NOK has signed. Also send a copy <strong>of</strong> all bills for interment costs to AFMAO/MA. The<br />

SF 1034 is <strong>the</strong> request to Finance for payment.<br />

1.12.2.3. When 27 SOCPTS returns <strong>the</strong> SF 1034, file it along with <strong>the</strong> bill in <strong>the</strong> case<br />

file. Record <strong>the</strong> amount on <strong>the</strong> DD Form 2063, Record <strong>of</strong> Preparation and Disposition <strong>of</strong><br />

Remains, amount allowed for that particular mortuary case.<br />

1.12.2.4. Forward a copy <strong>of</strong> <strong>the</strong> SF 1034 to AFMAO/MA. File <strong>the</strong> paid SF 1034 and DD<br />

Form 1375 in <strong>the</strong> case file according to <strong>the</strong> index.<br />

1.12.3. Transportation expenses. All transportation expenses from <strong>the</strong> place <strong>of</strong> death to <strong>the</strong><br />

place <strong>of</strong> burial are borne <strong>by</strong> <strong>the</strong> government.<br />

1.12.3.1. After <strong>the</strong> remains are moved from <strong>the</strong> local area to a receiving funeral home,<br />

type a letter to <strong>the</strong> funeral home requesting movement <strong>of</strong> remains and travel <strong>of</strong> an escort.<br />

1.12.3.2. The receiving funeral home will bill <strong>the</strong> MO for expenses incurred for<br />

transportation from <strong>the</strong> common carrier to <strong>the</strong> place <strong>of</strong> interment. Prepare a SF 1034 and<br />

a copy <strong>of</strong> <strong>the</strong> billing, and send <strong>the</strong>m to Finance. When Finance returns <strong>the</strong> paid SF 1034,<br />

annotate <strong>the</strong> amount on <strong>the</strong> DD Form 2063. Send a copy <strong>of</strong> <strong>the</strong> SF 1034 and a copy <strong>of</strong> <strong>the</strong><br />

bill to AFMAO/MA.<br />

1.12.4. O<strong>the</strong>r expenses:<br />

1.12.4.1. Use DSN or an authorized squadron long distance phone line, as appropriate, to<br />

communicate with <strong>the</strong> NOK.<br />

1.12.4.2. Clean all decedent’s clothing through <strong>the</strong> cleaning contract (for Blues only) or a<br />

local dry cleaner before shipping to <strong>the</strong> NOK. Clothing cannot be cleaned on a price per<br />

pound basis.<br />

1.12.5. Building <strong>the</strong> Finance Code: determine <strong>the</strong> category <strong>of</strong> each expense you are paying<br />

(AFI 34-242) and use <strong>the</strong> standard fund citation numbers used <strong>by</strong> Cannon AFB Finance.<br />

Numbers are provided <strong>by</strong> <strong>the</strong> squadron’s Resource Advisor (SQ/RA).<br />

1.13. Initial Counseling. Dependent deaths are generally referred to <strong>the</strong> Squadron Commander<br />

<strong>by</strong> <strong>the</strong> hospital. The Squadron Commander is also generally given prior warning <strong>of</strong> an impending<br />

death or serious injury.<br />

1.13.1. With due consideration for <strong>the</strong> feelings <strong>of</strong> surviving family members, obtain<br />

preliminary information about time <strong>of</strong> death, receiving funeral home, etc., using <strong>the</strong><br />

Deceased Dependent Worksheet (see Annex E, Appendix I).<br />

1.13.2. Explain that <strong>the</strong> government will only reimburse for transportation expenses. Make it<br />

clear that <strong>the</strong> sponsor incurs <strong>the</strong> expense first and <strong>the</strong>n <strong>the</strong> bill is paid or he/she is reimbursed.<br />

When <strong>the</strong> sponsor returns from <strong>the</strong> funeral home, he/she should bring five copies <strong>of</strong> all<br />

transportation bills/receipts.<br />

1.13.3. A certificate <strong>of</strong> dependency is required. Secure a fetal death report or regular death<br />

certificate, as applicable to state law. For stillborns or very young babies, <strong>the</strong> sponsor should<br />

call Records Section at <strong>the</strong> base hospital and request a copy <strong>of</strong> <strong>the</strong> AF Form 565, Record <strong>of</strong><br />

Impatient Treatment. For older children and spouses, he/she should call Records Section at<br />

MPF and ask for a copy <strong>of</strong> <strong>the</strong> DD Form 1172, Application for Uniformed Services<br />

Identification Card, or any pro<strong>of</strong> <strong>of</strong> dependency.
