Chapter 1<br />

MORTUARY AFFAIRS AND DISPOSITION<br />

1.1. Mortuary Case Files. The CAFB mortuary <strong>of</strong>ficer delegates <strong>the</strong> completion <strong>of</strong> a mortuary<br />

case file for all decedents (format at Attachment 15) to <strong>the</strong> Mortuary Office (27 SOFSS/FSOX).<br />

The mortuary file is an important document, which details all actions to take on a mortuary case.<br />

This case file is started as soon as possible and consists <strong>of</strong> <strong>the</strong> following items:<br />

1.1.1. Front Folder: The Summary Courts File (left side <strong>of</strong> first section) contains <strong>the</strong><br />

following as directed <strong>by</strong> <strong>the</strong> 27 SOFSS/CC:<br />

1.1.1.1. Tab 1 Order appointing SCO<br />

1.1.1.2. Tab 2 Chronological log <strong>of</strong> events<br />

1.1.1.3. Tab 3 Base bulletins<br />

1.1.1.4. Tab 4 All Correspondence<br />

1.1.1.5. Tab 5 AF Form 1122, Personal Property Inventory<br />

1.1.1.6. Tab 6 Shipping documents<br />

1.1.1.7. Tab 7 Any Staff Judge Advocate (SJA) opinions<br />

1.1.1.8. Tab 8 List <strong>of</strong> any property not shipped<br />

1.1.1.9. Tab 9 List <strong>of</strong> items destroyed<br />

1.1.2. Back Folder: The Case File (right side <strong>of</strong> second section) contains <strong>the</strong> following as<br />

directed <strong>by</strong> AFI 34-242:<br />

1.1.2.1. Tab 1 Form 57, Mortuary Guide.<br />

1.1.2.2. Tab 2 All message traffic.<br />

1.1.2.3. Tab 3 AF Form 970, Statement on Disposition <strong>of</strong> Remains, confirmation letter<br />

or DD Form 2065, Disposition <strong>of</strong> Remains, if OCONUS.<br />

1.1.2.4. Tab 4 All identification documents.<br />

1.1.2.5. Tab 5 DD Form 2063, Record <strong>of</strong> Preparation and Disposition <strong>of</strong> Remains<br />

(Within CONUS) or DD Form 2062, Record <strong>of</strong> Preparation and Disposition <strong>of</strong> Remains<br />

(Outside CONUS)<br />

1.1.2.6. Tab 6 AF Form 1947, Escort Report, and <strong>order</strong> appointing escort.<br />

1.1.2.7. Tab 7 Army or Navy forms similar to and used in lieu <strong>of</strong> <strong>the</strong> above.<br />

1.1.2.8. Tab 8 Copies <strong>of</strong> purchase and delivery <strong>order</strong>s and paid vouchers.<br />

1.1.2.9. Tab 9 All payment documents, forms and supporting documents (AF Form 969<br />

Request for Payment <strong>of</strong> Transportation Expenses for Deceased Dependent or Retiree, DD<br />

Form 1375, Request for Payment <strong>of</strong> Funeral and/or Interment Expenses).<br />

1.1.2.10. Tab 10 Travel <strong>order</strong>s, vouchers and supporting documents for persons<br />

authorized funeral travel.
