Background and objective: Fractional resurfacing was performed with a fractional
CO 2 laser (SmartXide DOT) which spreads energy with a scanned bean. It creates
small impulses or micro spots with the distance of 350 nm between them which are
distributed over the cutaneous surface and, thus, causes less thermal damage.
Heating dermis produces an immediate collagen contraction, collagen deposition and
remodeling. The object of this study was to examine the potential for eyelid tightening
and eye aperture opening in patients treated with fractional CO 2 laser for periorbital
rejuvenation.
Materials and methods: 200 subjects with superficial rhytides, skin laxity and photo
aging problems received an average of 2 treatments at 3 to 4 week; time per session:5
minutes. For all patients photographic evaluation (Canfield Omnia System was
utilised); 1 year follow up; patient satisfaction; tolerance index.
Results: The results are a significant reduction of eye wrinkles and an improvement
of the cutaneous surface tightening and texture. This process results in a considerable
rejuvenation of the periorbital zone. Down time with Fraxel is between 2 to 3
days.
Conclusion: This soft treatment is very effective and repairs the periorbital zone
from aging and photo damage. After 1 to 4 sessions the benefits are long lasting.
Fractional CO 2 really gives a more youthful appearance to the aging skin by tightening
and increasing the eyelid aperture.

Il laser CO 2 rappresenta il gold standard per lo
skin resurfacing.
Quando il raggio va a contatto con l’epidermide
produce calore e vaporizzazione sul tessuto. Lo
strato superficiale della pelle si rinnova ed il
calore stimola la produzione di nuovo collagene
nel derma.
Per questa ragione trova indicazione in trattamenti
di rughe, pori dilatati e macchie.
I risultati sono sicuramente incredibili ma ne
limitano l’uso gli svantaggi che possono derivare,
quali: down time prolungato, rischio di iperpigmentazione
o depigmentazione, cicatrici,
cura post operatoria alquanto delicata, lunga
permanenza dell’eritema, e non da ultimo, effetti
dell’anestesia per evitare il dolore.
Nel 2003 è stato introdotto il concetto di foto
termolisi frazionale per superare tutti i suddetti
problemi indotti dai laser CO 2 standard. Oggi
sul mercato possiamo trovare una grande quantità
di laser frazionati ablativi e non ablativi. La
nostra scelta deve cadere però su di un laser che
ci permetta di ottenere il massimo risultato con
il minimo rischio e con il minimo tempo di
recupero.
Il laser frazionato CO 2 ha tutti i vantaggi dell’ablazione:
perché determina, per il suo effetto
termico, una grossa stimolazione di fibroblasti e
delle fibre elastiche in assenza di effetti collaterali.
Il trattamento è personalizzato. La scelta dei
parametri quali potenza, tempo, dot spazio e
pausa, dipendono dalla situazione iniziale e
dallo stato della pelle dei singoli pazienti.
La potenza massima è di 30 Watt e lo scanner è
maneggevole, facile da usare non crea disturbi o
