<strong>Nova</strong> <strong>Southeastern</strong> University hereby designates the following student information as public or directory<br />

information. Such information may be disclosed by the institution for any purpose, at its discretion.<br />

• student name<br />

• local and home address<br />

• telephone numbers<br />

• major field <strong>of</strong> study<br />

• participation in sports<br />

• place <strong>of</strong> birth<br />

• dates <strong>of</strong> attendance<br />

• degrees, honors, and awards received<br />

• enrollment status<br />

• year in school<br />

• anticipated graduation date<br />

• email address<br />

Release <strong>of</strong> <strong>Student</strong> Information<br />

A student can give consent to permit <strong>Nova</strong> <strong>Southeastern</strong> University to discuss and/or release<br />

personal identifiable information to a third party such as a spouse, a parent, a guardian, etc. This<br />

consent must be provided in writing with the student’s signature. To provide a written consent,<br />

complete the Authorization for Release <strong>of</strong> Information form.<br />

A student may also withhold directory information (as defined above) by completing the Request to<br />

Prevent Disclosure <strong>of</strong> Directory Information form. A student is warned, however, that some <strong>of</strong> the<br />

consequences <strong>of</strong> preventing disclosure <strong>of</strong> directory information may be undesirable: a student’s name<br />

will not be published on the Dean’s List or commencement bulletins, and requests from prospective<br />

employers are denied.<br />

Deceased <strong>Student</strong> Records<br />

Records <strong>of</strong> a deceased student will be made available to the parent(s), spouse, or executor/executrix<br />

<strong>of</strong> the deceased student and other authorized parties upon written request. The request must include<br />

the need for the records; must identify the requestor’s relationship to the deceased student; and must<br />

be accompanied with an <strong>of</strong>ficial record certifying authorization to receive the student records, e.g.,<br />

assignment as executor/executrix. An <strong>of</strong>ficial copy <strong>of</strong> the death certificate must accompany the<br />

request, if the university does not have prior notice <strong>of</strong> the student’s death. The university reserves the<br />

right to deny the request. For further assistance on this matter, students should contact the University<br />

Registrar’s Office.<br />

Health Care Privacy (HIPAA) Statement<br />

The Health Insurance Portability and Accountability Act <strong>of</strong> 1996 (HIPAA) requires health<br />

care providers to abide by the regulations for privacy and confidentiality <strong>of</strong> protected health<br />

information (PHI). The HIPAA privacy rule covers all individually identifiable health<br />

information at <strong>Nova</strong> <strong>Southeastern</strong> University health care centers whether the information is<br />

electronic, paper, or spoken.<br />
