25.13 Each employee shall be supplied with a report of <strong>the</strong> balance of his sick leave credits on<br />
record.<br />
Article 26 - Pensions<br />
26.01 The Corporation and <strong>the</strong> employees shall make contributions in accordance with <strong>the</strong><br />
provisions of <strong>the</strong> <strong>Ontario</strong> Municipal Employees Retirement System and <strong>the</strong> <strong>Canada</strong><br />
Pension Plan.<br />
26.02 The Corporation shall supply to <strong>the</strong> Union, annually upon request, information<br />
concerning <strong>the</strong> rights under <strong>the</strong> pension plans of members who are employees of <strong>the</strong><br />
Corporation.<br />
26.03 Any amendments to <strong>the</strong> aforementioned pension plans causing reduction in pension<br />
benefits shall be cause for negotiations with <strong>the</strong> Union in accordance with <strong>the</strong> provisions<br />
of Article 7.02 and 7.03.<br />
Article 27 - Workers Compensation<br />
27.01 An employee covered by this <strong>Agreement</strong> who suffers an injury while in <strong>the</strong> course of his<br />
duties, and who is in receipt of a wage or salary award from <strong>the</strong> Workplace Safety<br />
Insurance Board, shall be paid by <strong>the</strong> Corporation for a period not to exceed one (1)<br />
month for each separate injury, <strong>the</strong> difference between <strong>the</strong> said wage or salary award and<br />
his current wages or salary.<br />
For absences in excess on one (1) month, <strong>the</strong> injured employee shall be paid directly by<br />
<strong>the</strong> Workplace Safety Insurance Board.<br />
27.02 An employee shall provide a completed WSIB Functional Abilities Form (FAF)<br />
certifying that <strong>the</strong>y are fit to return to perform <strong>the</strong> essential duties of <strong>the</strong>ir position.<br />
27.03 All workplace accidents shall be reported to <strong>the</strong> Union Secretary in writing along with<br />
a copy of <strong>the</strong> Form 7 that is filed with <strong>the</strong> Workplace Safety Insurance Board.<br />
27.04 If <strong>the</strong> employee is not fit to perform <strong>the</strong> essential duties of <strong>the</strong>ir pre-injury position<br />
but is able to perform modified work, <strong>the</strong> employee and employer shall jointly<br />
participate with <strong>the</strong> development of a modified work plan in accordance with <strong>the</strong><br />
WSIB Early and Safe Return to Work program.<br />
27.05 All workplace incidents/accidents shall be reported monthly to <strong>the</strong> appropriate<br />
JHSC and a copy sent to <strong>the</strong> Union Secretary.
