi du<br />
chromogène, et couvrir d’une nouvelle feuille adhésive.<br />
7. Incubation du substrat : laisser incuber 30 ± 2 minutes à +15/+25°C à l’abri de la lumière.<br />
8. Arrêt de la réaction : retirer la feuille adhésive et ajouter dans chaque cupule 100 µl de<br />
Solution d’arrêt POD en respectant le même rythme qu’en 6.<br />
9. Mesure : faire une mesure photométrique dans l’heure qui suit à 450 nm. La longueur d’onde<br />
de référence doit être de 650 nm (comprise entre 615 et 690 nm).<br />

Réalisation du test sur le BEP ® <strong>II</strong>I<br />
Pour une utilisation sur le BEP ® <strong>II</strong>I, les plaques-tests doivent être préparées jusqu’y compris la<br />
distribution des échantillons (points 1 et 2 du paragraphe « Réalisation du test sur le BEP ® <strong>II</strong> »).<br />
Immédiatement après cette étape, placer la plaque ouverte, c’est-à-dire non recouverte d’une<br />
feuille adhésive, dans le BEP ® <strong>II</strong>I. Si la plaque n’est pas totalement utilisée, la compléter au<br />
moins pour moitié (6 barrettes) avec des barrettes remplies d’eau. La suite du test est ensuite<br />
effectuée entièrement automatiquement (cf. Manuel d’utilisation du BEP ® <strong>II</strong>I).<br />
Les temps d’incubation prévus dans le logiciel du BEP ® <strong>II</strong>I peuvent, du fait de conditions<br />
techniques (cadence de l’appareil), diverger par rapport à ceux du BEP ® <strong>II</strong>. Ils ont toutefois été<br />
validés dans la combinaison BEP ® <strong>II</strong>I/Enzygnost * .<br />
Réalisation du test sur le BEP ® 2000<br />
La distribution des échantillons ainsi que toutes les étapes suivantes sont effectuées entièrement<br />
automatiquement par l’appareil (cf. Manuel d’utilisation du BEP ® 2000).<br />
Validation du test<br />
Calculer les valeurs moyennes des sérums de contrôle à partir des différentes valeurs mesurées si :<br />
-0,010 ≤ D.O. nég. ≤ 0,120<br />
D.O. Sérum <strong>Anti</strong>-<strong>HBs</strong> 100 ≥ 0,7<br />
Pour le contrôle négatif, une valeur sortant du domaine indiqué peut être négligée.<br />
Pour le Sérum <strong>Anti</strong>-<strong>HBs</strong> 100, les deux valeurs obtenues doivent se trouver dans le domaine<br />
indiqué.<br />
Si ces critères ne sont pas remplis, le test doit être recommencé.<br />
Pour le test quantitatif, les critères de validation sont les suivants :<br />
Avec une série standard :<br />

Avec l'α - méthode :<br />
les valeurs de densité optique obtenues pour le Sérum <strong>Anti</strong>-<strong>HBs</strong> 100 peuvent être utilisées pour le<br />
calcul de la valeur moyenne si :<br />
limite inférieure < D.O. Sérum <strong>Anti</strong>-<strong>HBs</strong> 100 < limite supérieure<br />
Les valeurs des limites inférieure et supérieure sont indiquées dans le tableau des valeurs ci-joint.<br />
Chacune des deux valeurs de densité optique du Sérum <strong>Anti</strong>-<strong>HBs</strong> 100 doivent répondre au critère.<br />
De plus, chacune des valeurs ne peut varier de plus 20% par rapport à la valeur moyenne des<br />
deux dosages.<br />
Si ces conditions ne sont pas remplies, seule une exploitation qualitative du test est possible.<br />
Exploitation du test<br />
Sur le BEP ® 2000 et le BEP ® <strong>II</strong>I, le calcul des résultats se fait automatiquement. Suivre le<br />
déroulement du manuel d’utilisation. Le protocole indiqué ci-après permet l’exploitation des<br />
résultats sans aide de logiciel.<br />
Test qualitatif :<br />
Calculer la valeur moyenne des densités optiques du contrôle négatif.<br />
Pour obtenir la valeur-seuil, ajouter 0,08 à la valeur moyenne du contrôle négatif :<br />

Selon les critères du test, les échantillons sont classés comme suit :<br />

Test quantitatif (α - méthode):<br />
Déterminer pour chaque plaque un facteur de correction qui permettra de corriger les valeurs<br />
obtenues pour chacun des échantillons (correction de la valeur mesurée). Les valeurs corrigées<br />
sont ensuite utilisées pour le calcul de l’activité en anticorps (calcul du résultat).<br />
Correction de la valeur mesurée<br />
Le facteur de correction est déterminé selon la formule suivante :<br />

La valeur nominale est indiquée dans le tableau des valeurs joint à chaque coffret. Multiplier les<br />
densités optiques des échantillons par le facteur de correction. Utiliser ensuite les valeurs<br />
corrigées ainsi obtenues pour le calcul du résultat.<br />
Si on utilise plusieurs plaques, déterminer un facteur de correction pour chaque plaque, et<br />
l’utiliser pour le calcul de la correction.<br />
Calcul du résultat<br />
Pour le calcul de l’activité en anticorps, utiliser la formule suivante en prenant les valeurs<br />
mesurées corrigées des échantillons (α-méthode) :<br />
log 10 mUI/l = α . D.O. β<br />
Les constantes α et β, qui varient selon les lots, sont indiquées dans le tableau des valeurs cijoint.<br />
L’activité en anticorps, exprimée en «UI/l», se réfère à l'immunoglobuline anti-<strong>HBs</strong> de<br />
référence internationale de l’OMS.<br />
Attention : pour le calcul, ne pas utiliser :<br />
- les valeurs mesurées corrigées < cut off<br />
- les valeurs mesurées non corrigées > 2,5<br />

Si l’échantillon a été prédilué (par ex. au 1/11), multiplier l’activité en anticorps calculée (et non<br />
pas le signal de mesure) par 11.<br />
Test quantitatif (de chaque concentration du standard) :<br />
Pour l’exploitation quantitative, calculer la valeur moyenne de chaque concentration du standard<br />
(5, 10, 25, 50 et 100 UI/l) testée en double, et tracer une courbe d’étalonnage sur papier<br />
bilogarithmique. Abscisse: concentrations d’anti-<strong>HBs</strong> de 5 à 100 UI/l. Ordonnée: densités<br />
optiques de 0,05 à 2,0. La courbe d’étalonnage permet de lire les concentrations en anti-<strong>HBs</strong><br />
des échantillons à partir des densités optiques obtenues.<br />
Si les échantillons ont été testés dilués, multiplier la concentration lue sur la courbe d’étalonnage<br />
par le facteur de dilution.<br />

Limites du test<br />
1. Les anticoagulants, comme l’héparine, l’EDTA ou le citrate, n’influencent pas le résultat du test.<br />
2. Les échantillons lipémiques, hémolytiques, contenant des facteurs rhumatoïdes ou ictériques<br />
ne perturbent pas le test.<br />
3. Les échantillons de femmes enceintes n‘ont entraîné aucune influence sur les résultats du test.<br />
4. Des tests ont été effectués avec des échantillons contenant les substances suivantes<br />
pouvant entraîner des interférences : antigène <strong>HBs</strong>, ANA, et anticorps anti-VHC, anti-VHA,<br />
anti-HBc, anti-HBe, anti-EBV et anti-CMV. Avec ces échantillons, aucune interférence n‘a<br />
été observée sur les résultats du test.<br />
5. On n’a pas observé de perturbations suite à l’utilisation d’échantillons traités à la chaleur (30<br />
min à +56°C).<br />
6. Ne pas utiliser de sérums insuffisamment coagulés ni d’échantillons contenant de l’azide de<br />
sodium ou contaminés. Les particules éventuellement présentes (par ex. des caillots de<br />
fibrine) doivent être éliminées avant le test.<br />
7. Si on utilise des échantillons décongelés, bien veiller à leur homogénéisation.<br />
8. Les réactifs (à l’exception de la Solution de lavage POD, de la Solution d’arrêt POD et de la<br />
solution d’emploi du chromogène obtenue à partir des lots de réactifs indiqués pour le<br />
Chromogène TMB et le Tampon/substat TMB) ne peuvent être utilisés que selon la<br />
combinaison des numéros de lots à 6 chiffres indiqués sur le coffret et dans le tableau des<br />
codes à barres joint.<br />
9. Le Tampon/substrat TMB, la solution d’emploi du chromogène et la Solution d’arrêt POD ne<br />
doivent pas entrer en contact avec des ions de métaux lourds ni des substances oxydantes<br />
(ne pas utiliser de pipettes à parties métalliques). La réaction du substrat ne doit pas être<br />
effectuée à proximité de désinfectants contenant de l’eau de Javel. Une coloration bleue<br />
spontanée de la solution d’emploi du chromogène avant son transfert dans la plaque indique<br />
une contamination ; préparer une nouvelle solution dans un récipient propre. Éviter tout<br />
contact de la peau avec les solutions sus-mentionnées.<br />
10. Les sérums de contrôle sont obtenus à partir de sérums humains natifs. Ils peuvent donc<br />
devenir trouble, sans que cela ait d’influence sur le résultat du test.<br />
11. La plaque doit rester immobile pendant toute l’incubation (la placer par ex. sur un support<br />
fixe ou dans un bain-marie sans circulation d’eau) ; le fond des cupules doit être en contact<br />
avec l’eau thermostatée. Si l’eau contient des stabilisateurs pour éviter une contamination,<br />
bien veiller à ce que ni la surface supérieure de la plaque ni l’intérieur des cupules n’entrent<br />
en contact avec cette eau, au risque d’obtenir des réactions non-spécifiques.<br />
12. En cas d’échantillon fortement réactif, il peut y avoir précipitation lors de l’addition de la<br />
Solution d’arrêt et du virage de la coloration, sans que cela ait d’influence sur l’exploitation<br />
photométrique.<br />
13. Dade Behring a validé l’utilisation de ces réactifs sur plusieurs analyseurs afin d’optimiser<br />
les performances du produit et répondre à ses spécifications. Les modifications apportées<br />
par l’utilisateur ne sont pas sous la responsabilité de Dade Behring dans la mesure où elles<br />
peuvent affecter les performances du système et les résultats des dosages. Il est de la<br />
responsabilité de l’utilisateur de valider toutes modifications apportées à ces instructions ou<br />
à l’utilisation des réactifs sur les analyseurs autres que ceux mentionnés dans les protocoles<br />
d’application Dade Behring ou dans la présente notice d’utilisation.<br />
14. Les résultats de ce test doivent toujours être interprétés en rapport avec les antécédents<br />
médicaux du patient, les signes cliniques et autres constatations.<br />
Caractéristiques du test<br />
Sensibilité et spécificité<br />
Les résultats de l’étude de sensibilité et de spécificité sont résumés dans les Tableaux 2 et 3 en<br />
annexe.<br />
Pour déterminer la sensibilité diagnostique, 515 échantillons anti-<strong>HBs</strong>-positifs ont été testés,<br />
donnant une sensibilité de 100%. La réactivité du test vis-à-vis d’échantillons en cours de<br />
séroconversion ou après une vaccination a été étudiée sur 41 échantillons. Dans ce domaine,<br />
Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong> a montré une sensibilité équivalente à celle des tests comparables.<br />

La sensibilité analytique a été obtenue par exploitation quantitative (courbe standard) d’après la<br />
préparation de référence de l’OMS ayant une concentration < 8 UI/l.<br />
Pour déterminer la spécificité, 4736 dons de sang anti-<strong>HBs</strong>-négatifs ont été testés, donnant une<br />
spécificité comprise entre 99,5 et 99,7%. Selon le collectif étudié, la réalisation du test et<br />
d’autres paramètres, on peut obtenir des valeurs divergentes.<br />
Reproductibilité<br />
Les résultats de l’étude de reproductibilité/répétabilité sont résumés dans le Tableau 4 en<br />
annexe. Il s’agit de données indicatives. Des différences dans la réalisation du test par exemple<br />
peuvent entraîner des valeurs divergentes.<br />

Settori d’impiego<br />
Metodo immunoenzimatico per l’identificazione qualitativa e la determinazione quantitativa degli<br />
anticorpi contro l’antigene di superficie dell’epatite B nel siero e nel plasma.<br />
L’esecuzione del test immunoenzimatico avviene su processore ELISA BEP ® <strong>II</strong>, BEP ® <strong>II</strong>I e BEP ®<br />
2000. Il test è stato sviluppato per l’analisi di campioni singoli e non per pool di campioni. Il<br />
prodotto deve essere impiegato solo per scopi diagnostici in vitro.<br />
Significato diagnostica<br />
Come l’antigene di superficie dell’epatite B (<strong>HBs</strong>Ag), anche l’anticorpo anti-<strong>HBs</strong> diretto contro la<br />
proteina di superficie è un parametro importante per la diagnosi di infezione da virus dell’epatite<br />
B (HBV) 1, 2 .<br />
Durante il periodo di incubazione e nella fase acuta dell’infezione da HBV, gli anticorpi anti-<br />
<strong>HBs</strong>Ag non sono evidenziabili. Gli anticorpi anti-<strong>HBs</strong>, che danno l’immunità, compaiono nel 90%<br />
dei casi solo nella tarda convalescenza circa 3 o 4 mesi dopo l’insorgenza della malattia, quando<br />
l’<strong>HBs</strong>Ag circolante non è più evidenziabile 3, 4 .<br />
Questa sieroconversione, cioè il passaggio dell’anti-<strong>HBs</strong> da negativo a positivo, rappresenta un<br />
parametro molto attendibile per la diagnosi di una infezione da HBV pregressa, tanto più che<br />
circa il 10% dei pazienti con infezione acuta, per i quali l’<strong>HBs</strong>Ag nella fase iniziale non è<br />
rilevabile, diventa successivamente anti-<strong>HBs</strong> positiva. L’identificazione dell’anti-<strong>HBs</strong> è adatto<br />
anche per la diagnosi delle infezioni subcliniche da HBV 3, 4 .<br />
La dimostrazione degli anticorpi anti-<strong>HBs</strong> indica una esposizione precedente con questo<br />
antigene a causa di una infezione da HBV o in seguito a vaccinazione, ed è perciò importante<br />
nelle seguenti ricerche:<br />
a) valutazione della convalescenza, e in una certa misura, anche prognosi per pazienti con<br />
infezione da HBV (controllo del decorso)<br />
b) ricerca sierologica nel contesto dei programmi di vaccinazione (screening e controllo dello<br />
stato di immunizzazione)<br />
c) studi epidemiologici<br />
Oltre alla identificazione qualitativa dell’anti-<strong>HBs</strong>, la determinazione quantitativa dell’anti-<strong>HBs</strong><br />
risulta rilevante dal punto di vista dell’immunizzazione attiva. Secondo le raccomandazioni del<br />
WHO, si può presumere una protezione contro l’infezione in seguito a vaccinazione se nel siero<br />
o nel plasma può essere riscontrata una concentrazione di anti-<strong>HBs</strong> superiore a 10 UI/L. Si<br />
raccomanda di eseguire in tempo le vaccinazioni di richiamo per garantire che i valori non siano<br />
al di sotto di questo limite 5, 6, 7 .<br />
Pazienti con una concentrazione di anticorpi anti-<strong>HBs</strong> < 100 UI/L dopo il completamento della<br />
vaccinazione di base, richiedono una vaccinazione di richiamo entro un anno. (Raccomandazioni della<br />
Commissione Permanente per le Vaccinazioni dell’Istituto Robert Koch in Germania, ottobre 1995).<br />
La necessità di una determinazione quantitativa risulta ulteriormente importante perché la<br />
durata dell’immunità è proporzionale ai livelli di anti-<strong>HBs</strong> presenti.<br />
Principio del metodo<br />
L’Enzygnost * anti-<strong>HBs</strong> <strong>II</strong> è un test ad una fase secondo il metodo sandwich. L’antigene utilizzato<br />
per la fase solida ed il coniugato è costituito da <strong>HBs</strong>Ag umano.<br />
L’<strong>HBs</strong>Ag marcato con perossidasi si lega agli anticorpi specifici anti-<strong>HBs</strong> presenti nel campione<br />
in esame. Questi si legano all’<strong>HBs</strong>Ag legato alla superficie della piastra per microtitolazione<br />
(sandwich fra antigeni).<br />
Dopo l’eliminazione tramite lavaggio delle componenti non legate si determina l’attività<br />
enzimatica del coniugato. La reazione enzimatica del substrato e del cromogeno produce una<br />
colorazione blu. La reazione viene interrotta con l’aggiunta della soluzione bloccante POD ed il<br />
colore vira al giallo. L’intensità del colore è proporzionale alla concentrazione degli anticorpi nel<br />
campione. I risultati vengono quantificati mediante calcolo con il metodo α o per confronto con<br />
una curva standard.<br />

Avvertenze e precauzioni<br />
1. Per uso diagnostico in vitro.<br />
2. Ogni donazione di sangue utilizzata per la produzione dei controlli dell'Enzygnost * anti-<strong>HBs</strong> <strong>II</strong><br />
è stata esaminata per la ricerca dell' <strong>HBs</strong>Ag e degli anticorpi anti-HCV, anti HIV1 e anti-HIV2.<br />
Solo i campioni risultati negativi sono stati impiegati per la produzione.<br />
Tuttavia, tutti i derivati da sangue umano devono essere trattati con le necessarie precauzioni<br />
rispettando le norme di sicurezza sul rischio biologico 8 , in quanto non è possibile escludere<br />
con assoluta certezza il pericolo di agenti patogeni.<br />
3. L'<strong>HBs</strong>Ag impiegato per la produzione della piastra test e del coniugato POD è stato isolato,<br />
con elevato grado di purezza, da plasmi umani positivi per l'<strong>HBs</strong>Ag e negativi ai test anti-HCV,<br />
anti-HIV1 ed anti-HIV2, ed è stato sottoposto ad un accurato processo di inattivazione.<br />
4. Si consiglia l’uso di guanti di protezione durante l’intera esecuzione del test.<br />
5. Per lo smaltimento del materiale solido infettivo si consiglia il trattamento in autoclave per<br />
almeno 1 ora a +121 °C. Tutti i liquidi aspirati devono essere raccolti in due contenitori<br />
collegati in serie, i quali dovrebbero contenere un disinfettante adatto all’inattivazione dei virus<br />
patogeni umani. Osservare le concentrazioni ed i tempi di azione indicati dal produttore.<br />
Preparazione dei reagenti<br />
Prima dell’inizio del test portare tutti i reagenti ed i campioni in esame a +15/+25 °C (non togliere<br />
la piastra test dal sacchetto di alluminio).<br />
Le file di pozzetti non necessarie devono essere tolte dal supporto e conservate per<br />
l’utilizzazione successiva (vedere Tabella 1).<br />
Per ogni piastra test diluire 20 mL di soluzione di lavaggio POD con 400 mL di acqua distillata<br />
o deionizzata.<br />
Per ogni piastra test diluire 1 mL di cromogeno TMB con 10 mL di tampone/substrato TMB<br />
nel flacone vuoto fornito nella confezione (soluzione d’uso del cromogeno) e conservare al<br />
riparo dalla luce. Dopo l’uso lavare accuratamente il flacone con acqua distillata.<br />
A causa della dimensione del flacone non è possibile versare il cromogeno TMB direttamente<br />
nel flacone del tampone/substrato.<br />
Prediluizione dei campioni: per l’analisi quantitativa i campioni in cui si sospetta una<br />
concentrazione di anti-<strong>HBs</strong> ≥ 100 UI/L dovrebbero essere diluiti come segue:<br />
fino a 1000 UI/L: diluire<br />
fino a 10.000 UI/L: diluire<br />
1:10 (ad esempio 20 µL+180 µL siero anti-<strong>HBs</strong> negativo)<br />
1:100 (ad esempio 20 µL+180 µL siero anti-<strong>HBs</strong> negativo; successivamente<br />
20 µL della prima diluizione + 180 µL siero anti <strong>HBs</strong> negativo)<br />
fino a 100.000 UI/L: diluire 1:1000 (ad esempio 20 µL+180 µL siero anti-<strong>HBs</strong> negativo; successivamente<br />
20 µL della prima diluizione + 180 µL siero anti <strong>HBs</strong> negativo; successivamente<br />

Se la valutazione quantitativa del test viene ottenuta utilizzando una serie di diluizioni, è<br />
necessario preparare inoltre le seguenti diluizioni del Siero anti-<strong>HBs</strong> 100 utilizzando il Siero anti-<br />
<strong>HBs</strong> negativo (ciò non vale se la valutazione viene eseguita con il metodo α) :<br />
50 UI/L: diluizione 1:2 (es.: 200 µL di siero anti-<strong>HBs</strong> (100) + 200 µL di siero di negativo anti-<strong>HBs</strong>)<br />
25 UI/L: diluizione 1:4 (es.: 150 µL dello siero 50 UI/L + 150 µL di siero di negativo anti-<strong>HBs</strong>)<br />
10 UI/L: diluizione 1:10 (es.: 50 µL di siero anti-<strong>HBs</strong> 100 + 450 µL di siero di negativo anti-<strong>HBs</strong>)<br />
5 UI/L: diluizione 1:20 (es.: 150 µL dello siero 10 UI/L + 150 µL di siero di negativo anti-<strong>HBs</strong>)<br />
Conservazione e validità<br />
Prima dell'apertura, tutte le componenti della confezione possono essere utilizzate fino alla data<br />
di scadenza indicata in etichetta, purché conservate a + 2/+ 8 °C.<br />
La conservazione e la validità dei reagenti pronti per l'uso, dopo l'apertura, sono riportate in<br />
appendice nella Tabella 1.<br />

Strumentazione necessaria<br />
BEP ® <strong>II</strong>: Per la dispensazione automatica dei reagenti e della soluzione di lavaggio<br />
BEP ® <strong>II</strong>I: Per l’esecuzione automatica e valutazione del test, dopo la distribuzione dei<br />
campioni<br />
BEP ® 2000: per l’esecuzione automatica del test e la valutazione dei risultati<br />
Pipette: Pipette automatiche da 25, 100 e 1000 µl.<br />
Sistema di incubazione: bagnomaria coperto (+ 37 ± 1°C) o analoghi metodi di incubazione.<br />
Tutta la strumentazione usata nel test deve essere validata.<br />
Campioni in esame<br />
Si impiegano come campioni in esame quelli singoli (siero umano o plasma citratato/<br />
eparinizzato/EDTA) ottenuti da tecniche standard di laboratorio. I campioni devono essere<br />
conservati a + 2/+ 8°C per non più di 3 giorni. Se i campioni devono essere conservati per un<br />
lungo periodo di tempo, devono essere congelati.<br />
Esecuzione del test<br />
Esecuzione del test con il BEP ® <strong>II</strong><br />
1. Schema del test: accertare il numero di pozzetti necessari (= numero dei campioni più 6<br />
pozzetti per i controlli)<br />
2. Distribuzione dei campioni:<br />

dispensare 100 µL/pozzetto di siero anti-<strong>HBs</strong> negativo in 4 pozzetti (A1-D1) e 100 µL di siero<br />

campioni. Alla fine della serie o della piastra dispensare in un ulteriore pozzetto 100 µL di<br />
siero anti-<strong>HBs</strong> 100.<br />
Effettuare la fase di dispensazione rapidamente entro un massimo di 15 minuti per ogni<br />
piastra.<br />
Importante:<br />
Non è consentito distribuire prima il siero anti-<strong>HBs</strong> 100 in tutte le posizioni previste e<br />
successivamente interporre i campioni.<br />
Test quantitativo (diluizioni standard)<br />
Distribuire 100 µL di siero negativo anti-<strong>HBs</strong> in ognuno dei primi 4 pozzetti e successivamente<br />
distribuire in doppio 100 µL di siero anti-<strong>HBs</strong> per ognuna delle diluizioni 100, 50, 25, 10 e 5 UI/L.<br />
Distribuire 100 µL di campione (evtl. prediluiti secondo la necessità) in ognuno dei successivi<br />
pozzetti.<br />
Immediatamente dopo la dispensazione dei campioni procedere con la dispensazione del<br />
coniugato.<br />
3. Distribuzione del coniugato: dispensare 25 µL di coniugato <strong>HBs</strong>Ag/POD (anti-<strong>HBs</strong> <strong>II</strong>) in<br />
ogni pozzetto.<br />
Ricoprire quindi con un foglio adesivo. La micropiastra deve essere inserita nel sistema di<br />
incubazione immediatamente dopo la distribuzione del coniugato.<br />
4. Incubazione: incubare la piastra per 60 ± 2 minuti a 37 ± 1 °C; procedere quindi immediatamente<br />
con la fase di lavaggio.<br />
5. Lavaggio: rimuovere il foglio adesivo ed aspirare il contenuto dei pozzetti; lavare 4 volte con<br />
circa 0,3 mL/pozzetto di soluzione di lavaggio. Conclusa la fase di lavaggio effettuare subito<br />
la dispensazione del reagente successivo per evitare l’essiccamento dei pozzetti.<br />
6. Distribuzione del substrato: dispensare in ogni pozzetto 100 µL della soluzione d’uso del<br />
cromogeno e coprire la piastra con un nuovo foglio adesivo.<br />
7. Incubazione: incubare per 30 ± 2 minuti a +15/+25 °C al riparo della luce.<br />
8. Arresto della reazione: rimuovere il foglio adesivo e dispensare in ogni pozzetto 100 µL di<br />
soluzione bloccante POD, con la stessa cadenza di tempo seguita per la distribuzione del<br />
substrato (punto 6).<br />
9. Lettura fotometrica: effettuare la lettura a 450 nm entro 1 ora. La lunghezza d’onda di<br />
riferimento raccomandata è di 650 nm (eventualmente tra 615 e 690 nm).<br />

Esecuzione del test usando il BEP ® <strong>II</strong>I<br />
Prima di utilizzare il BEP ® <strong>II</strong>I, preparare le piastre test ed eseguire le operazioni di<br />
dispensamento dei campioni (Paragrafo 1 e 2 in ”Esecuzione del test con il BEP ® <strong>II</strong>”). Subito<br />
dopo porre le piastre non coperte da foglio adesivo nel BEP ® <strong>II</strong>I. Attenzione: le piastre<br />
parzialmente riempite devono essere completate almeno a metà piastra (6 strip) aggiungendo<br />
”strip riempite con acqua”. Il test viene quindi eseguito in modo completamente automatico (v.<br />
Manuale d’uso BEP ® <strong>II</strong>I).<br />
Le impostazioni per i periodi di incubazione nel software del BEP ® <strong>II</strong>I possono differire dai periodi<br />
del BEP ® <strong>II</strong> per motivi tecnici (velocità del sistema), ma sono state validate per i metodi<br />
Enzygnost * sul BEP ® <strong>II</strong>I.<br />
Esecuzione del test usando il BEP ® 2000<br />
Le fasi di dispensazione del campione e la successiva esecuzione del test sono eseguite in<br />
maniera completamente automatica dall’analizzatore (v. manuale d’uso del BEP ® 2000).<br />
Criteri di validità del test<br />
I valori individuali di estinzione dei sieri di controllo vengono utilizzati per calcolare il valore<br />
medio quando:<br />
-0,010 ≤ E neg. ≤ 0,120<br />
E siero anti-<strong>HBs</strong> 100 ≥ 0.7<br />
Se uno dei quattro valori di estinzione dei controlli negativi non rientra nell’ambito indicato, può<br />
essere trascurato.<br />
Entrambi i valori di estinzione del siero anti-<strong>HBs</strong> 100 devono rientrare nell’ambito indicato.<br />
Se queste condizioni non vengono rispettate, il test deve essere ripetuto.<br />
Per il test quantitativo si applicano i seguenti criteri di validità:<br />

Se si utilizza il metodo α :<br />
Le singole letture fotometriche del Siero anti-<strong>HBs</strong> 100 vengono utilizzate per calcolare la media,<br />
a condizione che :<br />
limite inferiore < E siero anti-<strong>HBs</strong> 100 < limite superiore<br />
I valori per il limite inferiore e superiore sono riportati nell’allegata tabella dei valori.<br />
Entrambi i valori di estinzione del siero anti-<strong>HBs</strong> 100 devono essere compresi nell’ambito<br />
indicato.<br />
Inoltre, i singoli valori non devono differire di oltre il 20 % dalla media di questi due valori.<br />
Se queste condizioni non vengono rispettate, il test può essere valutato solo dal punto di vista<br />
qualitativo.<br />
Valutazione del test<br />
Sul BEP ® 2000 e BEP ® <strong>II</strong>I, il calcolo dei risultati viene eseguito automaticamente. Consultare il<br />
rispettivo manuale d’uso. I seguenti capitoli permettono la valutazione dei risultati senza l’ausilio<br />
del software.<br />
Test qualitativo<br />
Calcolare il valore medio di estinzione dei controlli negativi e quindi calcolare il cut-off<br />
aggiungendo 0,08:<br />

I campioni in base ai criteri del test vengono classificati come segue:<br />

Test quantitativo (metodo α)<br />
Per ogni piastra test utilizzata viene calcolato un fattore di correzione utilizzato per correggere i<br />
valori di estinzione di ogni singolo campione (correzione della misura). I valori di assorbanza<br />
corretti vengono quindi utilizzati per calcolare l’attività anticorpale (vedere «Calcolo dei risultati»).<br />
Correzione della misura<br />
Il fattore di correzione viene calcolato dalla formula seguente:<br />

Il valore nominale è riportato nella tabella dei valori acclusa. I valori di estinzione dei campioni<br />
vanno moltiplicati per il fattore di correzione. I valori di estinzione corretti ottenuti vengono quindi<br />
utilizzati per il calcolo dei risultati.<br />
Se vengono utilizzate varie piastre test, il fattore di correzione deve essere determinato<br />
separatamente per ogni singola piastra ed utilizzato per la correzione dei valori di estinzione<br />
della piastra corrispondente.<br />
Calcolo dei risultati<br />
Per calcolare l’attività anticorpale, i valori di estinzione corretti dei campioni, vengono utilizzati<br />
nella seguente formula (metodo α)<br />

dove α e β sono delle costanti lotto-dipendenti e sono riportate nella tabella dei valori acclusa. I<br />
risultati dell’attività anticorpale espressi in «UI/L» sono riferiti al Preparato di Riferimento<br />
Internazionale per l’HBV del WHO.<br />
Importante: NON utilizzare nella formula i dati seguenti:<br />

Se il campione è stato prediluito (es. 1:11), l'attività anticorpale calcolata (non l'assorbanza)<br />
deve essere moltiplicata per 11.<br />
Test quantitativo (diluizioni standard)<br />
Per la valutazione quantitativa si valutano i valori medi di estinzione della determinazione in<br />
doppio delle diluizioni standard 5, 10, 25, 50 e 100 UI/L. Viene allestita una curva di calibrazione<br />
su carta bilogaritmica.<br />
Ascissa: concentrazione di anti-<strong>HBs</strong> da 5 a 100 UI/L. Ordinata: valori di estinzione da 0,05 a 2,0.<br />
Le concentrazioni di anti-<strong>HBs</strong> dei campioni si leggono dalla curva di calibrazione sulla base dei<br />
singoli valori di estinzione.<br />
Se i campioni sono stati diluiti, moltiplicare la concentrazione che si ottiene dalla curva di<br />
calibrazione per il fattore di diluizione.<br />

Limitazioni della esecuzione del test<br />
1. <strong>Anti</strong>coagulanti come l’eparina, l’EDTA ed il citrato non interferiscono con il test.<br />
2. Campioni lipemici, emolizzati, itterici o contenenti fattori reumatoidi non influenzano i risultati<br />
del test.<br />
3. Nessuna interferenza con il risultato del test è stata osservata con campioni di donne in<br />
gravidanza.<br />
4. I campioni contenenti le seguenti potenziali fonti di interferenze sono stati controllati nel test:<br />
antigene <strong>HBs</strong>, ANA e anticorpi anti-HCV, HAV, HBc, HBe, EBV e CMV. Nessuna interferenza<br />
con il risultato del test è stata osservata con i campioni utilizzati.<br />
5. Non sono state riscontrate interferenze nei campioni trattati al calore (30 minuti a 56 °C).<br />
6. Non dovrebbero essere utilizzati campioni di siero da sangue non completamente coagulato,<br />
che contengano sodio azide o presentano contaminazione batterica. Ogni particella presente<br />
nel campione (ad esempio coaguli di fibrina) dovrebbe essere eliminata prima dell’uso.<br />
7. Quando si utilizzano campioni scongelati, assicurarsi di una buona omogeneizzazione del<br />
materiale prima dell’uso.<br />
8. E' necessario utilizzare solo reagenti appartenenti tutti ad uno stesso lotto (ad esclusione della<br />
soluzione di lavaggio POD, la Soluzione bloccante POD e la Soluzione d'uso del cromogeno<br />
preparata con i reagenti Cromogeno TMB e Tampone/Substrato TMB appartenenti tutti allo<br />
stesso lotto). La combinazione ammessa dei numeri di lotto a 6 cifre e quella stampata sulla<br />
confezione ed anche riportata nell'allegata tabella dei codici a barre.<br />
9. Il tampone/substrato TMB, la soluzione d’uso del cromogeno e la soluzione bloccante POD<br />
non devono entrare in contatto con ioni di metalli pesanti o sostanze ossidanti (non utilizzare<br />
pipette con parti metalliche a contatto con il liquido!). Non eseguire la reazione con il substrato<br />
in presenza di disinfettanti contenenti ipoclorito. Se la soluzione d’uso del cromogeno si colora<br />
spontaneamente di blu prima del dispensamento nella piastra test significa che la soluzione è<br />
stata contaminata; in tal caso preparare nuovamente la soluzione fresca in un contenitore<br />
pulito. Evitare il contatto di queste soluzioni con la cute.<br />
10. Il siero di controllo è stato ottenuto da siero umano nativo. Possono manifestarsi intorbidamenti,<br />
che però non interferiscono sui risultati del test.<br />
11. Durante la fase di incubazione la piastra test deve rimanere immobile (ad esempio utilizzare un<br />
supporto fisso ed evitare bagnomaria a circolazione d’acqua); i pozzetti della piastra devono<br />
essere a contatto con l’acqua termostatata. Se vengono utilizzati stabilizzanti per evitare la<br />
contaminazione batterica dell’acqua, occorre fare attenzione che la superficie della piastra test<br />
ed i pozzetti non vengano a contatto con queste soluzioni, in quanto simili contaminazioni<br />
possono produrre reazioni aspecifiche.<br />
12. In presenza di campioni fortemente reattivi si può verificare la formazione di un precipitato<br />
entro la soluzione colorata. Tuttavia la valutazione fotometrica non viene compromessa.<br />
13. Dade Behring ha validato l’uso di questi reagenti su vari analizzatori per ottimizzare le<br />
prestazioni e rispettare le specifiche dei prodotti. Le modifiche definite dall’utente non sono<br />
supportate da Dade Behring poiché possono influire sulle prestazioni del sistema e sui<br />
risultati del test. Pertanto è responsabilità dell’utente validare tutte le modifiche apportate a<br />
queste istruzioni, l’uso dei reagenti su analizzatori diversi da quelli inclusi nei fogli di<br />
istruzioni o nelle istruzioni d’uso fornite da Dade Behring.<br />
14. I risultati di questo test devono essere sempre interpretati alla luce della anamnesi del<br />
paziente, della presentazione clinica e valutando contestualmente l’esito di altri<br />
accertamenti.<br />
Caratteristiche del test<br />
Sensibilità e specificità<br />
I risultati degli studi di sensibilità e specificità sono riportati in appendice nelle Tabelle 2 e 3.<br />
La sensibilità diagnostica è stata stabilita analizzando 515 campioni positivi per l’anti-<strong>HBs</strong> ed il<br />
valore riscontrato è stato del 100 %. La reattività del test nei confronti dei campioni di<br />
sieroconversione/profili di vaccinazione è stata investigata utilizzando 41 profili. L’Enzygnost *<br />
anti-<strong>HBs</strong> <strong>II</strong> ha presentato un grado di sensibilità confrontabile con altri test.<br />

La sensibilità analitica del test, determinata con una valutazione quantitativa (curva standard)<br />
utilizzando il Preparato di Riferimento del WHO, è risultata essere < 8 UI/l.<br />
Per stabilire la specificità del test sono stati analizzati un totale di 4736 campioni di donatori<br />
negativi per l’anti-<strong>HBs</strong> ed il risultato ottenuto è stato del 99,5 - 99,7 %. Valori differenti possono<br />
essere riscontrati in funzione del tipo di popolazione, del metodo d’analisi utilizzato e di altri<br />
fattori.<br />
Riproducibilità<br />
I risultati degli studi di riproducibilità intra/inter assay sono riportati in appendice nella Tabella 4.<br />
Questi dati sono un esempio. Valori differenti possono essere ottenuti in funzione di vari fattori<br />
come il metodo etc.<br />

Campos de aplicación<br />
Prueba inmunoenzimática para la determinación cualitativa y cuantitativa del anticuerpo contra<br />
el antígeno de superficie de la hepatitis B (<strong>HBs</strong>Ag) en suero y en plasma.<br />
La realización del ensayo inmunológico se lleva a cabo en los ELISA Procesadores BEP ® <strong>II</strong>, BEP ®<br />
<strong>II</strong>I y BEP ® 2000. El test se desarrolló para la investigación de muestras individuales, no de muestras<br />
en pool. El producto debe ser usado únicamente en diagnósticos in vitro.<br />
Significado diagnóstico<br />
Al igual que el antígeno de superficie de la hepatitis B (<strong>HBs</strong>Ag), el anticuerpo dirigido contra la<br />
proteína de la superficie, <strong>Anti</strong>-<strong>HBs</strong>, es también un parámetro importante en el diagnóstico de<br />
una infección con el virus de la hepatitis B (HBV) 1, 2 .<br />
Durante el tiempo de incubación y durante la fase aguda de una infección con HBV no se<br />
pueden reconocer anticuerpos contra el <strong>HBs</strong>Ag. La inmunidad otorgada a los anticuerpos anti-<br />
<strong>HBs</strong> aparece en el 90% de todos los casos solamente al final de la convalecencia,<br />
aproximadamente entre 3 y 4 meses después del comienzo de la enfermedad, cuando ya no se<br />
puede reconocer más el <strong>HBs</strong>Ag circulante 3, 4 .<br />
Esta seroconverción, esto es el paso del anti-<strong>HBs</strong> negativo a positivo, representa un parámetro<br />
verdaderamente confiable para una infección por HBV terminada, cuanto más, cuando<br />
aproximadamente un 10% de los pacientes con una infección aguda, para los cuales en la fase<br />
inicial no se encontró ningún <strong>HBs</strong>Ag, también se van a volver más tarde anti-<strong>HBs</strong> positivos. Por<br />
esta razón, la determinación del anti-<strong>HBs</strong> es apropiada también para el diagnóstico de<br />
infecciones por HBV que se desarrollan de forma no aparente 3, 4 .<br />
Ya que un reconocimiento positivo del anti-<strong>HBs</strong> está indicando una exposición anterior con este<br />
antígeno ya sea mediante una infección por HBV o en forma de una vacuna, las investigaciones<br />
siguientes resultan ser las aplicaciones más importantes de la determinación del anti-<strong>HBs</strong>:<br />
a) Juicio sobre la convalecencia y en cierta forma sobre la prognosis de pacientes infectados<br />
por HBV (control del desarrollo).<br />
b) Investigaciones serológicas dentro del marco de un programa de vacunación (screenig y<br />
controles de inmunización).<br />
c) Encuestas epidemiológicas.<br />
Junto a la determinación cualitativa del anti-<strong>HBs</strong> la determinación cuantitativa del anti-<strong>HBs</strong> ha<br />
logrado obtener, desde el punto de vista de la inmunización activa, su propio significado. De<br />
acuerdo a una recomendación de la OMS se puede hablar de una protección contra la infección<br />
después de una vacunación cuando se puede determinar en el suero o en el plasma una<br />
concentración de anti-<strong>HBs</strong> > 10 UI/l. Se recomienda efectuar vacunaciones de recuerdo<br />
oportunamente para asegurar que no se queda debajo de este valor límite 5, 6, 7 .<br />
Para valores de anti-<strong>HBs</strong> < 100 UI/l después de la inmunización básica es indicado, de acuerdo<br />
con las recomendaciones para vacunación de la Comisión permanente de Vacunación del<br />
Instituto Robert-Koch (Alemania) (STIKO estado en octubre de 1995), realizar una vacunación<br />
de recuerdo dentro del plazo de un año.<br />
La necesidad de una cuantificación se va ha reforzar adicionalmente por el hecho de que la<br />
duración de la protección de la vacuna es proporcional a la concentración de anti-<strong>HBs</strong><br />
alcanzada después de la vacunación.<br />
Principio del método<br />
El Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong> es un ensayo de una etapa, construido de acuerdo al principio de sandwich.<br />
Como antígeno de la fase fija y como antígeno conjugado se va a utilizar el <strong>HBs</strong>Ag humano.<br />
El <strong>HBs</strong>Ag marcado con peroxidasa se va a unir al anticuerpo específico contra el <strong>HBs</strong> contenido en la<br />
muestra. Estos se van a unir luego al <strong>HBs</strong>Ag unido a la superficie de la placa de microtitulación<br />
(antígeno-sandwich).<br />
Después de eliminar con agua los componentes no ligados se determina la actividad enzimática<br />
fijada del conjugado. La transformación enzimática del sustrato y del cromógeno (reacción cromática<br />
azul) se interrumpe por adición de la solución de parada POD (reacción cromática amarilla). La<br />
intensidad del color es proporcional a la concentración de anticuerpo existente en la muestra. La<br />
cuantificación se realiza según el método-α o por comparación con una curva de referencia.<br />

Advertencias y medidas de seguridad<br />
1. Sólo para ser utilizado en diagnósticos in-vitro<br />
2. Cada donación individual de sangre, destinada a la preparación de los sueros de control del<br />
Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong> ha sido investigada para detectar la presencia de <strong>HBs</strong>Ag, anti-HCV,<br />
anti-HIV1 y anti-HIV2. En la elaboración sólo se utilizan donaciones con resultados negativos.<br />
Independientemente de esto, todos los materiales obtenidos a partir de sangre humana deben<br />
ser manipuladas con las precauciones necesarias, siguiendo las medidas de seguridad<br />
recomendadas en caso de riesgo biológico, puesto que nunca se puede excluir completamente<br />
la existencia de agentes patógenos 8 .<br />
3. El <strong>HBs</strong>Ag, utilizado para la elaboración de las placas de prueba y del conjugado POD, fue<br />
aislado de plasmas humanos de alta pureza y fue sometido a un proceso de inactivación<br />
conocido. Sólo se utilizan plasmas humanos con resultados negativos con respecto a anti-<br />
HCV, anti HIV1 y anti HIV2.<br />
4. Se aconseja utilizar guantes de seguridad durante el desarrollo del test.<br />
5. Para la eliminación del material infeccioso sólido se recomienda colocarlo en el autoclave por<br />
lo menos 1 hora a + 121°C. Todas las soluciones aspiradas se deben recoger en dos<br />
recipientes conectados uno con el otro. Estos recipientes deben contener un medio de<br />
desinfección apropiado para inactivar virus patógenos humanos. Deben observarse la<br />
concentración y el tiempo de acción dados por el fabricante.<br />
Preparación de reactivos<br />
Calentar todos los reactivos y las muestras a una temperatura entre +15 y +25°C antes de iniciar<br />
el test, sin sacar la placa de pruebas del recipiente que la contiene.<br />
Los elementos de la placa de prueba no necesarios para el desarrollo del test deben ser<br />
retirados del soporte y almacenados para un uso posterior (ver Tabla 1).<br />
Para cada placa de prueba diluir 20 ml de solución de lavado POD con agua destilada o<br />
desionizada hasta obtener 400 ml.<br />
Para cada placa de prueba diluir 1 ml de cromógeno TMB con 10 ml de tampón/sustrato<br />
TMB en el frasco plástico vacío adjunto (solución de uso del cromógeno) y guardarlo cerrado y<br />
protegido de la luz. Después de su uso, enjuagar cuidadosamente el frasco con agua destilada.<br />
Por razones técnicas (sobrellenado) no está permitido verter el contenido del cromógeno TMB<br />
en el frasco de tampón/sustrato TMB.<br />
Predilución de las muestras: Para el desarrollo cuantitativo del test, las muestras para las<br />
cuales se espera una concentración de anti-<strong>HBs</strong> ≥ 100 UI/l, se deben diluir de la siguiente<br />
manera:<br />
Hasta 1000 UI/l, dilución 1:10 (por ej. 20µl de muestra + 180 µl de suero anti-<strong>HBs</strong>, negativo).<br />
Hasta 10.000 UI/l, dilución 1:100 (por ej. 20 µl de muestra + 180 µl de suero anti-<strong>HBs</strong>, negativo; de aquí<br />
20 µl + 180 µl de suero anti-<strong>HBs</strong>, negativo).<br />
Hasta 100.000 UI/l, dilución 1:1000 (por ej. 20 µl de muestra + 180 µl de suero anti-<strong>HBs</strong>, negativo; de aquí<br />
20 µl + 180 µl de suero anti-<strong>HBs</strong>, negativo; de aquí<br />
20 µl + 180 µl de suero anti-<strong>HBs</strong>, negativo).<br />
Si el procedimiento cuantitativo del test se desarrolla con una serie estandar, se deben realizar<br />
adicionalmente con el suero anti-<strong>HBs</strong> 100 las siguientes diluciones con suero anti-<strong>HBs</strong>,<br />
negativo (no es válido para la evaluación por el método-α):<br />
50 UI/l: dilución 1:2 (por ej. 200 µl del suero <strong>Anti</strong>-<strong>HBs</strong> 100 + 200 µl del suero de <strong>Anti</strong>-<strong>HBs</strong>, negativo)<br />
25 UI/l: dilución 1:4 (por ej. 150 µl del suero de 50 UI/l + 150 µl del suero <strong>Anti</strong>-<strong>HBs</strong>, negativo)<br />

Estabilidad y almacenaje<br />
Todos los componentes del envase combinado Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong> aún cerrados, conservados<br />
a una temperatura entre +2 y +8°C, son utilizables hasta las fechas de vencimiento dadas en las<br />
etiquetas.<br />
La estabilidad y las condiciones de almacenamiento de los envases ya abiertos o de los<br />
reactivos ya diluidos, listos para el uso, se pueden tomar de la Tabla 1 del apéndice.<br />
Equipo necesario:<br />
BEP ® <strong>II</strong>: Para el desarrollo automático de la dosificación de reactivos y de las etapas de lavado<br />
BEP ® <strong>II</strong>I: Para la realización completamente automática del test después de la distribución de<br />
las muestras, así como para la evaluación.<br />
BEP ® 2000: Para la realización y valoración completamente automática del test.<br />
Pipetas: Pipetas a émbolo 25, 100 y 1000 µl<br />
Incubadora: Baño de agua cubierto (+37 ± 1°C) u otros métodos de incubación similares<br />
Todos los instrumentos utilizados para el desarrollo del test deben estar validados.<br />
Material a investigar<br />
Para la investigación se pueden utilizar muestras aisladas (sueros humanos o plasma con<br />
EDTA/heparina/citrato), las cuales hayan sido tomadas según las técnicas estándar de<br />
laboratorio. Las muestras se pueden almacenar máximo 3 días entre 2 y 8°C. Para tiempos más<br />
largos de almacenamiento las muestras se deben congelar.<br />
Procedimiento<br />
Realización del test usando el BEP ® <strong>II</strong><br />
1. Esquema de distribución: Determinar el número necesario de pocillos de la placa (= cantidad<br />
de muestras a investigar + 6 pocillos para los controles).<br />
2. Distribución de las muestras:<br />
Prueba cualitativa/Prueba cuantitativa (método - α) :<br />
Pipetear en cada uno de los 4 primeros pocillos (A1 hasta D1) 100 µl del suero anti-<strong>HBs</strong>,<br />
negativo, en el pocillo siguiente (E1) 100 µl de suero 100 anti-<strong>HBs</strong> y en cada uno de los otros<br />
pocillos 100 µl de la muestra. Al final de la serie o de la placa de prueba pipetear una vez más<br />
100 µl del suero 100 anti-<strong>HBs</strong>.<br />
La etapa de pipeteo debe efectuarse a buen paso dentro de un plazo máximo de 15 min. por<br />
placa de prueba.<br />
Importante: Es improcedente pipetear primero el suero 100 anti-<strong>HBs</strong> en las dos<br />
posiciones designadas para éste y a continuación «introducir» las muestras entre ellas.<br />
Prueba cuantitativa (de la serie de diluciones del estándar):<br />
Colocar en cada uno de 4 pocillos 100 µl de suero <strong>Anti</strong>-<strong>HBs</strong>, negativo y a continuación, en<br />
cada uno de 2 pocillos, 100 µl de suero <strong>Anti</strong>-<strong>HBs</strong> de las diluciones que contienen 100, 50, 25,<br />
10 y 5 UI/l. En cada uno de los pocillos siguientes pipetear 100 µl de la muestra<br />
(eventualmente, si es necesario, prediluida).<br />
La distribución del conjugado debe seguir inmediatamente después de la distribución de las<br />
muestras<br />
3. Distribución del conjugado:<br />
Agregar en cada pocillo 25 µl del conjugado <strong>HBs</strong>Ag/POD (anti-<strong>HBs</strong> <strong>II</strong>).<br />
Cubrir con lámina adhesiva y colocar inmediatamente después de terminada la distribución<br />
del conjugado en el incubador.<br />
4. Incubación: Incubar durante 60 ± 2 min. a +37 ± 1°C; lavar de inmediato.<br />
5. Lavado: Retirar la lámina adhesiva, aspirar el contenido de todos los pocillos y lavar 4 veces<br />
usando cada vez aprox. 0,3 ml de solución de lavado. Después de terminar la fase de lavado,<br />
seguir inmediatamente con la siguiente dosificación de reactivos para evitar la desecación .<br />
6. Distribución del sustrato: Pipetear en cada pocillo 100 µl de solución de uso del cromógeno<br />
y cubrir la placa con una nueva lámina adhesiva.<br />

7. Incubación del sustrato: Incubar 30 ± 2 min. entre 15 y +25°C protegiendo de la luz.<br />
8. Reacción de parada: Retirar la lámina adhesiva y agregar en cada pocillo 100 µl de la<br />
solución de parada, manteniendo el mismo ritmo que en el punto 6.<br />
9. Medición: Realizar la medición en el término de una hora a 450 nm. Como longitud de onda<br />
de la medida de referencia se recomienda 650 (o entre 615 y 690 nm).<br />
Procedimiento en el BEP ® <strong>II</strong>I<br />
Para el desarrollo del test en el BEP ® <strong>II</strong>I, las placas de prueba se deben preparar hasta la<br />
distribución de las muestras (punto 1 hasta punto 3 del «Procedimiento en el BEP ® <strong>II</strong>»).<br />
Directamente después de esto, colocar las placas de prueba abiertas, esto es, sin lámina<br />
adhesiva en el BEP ® <strong>II</strong>I. Aquí es importante observar que las placas de prueba que no estén<br />
llenas se deben completar con «elementos con agua» hasta por lo menos la mitad de la placa<br />
de prueba (6 elementos). Todas las etapas subsiguientes van a ser realizadas de forma<br />
completamente automática por el aparato (ver manual de funcionamiento del BEP ® <strong>II</strong>I).<br />
Los tiempos de incubación ajustados en el software del BEP ® <strong>II</strong>I pueden desviarse de los del<br />
procedimiento en el BEP ® <strong>II</strong> debido a condiciones técnicas básicas (ritmo del aparato), están sin<br />
embargo validados en la combinación BEP ® <strong>II</strong>I/Enzygnost * .<br />
Procedimiento en el BEP ® 2000<br />
La distribución de las muestras y la realización subsiguiente del test se efectúa de forma completamente<br />
automática por el aparato (ver manual de funcionamiento del BEP ® 2000).<br />
Validez del test<br />
Los valores aislados de las extinciones de los sueros de control se van a tomar para determinar<br />
el valor promedio, cuando:<br />
-0,010 ≤ E neg. ≤ 0,120<br />
E suero 100 anti-<strong>HBs</strong> ≥ 0,7<br />
Si uno de los valores de extinción de los controles negativos, se encuentra por fuera de las<br />
especificaciones puede ser no tenido en cuenta.<br />
Los dos valores de extinción del suero 100 anti-<strong>HBs</strong> deben cumplir las especificaciones.<br />
Si estas condiciones no se cumplen se debe repetir el test.<br />
Para el test cuantitativo son válidos los siguientes criterios de validación:<br />
Al utilizar una serie estándar:<br />
E neg. ≤ E estándar 5 UI/l<br />
Al utilizar el método - α :<br />
Los valores aislados de las extinciones del suero anti-<strong>HBs</strong> 100 se van a utilizar para calcular el<br />
valor promedio cuando:<br />
límite de tolerancia inferior < E suero anti-<strong>HBs</strong> 100 < límite de tolerancia superior.<br />
Los valores de los límites de tolerancia inferior y superior vienen dados en la Tabla de valores<br />
aquí incluida. Los dos valores de extinción del suero anti-<strong>HBs</strong> 100 deben de cumplir las<br />
especificaciones. Además de esto, los valores aislados solamente se deben apartar del valor<br />
promedio en máximo un 20%. Si estas condiciones no se cumplen, el test deberá ser valorado<br />
sólo cualitativamente.<br />
Valoración del test<br />
La evaluación con el BEP ® 2000 y el BEP ® <strong>II</strong>I, se realiza automáticamente. Para esto consultar<br />
los manuales de operaciones. Los siguientes capítulos son para tener en cuenta en el caso de<br />
evaluaciones sin la ayuda del software.<br />
Test cualitativo:<br />
Se determina el valor promedio de los valores de extinción de los controles negativos.<br />
Para calcular el valor límite se añade al valor promedio de los controles negativos un valor de 0,08:<br />

De acuerdo con los criterios del test las muestras a investigar se van a clasificar de la siguiente<br />
manera:<br />
1. E muestra < cut off = anti-<strong>HBs</strong> negativo<br />
2. E muestra ≥ cut off = anti-<strong>HBs</strong> positivo<br />
Test cuantitativo (método - α):<br />
Para cada una de las preparaciones de la placa de prueba se va a determinar un factor de<br />
corrección, con el cual se va a corregir cada muestra (corrección de los valores medidos). Los<br />
valores corregidos se van a utilizar finalmente para la determinación de la actividad de<br />
anticuerpo (cálculo de los resultados).<br />
Corrección de los valores medidos<br />
El factor de corrección se va a determinar de la siguiente manera:<br />

El valor nominal viene dado en la Tabla de valores adjunta. Los valores de extinción de las<br />
muestras a investigar se multiplican por el factor de corrección. Los valores corregidos así<br />
obtenidos se van a utilizar para el cálculo de los resultados.<br />
En el caso de usar varias placas de prueba, se debe determinar para cada placa el factor de<br />
corrección, el cual se debe emplear en el cálculo de la correspondiente corrección.<br />
Cálculo de los resultados<br />
Los valores corregidos de las muestras a investigar se van a emplear en el cálculo de la<br />
actividad de anticuerpo de acuerdo con la siguiente fórmula (método-α):<br />
log 10 mUI/l = α . E β<br />
Las constantes α y β, dependientes del lote, se deben tomar de la Tabla de valores adjunta. El<br />
dato de la actividad de anticuerpo en «UI/l» se refiere a un suero HBV de referencia de la OMS.<br />
¡Atención!: En el cálculo no se deben utilizar:<br />

Si la muestra a investigar estaba prediluida (por ej. 1:11), la actividad de anticuerpo (no la señal<br />
medida) se debe multiplicar por 11.<br />
Prueba cuantitativa (de la serie de diluciones del estándar):<br />
Para la valoración cuantitativa se calcula la media de extinción a partir de las dobles<br />
determinaciones de la serie de diluciones del estándar con 5, 10, 25, 50 y 100 UI/l y con estos<br />
valores se traza una curva de referencia sobre papel doble logarítmico.<br />
Abscisa: Concentración de <strong>Anti</strong>-<strong>HBs</strong> de 5 a 100 UI/l. Ordenada: Extinción de 0,05 a 2,0. En esta<br />
curva de referencia se leen las concentraciones de <strong>Anti</strong>-<strong>HBs</strong> en las muestras a partir de las<br />
respectivas extinciones.<br />
Si se utilizaron muestras diluidas, las concentraciones obtenidas en la curva de referencia<br />
deben ser multiplicadas por el factor de dilución.<br />

Limitaciones del Procedimiento<br />
1. Los anticoagulantes como heparina, EDTA y citrato no influyen sobre los resultados del test.<br />
2. Las muestras lipémicas, hemolíticas, las muestras que contienen factor reumatoideo y las<br />
muestras ictéricas no alteran el desarrollo del test.<br />
3. En las muestras de embarazadas no se observó ninguna influencia sobre los resultados del<br />
test.<br />
4. En el test se examinaron, además, muestras con las siguientes sustancias potencialmente<br />
alterantes: antígeno <strong>HBs</strong>, ANA, anticuerpos contra HCV, HAV, HBC, HBe, EBV y CMV. En<br />
ninguna de estas muestras se observó influencia sobre los resultados del test.<br />
5. No se ha observado ninguna alteración en muestras tratadas por calor (30 min. a 56 °C).<br />
6. No se deben emplear sueros que no estén completamente coagulados, muestras de material que<br />
contengan azida sódica y muestras contaminadas microbiológicamente. Si eventualmente<br />
existen otras partículas (por ej. coágulos de fibrina) estas deben ser separadas antes de empezar<br />
el test.<br />
7. En muestras descongeladas se debe tener en cuenta que el material se encuentre bien<br />
homogenizado.<br />
8. Los reactivos (con exepción de la solución de lavado POD, la solución de parada y la<br />
solución de uso del cromógeno, la cual ha sido preparada a partir de los reactivos<br />
dependientes del lote, cromógeno TMB y Tampón/sustrato TMB ) sólo se pueden utilizar<br />
juntos si provienen del mismo lote, esto es, con el mismo número de 6 cifras que está<br />
impreso en el envase o que se puede tomar de la Tabla do código de barras incluida en él.<br />
9. El tampón/sustrato TMB, la solución de uso del cromógeno y la solución de parada POD no<br />
deben entrar en contacto con iones de metales pesados o con sustancias oxidantes (no utilizar<br />
pipetas que tengan partes metálicas que entren en contacto con el líquido). Las reacciones del<br />
sustrato no se deben efectuar en la cercanía de medios de desinfección que contengan<br />
hiploclorito. Una coloración azul espontánea de la solución de uso del cromógeno antes de<br />
agregarse a la placa de prueba está indicando una contaminación; prepare una nueva solución en<br />
un recipiente limpio. Evite el contacto de la piel con las soluciones arriba mencionadas.<br />
10. Los sueros de control son fabricados a partir de sueros humanos nativos. Por esta razón<br />
puede aparecer turbidez, la cual no influye sobre los resultados del test.<br />
11. Durante la incubación la placa de prueba debe de permanecer en reposo (por ej. con ayuda de un<br />
flotador fijo o con un bañomaría no circulante). Si se utilizan agentes de conservación para evitar<br />
la contaminación del agua, hay que tener la precaución de que ni la superficie de la placa de<br />
prueba ni los pocillos entren en contacto con estas soluciones pues podrían provocar reacciones<br />
no especificas.<br />
12. En muestras altamente reactivas se puede presentar una precipitación del colorante al agregar la<br />
solución de parada. La valoración fotométrica de la muestra no va a ser perturbada por esto.<br />
13. Dade Behring ha validado el uso de los reactivos en varios analizadores para optimizar el<br />
rendimiento del producto y cumplir con las especificaciones del mismo. Las modificaciones<br />
definidas por el usuario no están garantizadas por Dade Behring dado que pueden afectar al<br />
rendimiento del sistema y a los resultados del ensayo. Es responsabilidad del usuario<br />
validar las modificaciones realizadas a estas instrucciones o el uso de los reactivos en<br />
analizadores distintos a los incluidos en las hojas de aplicaciones de Dade Behring o en<br />
estas instrucciones de uso.<br />
14. Los resultados de esta prueba deberán interpretarse siempre de acuerdo con la historia<br />
clínica del paciente, la sintomatologia clínica y otras observaciones.<br />
Caracteristicas del test<br />
Sensibilidad y especificidad<br />
Los resultados del control de la sensibilidad y la especificidad están resumidos en las Tablas 2 y<br />
3 (en el apéndice).<br />
Para encontrar la sensibilidad diagnóstica se investigaron 515 muestras con anti-<strong>HBs</strong> positivo y<br />
se hallo una sensibilidad del 100 %. La reactividad del test para muestras con seroconversión/<br />
desarrollo de la vacunación se estudió en 41 casos. En esto casos se demostró que el<br />
Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong> muestra la misma sensibilidad con relación a las muestra con<br />
seroconversión/desarrollo de la vacunación que otros ensayos comparables.<br />

La sensibilidad analítica para la valoración cuantitativa (curva estándar) se determinó en un<br />
preparado de referencia de la OMS con < 8 UI/l.<br />
Para encontrar la especificidad se investigaron en total 4736 muestras de sangre de donantes<br />
con anti-<strong>HBs</strong> negativo y se hallo una especificidad desde el 99,5 hasta el 99,7 %. Dependiendo,<br />
entre otros, del grupo colectivo a investigar, del desarrollo del test, es posible que se presenten<br />
desviaciones de estos valores.<br />
Reproducibilidad<br />
Los resultados de los ensayos de reproducibilidad intra/ inter están resumidos en la Tabla 4 (en<br />
el apéndice). En estos casos se trata de ejemplos de datos hallados. Dependiendo, entre otros,<br />
del desarrollo del test es posible encontrar valores que se desvíen de estos.<br />

Campo de aplicação<br />
Determinação qualitativa e quantitativa de anticorpos contra o antigénio de superfície do vírus<br />
da hepatite B (<strong>HBs</strong>Ag) no soro e no plasma.<br />
O processamento do teste imunoenzimático é feito com os processadores ELISA BEP ® <strong>II</strong>,<br />
BEP ® <strong>II</strong>I e BEP ® 2000. O teste foi desenvolvido para a análise de amostras individuais, não de<br />
amostras em pool. O produto só pode ser utilizado para efeitos de diagnóstico in vitro.<br />
Significado diagnóstico<br />
Tal como o antigénio de superfície do vírus da hepatite B (<strong>HBs</strong>Ag), também o anticorpo anti-<br />
<strong>HBs</strong>, que se dirige contra a proteína de superfície, constitui um importante parâmetro diagnóstico<br />
numa infecção pelo vírus da hepatite B (HBV) 1, 2 .<br />
Durante a incubação e a fase aguda de uma infecção pelo HBV não é possível detectar<br />
anticorpos contra o <strong>HBs</strong>Ag. Os anticorpos imunizantes anti-<strong>HBs</strong> aparecem em 90% dos casos<br />
só na convalescença tardia, 3 a 4 meses após o começo da doença, quando o <strong>HBs</strong>Ag circulante<br />
já não se pode detectar 3, 4 .<br />
Esta seroconversão, isto é, a passagem de anti-<strong>HBs</strong> negativo a positivo, é um indicador bastante<br />
fiável de uma infecção HBV superada, tanto mais que também aproximadamente 10% dos<br />
pacientes com infecção aguda, nos quais, na fase inicial, o <strong>HBs</strong>Ag é indetectável, se tornam<br />
mais tarde anti-<strong>HBs</strong> positivos. Assim, a comprovação de anti-<strong>HBs</strong> é também um meio adequado<br />
para diagnostico de infecções HBV sub-clínicas 3, 4 .<br />
Sendo a presença comprovada de anti-<strong>HBs</strong> indiciaidora de um contacto, no passado, com o<br />
antigénio, ou por infecção HBV ou por vacinação, a determinação de anti-<strong>HBs</strong> está particularmente<br />
indicada nas seguintes aplicações:<br />
a) avaliação da convalescença e, em certa medida, prognostico de infectados com HBV<br />
(controlo da evolução)<br />
b) exames serológicos no quadro de programas de vacinação (screening e controlo da<br />
imunização)<br />
c) censos epidemiológicos<br />
A par da detecção qualitativa, a determinação quantitativa de anti-<strong>HBs</strong> assume particular relevância<br />
no contexto de uma imunização activa. De acordo com uma recomendação da OMS, pode<br />
presumir-se uma protecção contra a infecção, na sequência de uma vacinação, quando é possível<br />
comprovar no soro ou no plasma uma concentração de anti-<strong>HBs</strong> > 10 UI/l. E recomendam-se<br />
oportunas vacinações de reforço para evitar que os valores desçam abaixo deste limiar 5, 6, 7 .<br />
Perante valores de anti-<strong>HBs</strong> < 100 UI/l após uma imunização básica, está indicado, de acordo<br />
com as recomendações da Comissão Permanente de Vacinações do Instituto Robert Koch (Alemanha)(STIKO,<br />
Outubro de 1995), efectuar uma revacinação no prazo de 1 ano.<br />
A necessidade de uma quantificação é acentuada ainda pelo facto de a duração da imunidade<br />
ser proporcional à concentração de anti-<strong>HBs</strong> alcançada após a vacinação.<br />
Princípio método<br />
Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong> é um imunoensaio enzimático, com formação de “sandwich” de<br />
antigénios. <strong>HBs</strong>Ag humano é utilizado como antigénio da fase sólida e como antigénio do<br />
conjugado.<br />
<strong>HBs</strong>Ag marcado com peroxidase liga-se aos anticorpos específicos contra o <strong>HBs</strong> contidos na<br />
amostra. Estes por sua vez fixam-se aos <strong>HBs</strong>Ag fixados à superfície da placa de microtitulação<br />
(sandwich de antigénios).<br />
Após eliminação, por lavagem, dos elementos não ligados, é determinada a actividade enzimática<br />
do conjugado. A transformação enzimática do substrato e cromogéneo (reacção cromática azul) é<br />
interrompida por adição da solução de paragem POD (reacção cromática amarela). A intensidade<br />
cromática é proporcional à concentração dos anticorpos da amostra. A quantificação é feita por<br />
cálculo segundo o método α ou por comparação com uma curva standard.<br />

Advertências e medidas de precaução<br />
1. Só para uso diagnóstico in vitro<br />
2. Cada dádiva individual de sangue destinada à preparação dos soros de controlo de<br />
Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong> foi previamente examinada com vista à detecção de <strong>HBs</strong>Ag, anti-<br />
HCV, anti-HIV1 e anti-HIV2. Na elaboração só são utilizadas dádivas com resultado negativo.<br />
No entanto, todos os materiais obtidos de sangue humano devem ser manuseados com as<br />
precauções devidas, seguindo as medidas de segurança recomendadas em caso de risco<br />
biológico, uma vez que nunca se pode excluir inteiramente o risco de contaminação por<br />
agentes patogénicos 8 .<br />
3. O <strong>HBs</strong>Ag utilizado na preparação das placas de teste e do conjugado POD foi isolado, com<br />
um alto grau de pureza, de plasmas humanos positivos relativamente ao <strong>HBs</strong>Ag e negativos<br />
com relação ao anti-HCV, ao anti-HIV1 e ao anti-HIV2, e submetido a um reconhecido<br />
processo de inactivação.<br />
4. Recomenda-se utilizar luvas para análise durante toda a realização do teste.<br />
5. Para descontaminação de materiais sólidos, aconselha-se uma autoclavagem durante pelo<br />
menos 1 hora a uma temperatura de, pelo menos, + 121° C. Todas as soluções aspiradas<br />
devem ser recolhidas em dois recipientes ligados em série, os quais devem conter um<br />
desinfectante apropriado para inactivação de vírus humano-patogénicos. Observem-se as<br />
concentrações e os tempos de incubação indicados pelo fabricante.<br />
Preparação dos reagentes<br />
Antes de iniciar o teste, levar todos os reagentes e amostras à temperatura de + 15 a + 25° C,<br />
mas sem tirar a placa de ensaio do seu recipiente.<br />
Retirar do suporte os conjuntos de poços desnecessários e reservá-los para posterior utilização<br />
(cf. Tabela 1).<br />
Para cada placa de ensaio, diluir 20 ml da solução de lavagem POD (concentrado) com água<br />
destilada ou desionizada até obter 400 ml.<br />
Diluir, por placa, 1 ml de cromogéneo TMB com 10 ml de tampão/substrato TMB no frasco<br />
de plástico anexo à embalagem (solução de trabalho do cromogéneo) e conservá-lo fechado e<br />
ao abrigo da luz. Depois do uso, lavá-lo cuidadosamente com água destilada.<br />
Dada a capacidade dos frascos, não passar todo o conteúdo do frasco de cromogéneo TMB<br />
para o de tampão/substrato TMB.<br />
Pré-diluição das amostras: No teste quantitativo, e para concentrações previstas de anti-<br />
<strong>HBs</strong> ≥ 100 UI/l, as amostras devem ser diluídas como segue:<br />
Até 1000 UI/l: diluição 1 : 10 (p. ex. 20 µl de amostra + 180 µl de soro anti-<strong>HBs</strong> negativo)<br />
Até 10.000 UI/l: diluição 1 : 100 (p. ex. 20 µl de amostra + 180 µl de soro anti-<strong>HBs</strong> negativo;<br />
e, de aí, 20 µl + 180 µl de soro anti-<strong>HBs</strong> negativo)<br />
Até 100.000 UI/l: diluição 1 : 1000 (p. ex. 20 µl de amostra + 180 µl de soro anti-<strong>HBs</strong> negativo;<br />
desta diluição 20 µl + 180 µl de soro anti-<strong>HBs</strong> negativo; e,<br />
desta, 20 µl + 180 µl de soro anti-<strong>HBs</strong> negativo).<br />
Se a realização quantitativa do teste é feita a partir duma série standard, é necessário preparar,<br />
com soro anti-<strong>HBs</strong> negativo, as diluições complementares seguintes do soro anti-<strong>HBs</strong> 100 (mas<br />
não no caso de uma avaliação pelo método α ):<br />
50 UI/l: diluição 1 : 2 (p. ex. 200 µl de soro anti-<strong>HBs</strong> 100 + 200 µl de soro anti-<strong>HBs</strong> negativo)<br />
25 UI/l: diluição 1 : 4 (p. ex. 150 µl do soro de 50 UI/l + 150 µl de soro anti-<strong>HBs</strong> negativo)<br />
10 UI/l: diluição 1 : 10 (p. ex. 50 µl do soro anti-<strong>HBs</strong> 100 + 450 µl de soro anti-<strong>HBs</strong> negativo<br />
5 UI/l: diluição 1 : 20: (p. ex. 150 µl do soro de 10 UI/l + 150 µl de soro anti-<strong>HBs</strong> negativo)<br />
Estabilidade e condições de conservação<br />
Antes de abertos, e enquanto conservados entre +2 e +8° C, todos os componentes da<br />
embalagem combinada Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong> se mantêm utilizáveis até às datas indicadas nos<br />
rótulos.<br />
Sobre estabilidade e condições de conservação dos reagentes abertos ou diluídos para uso, v.<br />
tabela 1 em apêndice.<br />

BEP ® <strong>II</strong>: para a execução automática da dosagem dos reagentes e das fases de lavagem<br />

BEP ® 2000: para o processamento e interpretação do teste, de forma inteiramente<br />
automática.<br />
Pipetas: pipetas com êmbolo 25, 100 e 1000 µl<br />
Incubador: Banho-maria tapado (+37 ± 1 °C) ou outros métodos de incubação comparáveis<br />
Todos os aparelhos utilizados no ensaio têm de estar validados.<br />
Amostras<br />
Para a análise podem ser utilizadas amostras individuais (soros humanos e plasmas EDTA/<br />
Heparina/Citrato), colhidos de acordo com as técnicas do laboratório. As amostras poderão ser<br />
conservadas durante de 3 dias, no máximo, entre 2 - 8 °C. Se pretender conservá-las durante<br />
um período de tempo superior, deverão ser congeladas.<br />
Procedimento<br />
Realização do teste com o BEP ® <strong>II</strong><br />
1. Esquema de distribuição: Determinar o número necessário de poços (= número das<br />
amostras a investigar + 6 poços para os controlos).<br />
2. Distribuição das amostras<br />
Teste qualitativo / Teste quantitativo (Método α)<br />
Distribuir em cada um de 4 poços (A1 a D1) 100 µl de soro anti-<strong>HBs</strong> negativo; em 1 poço (E1)<br />
100 µl de soro anti-<strong>HBs</strong> 100; em cada um dos poços seguintes 100 µl de amostra; e no fim da<br />
série, ou da placa, novamente 100 µl de soro anti-<strong>HBs</strong> 100.<br />
A distribuição tem de ser feita num máximo de 15 minutos por placa.<br />
Importante:<br />
Não é permitido distribuir primeiro o soro anti-<strong>HBs</strong> 100 nas duas posições previstas e só<br />
depois as amostras entre elas.<br />
Teste quantitativo (série standard):<br />
Distribuir em 4 poços 100 µl, em cada um, de soro anti-<strong>HBs</strong> negativo, e a seguir em 2 poços 100<br />
µl, em cada um, das diluições do soro anti-<strong>HBs</strong> correspondentes a 100, 50, 25, 10 e 5 UI/l. Nos<br />
poços seguintes distribuir 100 µl, em cada um, de amostra (eventualmente pré-diluída).<br />
Terminada a distribuição das amostras, iniciar imediatamente a distribuição do conjugado.<br />
3. Distribuição do conjugado: Distribuir em cada poço 25 µl de conjugado <strong>HBs</strong>Ag/POD (<strong>Anti</strong>-<br />
<strong>HBs</strong> <strong>II</strong>). A seguir, cobrir com folha adesiva e colocar a placa imediatamente na incubadora.<br />
4. Incubação: Deixar incubar durante 60 ± 2 min. a + 37 ± 1° C, imediatamente a seguir dar<br />
início à lavagem.<br />
5. Lavagem: Retirar a folha adesiva, aspirar o conteúdo de todos os poços e lavar cada um 4<br />
vezes com aprox. 0,3 ml, de cada vez, de solução de lavagem. Depois das operações de<br />
lavagem passar imediatamente à distribuição do substrato para evitar uma dessecação.<br />
6. Distribuição do substrato: Distribuir 100 µl de solução de trabalho de cromogéneo em cada<br />
poço, cobrir a placa com nova folha adesiva.<br />
7. Incubação do substrato: Deixar incubar durante 30 ± 2 min ao abrigo da luz entre + 15 e + 25° C.<br />
8. Reacção de paragem: Retirar a folha adesiva. Acrescentar a cada poço 100 µl de solução<br />
de paragem POD, mantendo o mesmo ritmo que em 6.<br />
9. Leitura: Fazer uma medição fotométrica a 450 nm dentro de 1 hora. Como comprimento de<br />
onda de referência aconselha-se 650 nm (eventualmente entre 615 e 690 nm).<br />

Realização do teste com o BEP ® <strong>II</strong>I<br />
No caso do processamento ser feito com BEP ® <strong>II</strong>I é necessário preparar as placas de ensaio<br />
incluindo a dosagem das amostras (ponto 1 e 2 da “execução do teste com BEP ® <strong>II</strong>").<br />
Imediatamente a seguir, as placas de ensaio são colocadas no BEP ® <strong>II</strong>I abertas, ou seja, sem<br />
película colada. Ao mesmo tempo, é necessário prestar atenção a que as placas de ensaio<br />
parcialmente providas de ”barras de água” sejam acrescentadas até ficarem, no mínimo, pelo<br />
meio ( 6 barras de água). O processamento subsequente do teste realiza-se automaticamente<br />
(vide instruções de serviço do BEP ® <strong>II</strong>I).<br />
Devido às condições técnicas básicas (ciclo dos aparelhos), os tempos de incubação ajustados<br />
no software do BEP ® <strong>II</strong>I podem divergir dos tempos do processamento com o BEP ® <strong>II</strong>, mas<br />
foram validados na combinação BEP ® <strong>II</strong>I/Enzygnost * .<br />
Realização do teste com o BEP ® 2000<br />
A dosagem das amostras e subsequente processamento do teste são efectuados de forma<br />
totalmente automática no aparelho (vide instruções de serviço do BEP ® 2000).<br />
Validação do teste<br />
Os valores de absorvância dos soros de controlo serão utilizados para cálculo dos valores médios, se:<br />
- 0,010 ≤ E neg. ≤ 0,120<br />
E soro anti-<strong>HBs</strong> 100 ≥ 0,7<br />
Dos valores de absorvância dos controlos negativos, caso exista um fora da especificação poderá<br />
ser ignorado. Os valores de absorvância do soro standard anti-<strong>HBs</strong> 100 têm de corresponder<br />
ambos às especificações. Não estando preenchidas estas condições, há que repetir o teste.<br />
Para o teste quantitativo, os critérios de validação são os seguintes:<br />
Com uma série standard:<br />
E neg ≤ E standard 5 UI/l<br />
Com o método α:<br />
Os valores de absorvância dos soros de controlo serão utilizados para cálculo dos valores médios, se:<br />
limite inferior < E soro anti-<strong>HBs</strong> 100 < limite superior<br />
Os valores dos limites inferior e superior vêm indicados na tabela de valores adjunta.<br />
Os valores de absorvância do soro anti-<strong>HBs</strong> 100 têm de corresponder ambos à especificação.<br />
Além disso, a margem de variação de cada valor não pode ir além de 20% relativamente à<br />
média destes dois valores.<br />
Não estando preenchidas estas condições, o teste só pode ser valorado qualitativamente.<br />
Cálculo de Resultados<br />
As interpretações são realizadas automaticamente com o BEP ® 2000 e BEP ® <strong>II</strong>I. Para o efeito,<br />
consultar as instruções de serviço. Para uma interpretação sem a ajuda do software, é<br />
necessário prestar atenção aos seguintes capítulos.<br />
Teste qualitativo:<br />
Calcular a média dos valores de absorvância dos controlos negativos.<br />
Para calcular o valor-limite, adicionar 0,08 ao valor médio de absorvância dos controlos<br />
negativos:<br />

De acordo com os critérios do teste, as amostras serão classificadas do seguinte modo:<br />

Teste quantitativo (método α):<br />
É determinado para cada placa um factor de correcção com o qual se corrige o valor de<br />
absorvância obtido para cada amostra (correcção dos valores medidos). Com base nesses<br />
valores corrigidos é feito então o cálculo da concentração de anticorpos (cálculo dos<br />
resultados).<br />
Correcção dos valores medidos<br />
O factor de correcção é determinado segundo a fórmula seguinte:<br />

O valor nominal vem indicado na tabela de valores adjunta à embalagem. Os valores de<br />
absorvância das amostras são multiplicados pelo factor de correcção. Os valores corrigidos<br />
assim obtidos são depois utilizados para o cálculo dos resultados.<br />
Para cada placa, determinar o factor de correcção e utilizá-lo para o respectivo cálculo de<br />
correcção.<br />
Cálculo dos resultados<br />
Para calcular a concentração de anticorpos, utilizar a fórmula seguinte com base nos valores<br />
corrigidos das amostras (método α):<br />

As constantes α e β, que variam segundo os lotes, vêm indicadas na tabela de valores adjunta.<br />
A concentração de anticorpos, expressa em "UI/l", refere-se ao soro de referência HBV<br />
internacional da OMS.<br />
Atenção: Não utilizar para o cálculo:<br />
- valores corrigidos < cut off<br />
- valores não corrigidos > 2,5<br />

Se a amostra estava pré-diluída (p. ex. a 1:11), multiplicar a concentração de anticorpos (não o<br />
sinal de medida) por 11.<br />
Teste quantitativo (série standard)<br />
Para a leitura quantitativa, calcular os valores médios de absorvância das duplas determinações<br />
da série de diluições standard de 5, 10, 25, 50 e 100 UI/l e, com estes valores, traçar uma curva<br />
de referência sobre papel bilogarítmico. Abcissa: concentração de anti-<strong>HBs</strong> de 5 a 100 UI/l.<br />
Ordenada: absorvância 0,05 a 2,0. As concentrações de anti-<strong>HBs</strong> das amostras lêem-se desta<br />
curva com base em cada um dos valores de absorvância.<br />
Se as amostras foram diluídas, multiplicar a concentração lida na curva pelo factor de diluição.<br />

Limitações do procedimento<br />
1. <strong>Anti</strong>coagulantes como heparina, EDTA e citrato não influenciam o resultado do teste.<br />
2. Amostras lipémicas, hemolíticas, ictéricas ou que contenham factor reumatóide não prejudicam<br />
o teste.<br />
3. Com amostras de mulheres grávidas não foi observada qualquer influência sobre o<br />
resultado do teste.<br />
4. Com o teste foram examinadas amostras com as seguintes substâncias potencialmente<br />
perturbadoras: <strong>Anti</strong>génio <strong>HBs</strong>, ANA e anticorpos contra HCV, HAV, HBC, HBe, EBV e CMV.<br />
Com as amostras usadas não foi observada qualquer influência sobre o resultado do teste.<br />
5. Não foram observadas alterações em amostras sujeitas a tratamento térmico (30 min, 56° C).<br />
6. Não empregar soros coagulados, com azida sódica ou contaminados. Partículas<br />
eventualmente existentes (p. ex. coágulos de fibrina) devem ser previamente retiradas.<br />
7. Amostras descongeladas devem estar bem homogeneizadas.<br />
8. Os reagentes (com excepção da solução de lavagem POD, da solução de interrupção POD e<br />
da solução de trabalho do cromogénio, preparada esta última a partir dos reagentes<br />
específicos de cada lote Cromogénio TMB e Tampão/substrato TMB) só podem ser utilizados<br />
como constituintes de um mesmo lote, isto é, somente na combinação de 6 cifras que vem<br />
impressa na embalagem e indicada na Tabela de código de barras adjunta.<br />
9. O tampão/substrato TMB, a solução de trabalho do cromogénio e a solução de paragem POD<br />
não devem entrar em contacto com iões de metais pesados nem com substâncias oxidantes<br />
(não usar pipetas com partes metálicas portadoras de líquido). Não efectuar a reacção do<br />
substrato na proximidade de desinfectantes que contenham hipoclorito. Uma coloração azul<br />
espontânea da solução de trabalho do cromogéneo antes de esta ser colocada na placa de<br />
ensaio é indício de contaminação; deverá então preparar-se uma solução fresca num<br />
recipiente limpo. Evite-se o contacto cutâneo com as soluções acima mencionadas.<br />
10. Os soros de controlo são preparados utilizando soros humanos congénitos. Por essa razão,<br />
podem ocorrer turvações que, todavia, não influenciam o resultado do teste.<br />
11. Durante a incubação deve manter-se a placa de ensaio imóvel (p. ex. sobre um suporte fixo<br />
ou num banho-maria sem circulação de água); deste modo as cavidades mantêm-se em<br />
contacto com a água temperada. Caso se utilizem estabilizadores para evitar uma<br />
contaminação da água, tomar todo o cuidado para que nem a superfície da placa de ensaio<br />
nem os receptáculos entrem em contacto com estas soluções, pois tais contaminações<br />
podem produzir reacções inespecíficas.<br />
12. Em amostras altamente reactivas o corante pode precipitar ao adicionar-se a solução de<br />
paragem. O facto é irrelevante para os resultados fotométricos.<br />
13. A Dade Behring validou a utilização destes reagentes em vários analisadores, com o intuito<br />
de optimizar o desempenho do produto e satisfazer as especificações do produto. As<br />
modificações definidas pelo utilizador não são suportadas pela Dade Behring, na medida<br />
em que podem afectar o desempenho do sistema e os resultados do ensaio. Constitui<br />
responsabilidade do utilizador validar as modificações efectuadas nestas instruções ou em<br />
relação ao uso dos reagentes noutros analisadores que não os incluídos nas folhas de<br />
instruções de aplicação da Dade Behring ou nestas instruções de utilização.<br />
14. Os resultados deste teste devem sempre ser interpretados em conjunto com o histórico<br />
médico do doente, estado clínico e outros dados de interesse.<br />

Características do teste<br />
Sensibilidade e especificidade<br />
Os resultados das provas de sensibilidade e especificidade encontram-se resumidos nas<br />
tabelas 2 + 3 (em anexo).<br />
Para averiguar a sensibilidade diagnóstica foi investigado um total de 515 amostras anti-<strong>HBs</strong><br />
positivas, tendo sido verificada uma sensibilidade de 100%. A reactividade ao ensaio<br />
relativamente a amostras em seroconversão ou após vacinação foi testada em 41 casos. Pôde<br />
observar-se que o Enzygnost * <strong>Anti</strong>-<strong>HBs</strong> <strong>II</strong> apresenta, no que diz respeito ao reconhecimento de<br />
seroconversões ou de evoluções de vacinação, um grau de sensibilidade equivalente à de<br />
métodos similares.<br />
A sensibilidade analítica foi determinada com a avalização quantitativa (curva standard) do<br />
preparado de referência da WHO com < 8 UI/l.<br />
Para determinação da especificidade foi testado um total de 4.736 amostras de sangue anti-<strong>HBs</strong><br />
negativas, tendo sido achada uma especificidade de 99,5 a 99,7%. Estes valores podem variar<br />
em função da amostragem sujeita a teste, da modalidade de execução do teste ou de outros<br />
factores.<br />
Reprodutibilidade<br />
Os resultados para a reprodutibilidade em intra/inter-ensaio estão resumidos na Tabela 4 (em<br />
apêndice). Estes dados devem ser compreendidos somente a título de exemplo. Eles podem<br />
variar em função da modalidade de execução do teste ou de outros factores.<br />
