5. MMR (Measles, Mumps, Rubella)<br />
• Two doses <strong>of</strong> the vaccine may be given instead <strong>of</strong> individual immunizations.<br />
• One dose <strong>of</strong> the MMR vaccine on or after the first birthday, the second dose<br />
must be at least 30 days after the first, and both must be in 1968 or later<br />
Exemptions or waivers may be obtained at the university’s discretion if the individual is 18 years<br />
<strong>of</strong> age or older, or the individual’s parent, if the individual is a minor, declines the vaccinations<br />
by signing a separate waiver provided by the institution for each <strong>of</strong> these vaccines, acknowledging<br />
receipt and review <strong>of</strong> the information provided. Exemptions will not necessarily be accepted by the<br />
student’s program center. Other students may be subject to the immunization requirements in order<br />
to participate in clinical or practicum studies. <strong>Student</strong>s are advised to consult with their program to<br />
determine whether the refusal will affect their ability to continue their studies.<br />
Immunization requirements are based on Florida state health regulations. Valid exemptions from<br />
providing immunity:<br />
• Medical exemptions—Must produce a signed letter from a doctor, on his / her stationery, stating the<br />
reason for exemption and whether it is a temporary or permanent exemption<br />
• Religious exemptions—On church stationery, signed by a minister, priest, rabbi, or head<br />
<strong>of</strong> church<br />
Acceptable forms <strong>of</strong> documentation—The following documents are acceptable pro<strong>of</strong> <strong>of</strong> immunity <strong>of</strong><br />
measles and rubella. Forms must include specific dates, and the dates must satisfy the requirements<br />
stated previously.<br />
• HRS (Department <strong>of</strong> Health and Rehabilitative Services)<br />
• Childhood immunization records<br />
• School immunization records<br />
• Military service records<br />
• Document indicating blood tests<br />
Health Insurance<br />
<strong>Nova</strong> <strong>Southeastern</strong> University requires all undergraduate day students, residential on-campus<br />
students, and Health Pr<strong>of</strong>essions Division (HPD) students to have some form <strong>of</strong> health insurance.<br />
Federal visa regulations also require international J-1 students and their J-2 dependents to maintain<br />
adequate health insurance throughout their stay in the United States.<br />
If the above paragraph applies to you, your NSU account will be charged for the health insurance<br />
premium. If you currently maintain comparable coverage that is greater or equal to that <strong>of</strong> NSU’s<br />
policy, you must complete the online waiver form, available on the student medical center Web<br />
site (www.nova.edu/smc) under “Health Insurance.”<br />

Hurricane Procedures<br />
A hurricane watch is a governmental agency announcement issued for an area when there is a threat<br />
<strong>of</strong> hurricane conditions, generally to strike within 36 hours.<br />
A hurricane warning is an announcement issued for an area when hurricane conditions are expected<br />
to strike within 24 hours.<br />
