1.14. Financial Reimbursement.<br />

1.14.1. Payment must be made to <strong>the</strong> sponsor or funeral home, depending on <strong>the</strong> payment<br />

arrangement. Upon notification <strong>of</strong> a dependent death send a message within 16 hours to:<br />

1.14.2. NOK will sign completed AF Form 969, Request for Payment <strong>of</strong> Dependent<br />

Transportation Expenses, designating who will receive payment.<br />

1.14.3. Prepare a SF 1034 and attach <strong>the</strong> AF Form 969 and bill. Send to <strong>the</strong> Finance Office.<br />

1.14.4. When <strong>the</strong> SF 1034 is returned from finance, annotate <strong>the</strong> voucher number, and <strong>the</strong><br />

date <strong>of</strong> payment. Enter any remarks as appropriate. File <strong>the</strong> paid voucher in <strong>the</strong> mortuary<br />

case file.<br />

1.15. Dependent Case Files.<br />

1.15.1. Table <strong>of</strong> Contents.<br />

1.15.1.1. Tab 1 All Message Traffic.<br />

1.15.1.2. Tab 2 AF Form 969, confirmation letter, or DD Form 2065 if OCONUS.<br />

1.15.1.3. Tab 3 All identification documents.<br />

1.15.1.4. Tab 4 DD Form 2063 or 2062<br />

1.15.1.5. Tab 5 Department <strong>of</strong> <strong>the</strong> Army or Navy Forms if used.<br />

1.15.1.6. Tab 6 Payment documents, copies <strong>of</strong> purchase and delivery <strong>order</strong>s, paid<br />

vouchers and any o<strong>the</strong>r documents relating to expenses.<br />

1.15.1.7. Tab 7 AF Form 140, Creation Authorization and Disposition <strong>of</strong> Remains<br />

Request.<br />

1.15.1.8. Tab 8 DD Form 1300, DD Form 2064 and/or copy <strong>of</strong> state death certificate.<br />

1.15.1.9. Tab 9 Chronological log <strong>of</strong> events.<br />

1.15.1.10. Tab 10 Any o<strong>the</strong>r necessary documents. Contents <strong>of</strong> this tab will be listed in<br />

<strong>the</strong> Table <strong>of</strong> Contents.<br />

1.15.2. Files will be kept until all forms are received and forwarded to AFMAO. Copies <strong>of</strong><br />

<strong>the</strong> file will be maintained until 3 years after <strong>the</strong> date <strong>of</strong> death <strong>the</strong>n destroyed.<br />

1.16. Honor Guard Team Composition. Honors are given to all eligible service members<br />

IAW AFI 34-242, to <strong>the</strong> extent that manpower and circumstances allow. Full honors will always<br />

be given to deceased active duty members and ―Medal <strong>of</strong> Honor‖ recipients. The following<br />

procedures are specific and relevant only to Cannon AFB.<br />

1.16.1. Full military honors as determined <strong>by</strong> <strong>the</strong> 27 SOW/CC consists <strong>of</strong> a 20-person detail<br />

that includes six pallbearers, an eight person firing party (seven members and <strong>the</strong> firing party<br />

and NCOIC), a bugler, four person color guard and <strong>the</strong> detail OIC or NCOIC. An <strong>Air</strong> <strong>Force</strong><br />

chaplain may also participate in <strong>the</strong> ceremony if requested <strong>by</strong> <strong>the</strong> family.
