number of focal therapy techniques which destroy<br />
cancer cells directly are currently used for prostate<br />
carcinoma. Cryotherapy (»freezing of the tissues«)<br />
and HIFU (high-intensity focused ultrasound) use<br />
thermal energy which destroys cancer cells directly.<br />
Irreversible electroporation (IRE, also known as<br />
the NanoKnife) uses electric pulses in the microsecond<br />
to millisecond range in order to open pores in<br />
the wall of the tumour cell, thereby<br />
killing the cell immediately and<br />
cause the tumour to shrink. The<br />
electrical impulses are carried to<br />
the tumour through probes (needles).<br />
Since IRE is not a thermal<br />
ablation procedure, thus neither<br />
heat nor cold directly destroy the<br />
tumour cells, there is minimal risk<br />
of damage to surrounding structures<br />
(urethra, nerves, blood vessels<br />
etc.). IRE was used to treat<br />
localised prostate cancer for the<br />
first time by Onik and Rubinsky in<br />
2007. Since then the effectiveness and safety of<br />
this technique have been proved in numerous studies.<br />
The procedure lasts about 45-90 minutes and<br />
is carried out under general anaesthetic. It is only<br />
necessary to stay in hospital for 2-3 days. There<br />
is also very little pain following the procedure and<br />
this recedes spontaneously within 4-5 days. The<br />
patient is able to function normally after a week.<br />
IRE has been carried out by Dr Erik Schulten in collaboration<br />
with Prof Dr Ulf Tunn in the Ochoa Hospital,<br />
<strong>Marbella</strong> and the CERAM Hospital, <strong>Marbella</strong><br />
since the beginning of 20<strong>17</strong>. The medic is a colleague<br />
of Dr Erik Schulten, specialist in urology and<br />
andrology, OTS Clinic <strong>Marbella</strong>.<br />

stehen. Aktuell werden verschiedene Techniken zur<br />
Fokaltherapie des Prostatakarzinoms verwendet. Die<br />
Irreversible Elektroporation (IRE, sog. NanoKnife)<br />
verwendet elektrische Pulse im Mikro- bis Millisekundenbereich,<br />
um Poren in der Zellwand der Tumorzellen<br />
zu schaffen, die zum sofortigen Tod der Zelle<br />
führen und somit zum Schrumpfen des Tumors. Die<br />
elektrischen Impulse werden durch<br />
Sonden (»Nadeln«) an den Tumor<br />
gebracht. Da die IRE ein nicht thermisches<br />
Ablationsverfahren ist,<br />
somit weder Hitze noch Kälte direkt<br />
die Tumorzellen zerstört, besteht<br />
kaum eine Gefahr der Schädigung<br />
umliegender Strukturen (Harnröhre,<br />
Nerven, Blutgefässe etc.). Im Jahr<br />
2007 wurde die IRE erstmals von<br />
Onik und Rubinsky zur Behandlung<br />
von lokalisiertem Prostatakrebs<br />
verwendet. Die Wirksamkeit und<br />
Sicherheit der Technik wurde seither<br />
in zahlreichen Studien nachgewiesen. Der Eingriff<br />
dauert etwa 45-90 Minuten und wird in Vollnarkose<br />
durchgeführt. Die stationäre Verweildauer liegt<br />
bei nur 2-3 Tagen. Auch bestehen nach dem Eingriff<br />
lediglich geringfügige Beschwerden, welche innerhalb<br />
von 4-5 Tagen spontan sistieren. Nach einer<br />
Woche ist der Patient wieder voll einsatzfähig.<br />
Die IRE wird seit Anfang des Jahres 20<strong>17</strong> von Dr.<br />
med. Erik Schulten in Kooperation mit Prof. Dr. Ulf<br />
Tunn im Krankenhaus Ochoa <strong>Marbella</strong> und im Krankenhaus<br />
CERAM <strong>Marbella</strong> durchgeführt. Der Mediziner<br />
ist ein Kollege von Dr. med. Erik Schulten,<br />
Facharzt für Urologie und Andrologie, OTS Clinic<br />
<strong>Marbella</strong>.
