3. the exceeding <strong>of</strong> the seven-year limit for completing all graduation requirements, exclusive <strong>of</strong> any<br />
approved leave <strong>of</strong> absence in good standing<br />
4. circumstances <strong>of</strong> a legal, moral, behavioral, ethical, or academic nature that warrant such action<br />
5. determination by the dean that there are factors that would interfere with or prevent the student<br />
from practicing the highest quality <strong>of</strong> pharmacy<br />
Readmission Policy<br />
If a student is dismissed or withdraws from the <strong>College</strong> <strong>of</strong> <strong>Pharmacy</strong>, he or she may not apply for<br />
admission for a class starting less than 12 calendar months after the date <strong>of</strong> dismissal or withdrawal.<br />
He or she may only apply as a first-year student. In order to be considered for admission, the candidate<br />
must provide adequate evidence that the conditions and/or factors that caused the prior dismissal or<br />
withdrawal have changed significantly, so that there is reasonable expectation that the applicant can<br />
perform satisfactorily if admitted.<br />
Any applicant applying for admission must do so through the Office <strong>of</strong> Admissions <strong>of</strong> the<br />
Health Pr<strong>of</strong>essions Divisions and must follow the same procedure as any other entering student.<br />
Admission will be solely at the discretion <strong>of</strong> the dean as are all admissions. The student’s prior<br />
academic records will remain part <strong>of</strong> his or her overall academic record and will be recorded on<br />
the permanent transcript. If admitted, none <strong>of</strong> the student’s prior grades will be used in calculating<br />
his or her new grade point average.<br />

Any student has the right to seek redress <strong>of</strong> a grievance with immunity from disciplinary action or<br />
retaliation without regard as to the sex, race, religion, disability, color, creed, ethnic, or national origin as<br />
included in the regulations <strong>of</strong> Title VI, Title IX, the Americans with Disabilities Act, and section 504 <strong>of</strong> the<br />
Rehabilitation Act. For a student to address a grievance, he or she must use the following procedures:<br />
1. The student will present the grievance to the faculty member. If the faculty member cannot affect<br />
a resolution to the problem, the student may then consult with the chair <strong>of</strong> the department or<br />
program director. If the chair/program director cannot affect a resolution, the student may then<br />
consult with the assistant dean for experiential programs and student services or the director <strong>of</strong><br />
student services.<br />
2. The assistant dean for experiential programs and student services or the director <strong>of</strong> student services<br />
will hear the grievance. If a resolution cannot be affected, the student may consult with the<br />
executive associate dean.<br />
3. If a satisfactory solution cannot be achieved, the student will be advised to prepare a written,<br />
signed request to the chair <strong>of</strong> the <strong>Student</strong> Progress Committee, setting forth the grievance and<br />
requesting a hearing with the <strong>Student</strong> Progress Committee.<br />
4. Copies <strong>of</strong> appropriate and relevant documentation must be appended to this request, which will<br />
include a statement to redress the student requests.<br />
5. The chair <strong>of</strong> the SPC shall convene the committee after receipt <strong>of</strong> a written request.<br />
6. The student will be notified in advance <strong>of</strong> the date, time, and place <strong>of</strong> the meeting.<br />
