where formation of hematoma is a great problem. So it is advised to avoid block injections in these
patients.
Intra muscular injections should also be avoided because of chances of haematoma formation
ORAL SURGERY
Mandibular block injections are absolutely Contra indicated due to persistent haemorrhage into para
pharyngal tissues.
Local infiltration causes bleeding at each point
Safe site of local infiltration is down the periodontal membrane.
Haemorrhage plate may be given to protect the blood clot from trauma of food and tongue.
Atraumatic extraction should be done
Sides of Sockets should be gently squeezed.
It is better to avoid Suturing because of needle wound as well as blood may be directed to the facial
planes of neck.
Antifibrinolytics like EACA (Epsilon amino Caproic acid) and tranexamic acid may be used for
stabilization of clot.
Tranexamic acid is more potent and longer acting (IV and oral 500mg tab dissolved in, oral of water)
Avoid aspirin like drugs
Paracetamol or dihydrocodeine may be effective.
NSAIDs are to be avoided.
