H. Ayoğlu ve ark., Preoperatif Oral Karbonhidrat Solüsyonu Kullanılmasının Etkileri
rol grubu ile oral KS kullanan gruplarda
benzer olduğunu ve oldukça az olduğunu
(yaklaşık 6-7 mL) gözledik.
Laparoskopik cerrahi sonrasında bulantıkusma
insidansının % 37-72 arasında
olduğu bilinmektedir. (25-27) Oral KS kullanımının
insülin salınımına etkisi ile hipotalamik
serotonin sistemi üzerinde
etkisinin olası paralellik gösterebileceği,
bu nedenle postoperatif bulantı-kusma
insidansını azaltabileceği ve postoperatif
bulantı-kusmanın önlenmesinde multimodal
bir yöntem olabileceği belirtilmiştir.
(13,20,28) Biz de çalışmamızda postoperatif
ilk saatte (özellikle anestezinin etkisi
ile ortaya çıkabilecek bulantı-kusma varlığında)
bulantı kusmanın oral KS kullanan
grupta daha az olduğunu gözledik. Postoperatif
bulantı kusmanın önlenmesinde
preoperatif anksiyete giderilmesinin
önemi de çeşitli çalışmalarda vurgulanmıştır.
(29,30) Çalışmamızda oral KS kullanan
grupta preoperatif anksiyetenin daha az
olmasının da postoperatif bulantı-kusma
sıklığı üzerinde olumlu etkiye neden olabileceğini
düşündük.
Sonuç olarak, laparoskopik kolesistektomi
operasyonlarında oral karbonhidrat
solüsyonlarının, preoperatif açlık süresini
kısaltmak amacı ile kullanımlarının, preoperatif
anksiyete ve susuzluk hissi ile postoperatif
erken dönemde bulantıyı azaltarak
hasta konforunu artırdığı kanısına
varıldı.
