HIGH RISK<br />
Imminent Danger - Requires: IMMEDIATE Response<br />
1. Contact <strong>Crisis</strong> Response Team leader immediately.<br />
DO NOT LEAVE THE STUDENT UNATTENDED.<br />
2. Contact parents/guardians. If they don’t respond, call protective services<br />
<strong>and</strong> report as medical neglect.<br />
3. Call police <strong>and</strong> if required, <strong>and</strong> ambulance.<br />
4. Call community mental health resource person <strong>and</strong> request his/her<br />
immediate assistance.<br />
5. Complete Suicide Prevention Assessment Response (SPAR) Form <strong>and</strong><br />
prepare for follow up.<br />
MODERATE RISK<br />
Potentially Dangerous- Requires: Thorough Evaluation/Referral<br />
LOW RISK<br />
1. Contact crisis team leader.<br />
2. Obtain phone consultation from identified community mental health person.<br />
2. Contact parents <strong>and</strong> request parents to pick up the student. Provide<br />
referral options.<br />
3. Complete SPAR Form.<br />
Requires: Short Term Monitoring as Needed<br />
1. Contact crisis team leader.<br />
2. Complete SPAR form within 24 hours.<br />
3. If warning signs persist, set up an appointment to see student the<br />
following day.<br />
4. Contact parents <strong>and</strong> set up a conference.<br />
