Experiential Grade<br />
A student seeking to appeal a decision regarding an experiential grade should seek solutions through<br />
the following administrative channels by entering at the appropriate level and proceeding in the<br />
order stated:<br />
1. experiential preceptor<br />
2. site director <strong>of</strong> experiential programs<br />
3. assistant dean for experiential and student services<br />
4. executive associate dean, pr<strong>of</strong>essional program (final level <strong>of</strong> appeal)<br />
A student seeking to resolve a grade problem through the administrative channels above must<br />
initiate such action in writing within 30 days from the date the grade is recorded at the registrar’s<br />
<strong>of</strong>fice. Review <strong>of</strong> a student problem and complaint at each administrative level will be carried out<br />
as expediently as possible. If the student is not satisfied with the decision, he or she may appeal<br />
to the next administrative level. If the student chooses to continue to appeal, this must be done<br />
in writing within 10 business days <strong>of</strong> the date the decision was rendered, excluding weekends and<br />
<strong>of</strong>ficial school holidays. No administrative grade changes will be accepted 60 days after the grade<br />
is recorded (unless a grade review is still active).<br />
Appeals Board<br />
1. Appeals Board Hearing Guidelines<br />
The student appeal hearing is an informal proceeding. No rules <strong>of</strong> evidence will be used. The hearing<br />
shall be internal, private, and closed to nonuniversity persons. Nonuniversity personnel are not<br />
available for consultation during these hearings. All procedures and actions are aimed to safeguard<br />
and preserve the educational and developmental mission <strong>of</strong> the <strong>College</strong> <strong>of</strong> <strong>Pharmacy</strong>.<br />
2. Appeals Board Responsibilities<br />
The Appeals Board will hear all student appeals <strong>of</strong> decisions made by the dean <strong>of</strong> COP relating<br />
to either dismissal or disciplinary actions as defined in the <strong>Student</strong> <strong>Handbook</strong>. If a student appeals<br />
the decision <strong>of</strong> the dean, the appeal must be in writing and submitted to the chair <strong>of</strong> the Appeals<br />
Board within 10 business days (excluding holidays and weekends) after the date <strong>of</strong> receiving<br />
notification <strong>of</strong> the dean’s decision. Any appeals not submitted to the chair <strong>of</strong> the Appeals Board<br />
within this time frame shall not be heard. The appeal must contain a concise statement <strong>of</strong> all<br />
relevant facts and the result sought.<br />
3. Appeals Board Membership<br />
The Appeals Board shall consist <strong>of</strong> the chancellor <strong>of</strong> the Health Pr<strong>of</strong>essions Division or designee<br />
and four to five faculty members. One member will be designated as the chair <strong>of</strong> the board by the<br />
dean <strong>of</strong> the <strong>College</strong> <strong>of</strong> <strong>Pharmacy</strong>. The executive associate dean, the chair <strong>of</strong> the <strong>Student</strong> Progress<br />
Committee, and the assistant dean for experiential programs and student services and/or the director<br />
<strong>of</strong> student services will attend the hearing, but will not participate during the vote <strong>of</strong> the board’s<br />
decision. The chair may request representatives from distant sites when appropriate.<br />
