access (See Fig. 5). In the example shown, patient Henry is still waiting for Dr.<br />

Anthony to approve his requests. Therefore, Henry sees the status “pending” while<br />

Dr. Anthony sees two options for Henry’s request “to Approve” or “to Reject”,<br />

along with approved request from another patient (John Smith) done earlier. If<br />

Henry’s request is rejected, the next time he checks back into the system, he<br />

would see the request status to go from “Pending” to “Reject”.<br />

5.3 Access Personal Health Charts<br />

After permission to retrieve his/her EMR from the hospital is granted by authorized<br />

physicians, patients can then access their personal health record. To show<br />

how the transformation was made to make the <strong>medical</strong> record easy to interpret, we<br />

show one of the many health events retrieved visually using charts. Fig. 6 below<br />

shows a pie chart of lab tests frequency distribution.<br />
