progress, or for documented medical reasons. A student requesting a leave <strong>of</strong> absence for medical or<br />
personal reasons must submit a written request to the dean’s <strong>of</strong>fice. The request must be accompanied<br />
by one <strong>of</strong> the following documents:<br />
• Medical Leave—This is a letter from the student’s licensed health care provider documenting<br />
the student’s condition and assessing when he or she can return to active academic status. The<br />
purpose <strong>of</strong> medical leave is to allow a student to take time away from his or her studies to obtain<br />
medical care. Leave cannot be requested retroactively.<br />
• Personal Leave—This is a personal statement from the student documenting the reason why a<br />
personal leave is needed.<br />
A student on medical leave may be asked to provide a licensed health care provider’s evaluation<br />
before being allowed to resume studies.<br />
A student on personal leave may be asked to provide a personal statement before being allowed to<br />
resume studies. If a personal leave is granted for financial reasons, the student may be asked to provide<br />
the financial departments <strong>of</strong> the university with pro<strong>of</strong> that he or she has the financial capability to<br />
continue his or her studies.<br />
A leave <strong>of</strong> absence will not be granted in excess <strong>of</strong> two semesters (one calendar year) over the course<br />
<strong>of</strong> a student’s degree program, either cumulatively or within a single leave, during the student’s<br />
matriculation.<br />
If a student is granted a leave <strong>of</strong> absence while current coursework is still in progress, he or she will be<br />
withdrawn from those courses. In all such cases, an appropriate designation for each course in progress<br />
will be entered on the transcript. A W will be given if no graded coursework has been completed or if<br />
graded coursework is at a passing level. A WF will be given if graded coursework has been completed<br />
and is not at a passing level.<br />
A student taking a leave <strong>of</strong> absence also foregoes any tuition refund, if applicable.<br />
Attending Class at a Different Site<br />
<strong>Student</strong>s must have prior approval from the Office <strong>of</strong> <strong>Student</strong> Services before attending class at a<br />
different site than that at which they are registered. The Office <strong>of</strong> <strong>Student</strong> Services at both sites<br />
and the course coordinator must be informed at least 24 hours in advance <strong>of</strong> the change. In case<br />
<strong>of</strong> an emergency, the student must leave a message with the Office <strong>of</strong> <strong>Student</strong> Services and inform<br />
the course coordinator/facilitator that he or she is attending class at an alternate site.<br />
Dress Code<br />
<strong>Student</strong>s in the Health Pr<strong>of</strong>essions Division must maintain a neat and clean appearance<br />
befitting students attending pr<strong>of</strong>essional school. Therefore, attire should convey a pr<strong>of</strong>essional<br />
appearance whenever the student is on the division campus, at an NSU student educational<br />
center or distance site, in classes or laboratories, or on a pharmacy practice experience. <strong>Student</strong>s<br />
must adhere to the HPD dress code found in the HPD section <strong>of</strong> this handbook. The following<br />
constitutes acceptable attire:<br />
• shirt, tie, slacks, and regular shoes for men, and for women it should be pr<strong>of</strong>essional business dress,<br />
which includes slacks, pants, or skirt with blouse, or dress and appropriate shoes<br />
• matching scrub sets and shoes<br />
In addition to the above attire, students must wear their white clinical jacket at all times while on<br />
campus or at experiential sites.<br />
