shipping case is used, this envelope will be delivered to <strong>the</strong> funeral director <strong>by</strong> <strong>the</strong> driver.<br />
d. You will hand carry <strong>the</strong> following documents and present <strong>the</strong>m to <strong>the</strong> NOK and assist<br />
<strong>the</strong>m in <strong>the</strong> completion <strong>of</strong> <strong>the</strong>se documents:<br />
(1) Request for Reimbursement <strong>of</strong> Interment Expenses (DD Form 1375). Upon<br />
completion make five copies and give one to <strong>the</strong> PADD. Original and three copies are<br />
returned to Mortuary Officer <strong>by</strong> mail or in person within two duty days <strong>of</strong> your return.<br />
(2) Request for additional death certificates or information concerning death certificates.<br />
Additional copies <strong>of</strong> DD Form 1300 (Report <strong>of</strong> Causality) can be obtained from Casualty<br />
Aff<strong>air</strong>s. Approximately ten copies <strong>of</strong> <strong>the</strong> DD Form 1300 are initially furnished to <strong>the</strong><br />
PADD. Mr. David Boiko ext. 4228.<br />
(3) Application for Headstone or Marker (VA Form 40-1330). If headstone or marker is<br />
desired, advise PADD that this form will be completed and both copies (original and<br />
duplicate) will be forwarded to <strong>the</strong> address on <strong>the</strong> VA Form 40-1330. (This form will be<br />
omitted when remains are consigned to a National Cemetery.) The Survivor Assistance<br />
Officer and/or funeral director may assist PADD in completing this form. The local<br />
Veterans Administration will also provide assistance.<br />
(4) Funeral home consignment letter to <strong>the</strong> receiving funeral director pertaining to<br />
transportation invoice.<br />
e. Secure tickets and Special Orders.<br />
f. Prior to departure and if traveling in same vehicle, check <strong>the</strong> shipping case <strong>by</strong> reading <strong>the</strong><br />
name on <strong>the</strong> envelope to assure you have <strong>the</strong> proper remains (reference paragraph 4c). Also,<br />
check <strong>the</strong> baggage form attached to <strong>the</strong> handle to insure proper destination.<br />
If remains are being transported in a hearse, <strong>the</strong> shipping case will be draped with a flag. The<br />
casket and <strong>air</strong> tray are not draped during <strong>the</strong> delivery to <strong>the</strong> <strong>air</strong>port <strong>air</strong> freight. The casket or<br />
<strong>air</strong> tray is draped on <strong>the</strong> removal from <strong>the</strong> <strong>air</strong>port to <strong>the</strong> funeral home.<br />
g. DO NOT BECOME SEPARTATED FROM THE REMAINS WHICH YOU ARE<br />
ESCORTING. Be particularly cautious when transfer is necessary.<br />
h. Upon arrival at destination <strong>air</strong>port and prior to <strong>the</strong> departure <strong>by</strong> hearse to funeral home,<br />
drape shipping container or casket, as applicable, with flag provided.<br />
i. Upon arrival at final destination, after inspection <strong>of</strong> remains <strong>by</strong> receiving funeral director,<br />
escort will again drape <strong>the</strong> flag (located on top <strong>of</strong> casket in shipping case) over <strong>the</strong> casket.<br />
(1) When <strong>the</strong> flag is used to drape a half-couch casket (Viewable Remains), it should be<br />
placed in three layers (10-inch folds) to cover <strong>the</strong> closed half <strong>of</strong> <strong>the</strong> casket with <strong>the</strong> union<br />
(blue field) on <strong>the</strong> deceased’s left.
