Applications for licensure may be obtained online from the Florida Board <strong>of</strong> <strong>Pharmacy</strong>. Applications<br />

for the board exam <strong>of</strong> any other state may be obtained in writing from the respective state board <strong>of</strong><br />

pharmacy. The college will certify to the board the candidate’s graduation, as well as internship hours<br />

completed during experiential courses. This documentation will not be provided by the Office <strong>of</strong> the<br />

Dean until after degree conferral.<br />

STuDenT ConDuCT<br />

Conduct<br />

<strong>Student</strong>s are expected to conduct themselves in a responsible manner, which will reflect well on<br />

themselves, the college, and the university, in terms <strong>of</strong> morality, honor, truth, and good citizenship.<br />

<strong>Student</strong>s are also expected to abide by the regulations <strong>of</strong> the division and the university.<br />

Furthermore, students are expected to conduct themselves in a pr<strong>of</strong>essionally ethical fashion and<br />

to maintain and observe high standards <strong>of</strong> conduct so that the integrity <strong>of</strong> the university may be<br />

preserved. A student should avoid impropriety and the appearance <strong>of</strong> impropriety in all activities.<br />

Attendance<br />

At <strong>Nova</strong> <strong>Southeastern</strong> University Health Pr<strong>of</strong>essions Division, attendance at all scheduled<br />

instructional periods is mandatory. Failure to fulfill this requirement is considered in the evaluation<br />

<strong>of</strong> a student’s academic performance and pr<strong>of</strong>essional attitude and may result in a failing grade for<br />

the course and/or other disciplinary actions. Requests for excused absences must be submitted to the<br />

Office <strong>of</strong> <strong>Student</strong> Services within 24 hours <strong>of</strong> the occurrence. <strong>Student</strong>s are also required to notify<br />

course coordinators/course facilitators if they will be absent from classes within 24 hours following<br />

the absence.<br />

<strong>Student</strong>s whose reasons are unacceptable will be subject to disciplinary action. In the event <strong>of</strong> an<br />

emergency absence, the request for an excused absence must be made to the course coordinator/<br />

facilitator or the Office <strong>of</strong> <strong>Student</strong> Services or designee.<br />

Excused Absences<br />

Illness—for absences due to illness, the course coordinator, the course facilitator, or the Office <strong>of</strong><br />

<strong>Student</strong> Services must be notified as soon as possible. Absences will be evaluated on an individual<br />

basis. Extended absences (i.e., three consecutive days or more) or absences on test/exam or lab days<br />

will require the submission <strong>of</strong> a doctor’s note, or other supporting documentation.<br />

Religious holidays—absences for major religious and ethnic holidays may be excused at the discretion<br />

<strong>of</strong> the course coordinator or the Office <strong>of</strong> <strong>Student</strong> Services. <strong>Student</strong>s are required to obtain approval<br />

for their absences one week prior to the holiday.<br />

Special circumstances—unusual circumstances resulting in absences—e.g., death in the immediate<br />

family—must be cleared with the course coordinator or the Office <strong>of</strong> <strong>Student</strong> Services on an<br />

individual basis, preferably before the student is absent from class.<br />

Excused absences do not entitle students to have the opportunity to make up in-class quizzes or<br />

assignments. Assignments and examinations missed due to excused absences will be made up at the<br />

discretion and convenience <strong>of</strong> the course coordinator. Each laboratory assignment or examination<br />

missed may be made up at the discretion and convenience <strong>of</strong> the instructor. Procedures for absences<br />

are outlined in individual course syllabi. If, in the judgment <strong>of</strong> the dean, or program director if<br />
