by one of the following documents:
• Medical Leave—This is a letter from the student’s licensed health care provider documenting
A student on medical leave may be asked to provide a licensed health care provider’s evaluation
matriculation.
If a student is granted a leave of absence while current coursework is still in progress, he or she will be
withdrawn from those courses. In all such cases, an appropriate designation for each course in progress
will be entered on the transcript. A W will be given if no graded coursework has been completed or if
graded coursework is at a passing level. A WF will be given if graded coursework has been completed
of an emergency, the student must leave a message with the Office of Student Services and inform
