3. Retrieve. The PHO system documents the consenting process <strong>and</strong> process<br />

the data collection request, using the approving physician’s credentials to<br />

automatically retrieve records in batch on behalf of all approved patients<br />

from all hospital physicians who approved access.<br />

4. Access. The patient checks the status of submitted requests. He will find<br />

some requests get approved, some pending, while others get rejected with<br />

rejection reasons stated in the commenting fields. The retrieved records<br />

may be completely reorganized by the PHO system to improve patient selfeducation<br />

<strong>and</strong> ease of data interpretations.<br />

5. Annotate. The patient reviews his/her personal health charts, event logs,<br />

<strong>and</strong> <strong>medical</strong> treatment transcripts about themselves. He/She adds<br />

comments to the health log events for his/her own health record keeping.<br />

6. Share. The patients, after filtering private information, may choose to<br />

share some information online with others, including his/her peers <strong>and</strong><br />

physicians, to promote health self-education in his community.<br />

The benefits of this physician-mediated patient EMR review paradigm are<br />

potentially huge. The patients can quickly gain access to their EMR <strong>and</strong> integrate<br />

accessed records into their PHR immediately. The patients can also discover<br />

potential inaccuracies (such as personal information or current medications),<br />

which allows the hospitals to correct. The hospitals don’t need to worry about<br />

developing increasingly complex EMR system to yet another audience—individual
