5.1 Eff ects of surface hydrophilicity and<br />
microtopography on early stages of soft and<br />
hard tissue integration at nonsubmerged<br />
titanium implants. An immunohistochemical<br />
study in dogs.<br />

The aim of the present study was to investigate the<br />
eff ects of surface hydrophilicity and microtopography<br />
on soft and hard tissue integration at nonsubmerged<br />
titanium implants. Implantation of conventional<br />
sand-blasted large grit and acid-etched (SLA) and<br />
chemically modifi ed SLA (modSLA) titanium implants<br />
with diff erently structured transmucosal surfaces (SLA<br />
implants: machined (M), or SLA; modSLA implants:<br />
modA, or modSLA) was performed bilaterally in both<br />
upper and lower jaws of 15 beagle dogs. The anim<strong>als</strong><br />
were sacrifi ced after 1, 4, 7, 14, and 28 days.<br />
Tissue reactions were assessed histomorphometrically<br />
and immunohistochemically (IH) using monoclonal<br />
antibodies to transglutaminase II (angiogenesis)<br />
and osteocalcin. While the junctional epithelium<br />
was commonly separated from M-SLA and SLA-SLA<br />
implants by a gap, the epithelial cells appeared to<br />
be in close contact to modA-modSLA surfaces after<br />
14 days of healing. Moreover, modA-modSLA and<br />
modSLA-modSLA groups revealed a well vascularized<br />
subepithelial connective tissue exhibiting collagen<br />
fi bres that have started to extend and attach partially<br />
perpendicular to the implant surface. Signifi cantly<br />
highest mean bone-to-implant contact areas were<br />
observed in the modA-modSLA and modSLA-modSLA<br />
groups at days 7, 14, and 28. Within the limits of this<br />
study, it might be concluded that both soft and hard<br />
tissue integration was mainly infl uenced by surface<br />
hydrophilicity, rather than by microtopography.<br />

5.2 Evaluierung von Insulin-like growth<br />
factor binding protein 2 <strong>als</strong> Tumormarker bei<br />
verschiedenen Tumorentitäten<br />

Hintergrund und Fragestellung: Die insulinähnlichen<br />
Wachstumsfaktoren (IGF I+II = insulin like growth<br />
factor I+II), die IGF-Rezeptoren (IGF I+II-Rez.) und<br />
die IGF-Bindungs-Proteine (IGFBPs) sind an der<br />
Proliferation und Diff erenzierung von Zellen beteiligt.<br />
In dieser Arbeit wurde die Bedeutung des IGF-<br />
Systems für neue Tumormarker bei verschiedenen<br />
soliden malignen Tumoren untersucht. Methodik.<br />
In dieser prospektiven Studie untersuchten wir<br />
am Anfang und nach Abschluss der Radiotherapie<br />
von 163 Tumorpatienten mit kolorektalem<br />
Karzinom (26), Prostatakarzinom (21), Glioblastom<br />
(12), Mammakarzinom (9), Zervixkarzinom (5),<br />
Endometriumkarzinom (1), Plattenepithelkarzinomen<br />
im Kopfh<strong>als</strong>-Bereich (17), Lymphomen (20),<br />
Bronchialkarzinom (<strong>34</strong>), Nierenzellkarzinom<br />
(3), Pankreaskarzinom (3), Blasenkarzinom (1),<br />
Malignem Melanom (5), Magenkarzinom (1) und<br />
Ösophaguskarzinom (5) sowie bei 13 gesunden<br />
Kontrollpersonen IGF-I, IGF-II, IGFBP-2, IGFBP-3<br />
(Radioimmunassay) und ALS-Spiegel im Serum.<br />
Ergebnis: Die bei Patienten mit soliden Tumoren<br />
gemessenen Serumspiegel der Parameter des<br />
IGF-Systems (IGF-I, IGF-II, IGFBP-2, IGFBP-3<br />
und ALS) weichen von den altersentsprechenden<br />
Referenzwerten gesunder Personen deutlich ab. IGF-I,<br />
IGF-II und IGFBP-2 Serumspiegel liegen insgesamt<br />
höher <strong>als</strong> die Referenzwerte gesunder Probanden.<br />
IGFBP-3 und ALS Serumspegel liegen dagegen<br />
niedriger <strong>als</strong> die Referenzwerte. Dieses Muster<br />
zeigte sich einheitlich für die einzeln ausgewerteten<br />
Tumortypen der HNO-, Prostata-, Rektum- und<br />
Bronchial-Karzinome. Bei der Einzelauswertung<br />
der Tumortypen Prostata-Ca, Rektum-Ca, HNO-<br />
Tumoren, Bronchial-Ca und Lymphome zeigten<br />
sich uneinheitliche und zum Teil gegensätzliche<br />
Abhängigkeiten der Serumspiegel der Parameter des<br />
IGF-Systems in Abhängigkeit von der Tumormasse,<br />
Außerdem fanden sich keine Unterschiede der Werte<br />
vor und nach Bestrahlung. Schlussfolgerung: Die<br />
Regulierung der Serumspiegel des IGF-Systems ist<br />
sehr komplex und hängt von vielen verschiedenen<br />
Faktoren ab. Sie ist bei den verschiedenen soliden<br />
Tumoren nicht einheitlich. Da es keine Abhängigkeit<br />
zur Tumormasse gibt, können sie zum gegenwärtigen<br />
Zeitpunkt bei den untersuchten Tumoren nicht <strong>als</strong><br />
Tumormarker verwendet werden.
