month following <strong>the</strong> signing of this <strong>Agreement</strong>.<br />

Seasonal Employees: $10,000.00<br />

Any permanent employee who retires early using any early retirement options<br />

offered by OMERS and those employees who retire at normal retirement age, or<br />

earlier because of health reasons approved by a qualified medical practitioner:<br />

$10,000 up to <strong>the</strong> end of <strong>the</strong> month in which <strong>the</strong> retiree reached <strong>the</strong> age of 70.<br />

The A.D.& D. Provision shall not apply to retired employees.<br />

Supplemental Hospital (semi-private)<br />

Pay Direct Drug Card program with a dispensing fee limitation of $5.00 per<br />

script, including positive enrolment and coordination of benefits.<br />

Extended Health Care Plan $10 - $20 deductible, based on <strong>the</strong> 1984 Excelsior<br />

Live Plan with <strong>the</strong> Corporation, or an equivalent, and $350 per person every two<br />

(2) years for eye wear, no deductible, $50 per person for eye examinations every<br />

two (2) years, and $500 per person per year for hearing aids.<br />

Licensed psychologists, osteopaths, chiropodists, naturopaths, podiatrists<br />

maximum $225 per dependent per year and $350 per employee per year.<br />

Licensed physio<strong>the</strong>rapists, chiropractors or masseurs maximum $275 per<br />

dependent per year and $400 per employee per year.<br />

Effective January 1, 2012, licensed psychologists, osteopaths, chiropodists,<br />

naturopaths, podiatrists, physio<strong>the</strong>rapists, chiropractors or masseurs<br />

maximum $500 per employee per year. Licensed psychologists, osteopaths,<br />

chiropodists, naturopaths, podiatrists maximum $225 per dependent per<br />

year. Licensed physio<strong>the</strong>rapists, chiropractors or masseurs maximum $275<br />

per dependent per year.<br />

Out-of-Country Medical Benefit (All Active Permanent Employees)<br />

The following services outside <strong>the</strong> person’s Province of residence for<br />

emergencies:<br />

(i) room and board in a hospital up to <strong>the</strong> hospital’s ward rate (including<br />

where permitted by law, any admittance, coinsurance, or utilization<br />

charges.)<br />

(ii) o<strong>the</strong>r hospital services (provided out of <strong>Canada</strong>).<br />

(iii) out-patient services in a hospital<br />

(iv) services of a Doctor.
